Transarterielle Chemoembolisierung (TACE) ist die Standardtherapie für Patienten im Zwischenstadium des hepatozellulären Karzinoms und wird in der Regel durch Femoralarterienzugang durchgeführt. Im Vergleich zum transfemoralen Zugang kann der transradiale Zugang (TRA) die Rate der Blutungskomplikationen verringern und die Patiententoleranz verbessern. Hier wird eine Methode zur transarteriellen Chemoembolisierung über radialen Arterienzugriff vorgestellt.
Transarterielle Chemoembolisierung (TACE) ist die häufigste Modalität zur Behandlung des hepatozellulären Karzinoms (HCC) im Zwischenstadium. TACE wird in der Regel über transfemoralen Zugang (TFA) durchgeführt. Der transradiale Zugang (TRA) wird jedoch bei koronaren Arterieninterventionen aufgrund verminderter Komplikationen und Mortalität bevorzugt. Ob die Vorteile von TRA auf TACE angewendet werden können, musste untersucht werden.
Patienten, die TRA TACE in einem einzigen Zentrum erhielten, wurden rückwirkend für die Studie eingeschrieben. Verfahrensdetails, technischer Erfolg, radiale Arterienverschlussrate (RAO) und Zugriffs-Standort-bedingte Blutungskomplikationen wurden bewertet. Von Oktober 2017 bis Oktober 2018 wurden 112 Patienten 160 TRA TACE-Verfahren unterzogen. Die technische Gesamterfolgsquote betrug 95,0% (152/160). Die Frequenz weiche Frequenz von TRA zu TFA betrug 1,9%. In keinem Fall wurden durch blutungsbedingte Komplikationen im Zusammenhang mit dem Zugriff auf die Seite festgestellt. Asymptomatische RA-Okklusion trat bei drei Patienten auf (2,7%). Im Vergleich zu TFA kann TRA die Sicherheit und Patientenzufriedenheit erhöhen und gleichzeitig die durch Denerhinaus bedingten Blutungskomplikationen verringern. Darüber hinaus können TRA-Interventionen Patienten mit fortgeschrittenem Alter, Adipositas oder einem hohen Risiko für Blutungskomplikationen zugute kommen.
Hepatozelluläres Karzinom (HCC) ist eine sehr häufige Malignität, mit der sechsthöchsten Inzidenzrate weltweit. Es ist auch die zweithäufigste Ursache für Krebssterblichkeit auf der ganzen Welt1. Da nur 5%–20% der Patienten eine kurative Therapie erhalten können, ist die transarterielle Chemoembolisierung (TACE) die beliebteste palliative Behandlung für Patienten mit nicht resezierbarem HCC2. TACE wurde als der am häufigsten verwendete und effektivste Behandlungsansatz für HCC-Patienten in der Zwischenstufe3anerkannt. Transfemoral access (TFA) Chemoembolisierung ist der häufigste Ansatz für TACE4. Es gibt jedoch Risiken im Zusammenhang mit TFA-Interventionen, einschließlich Blutungen an der Zugangsstelle und größere vaskuläre Komplikationen5. Diese Komplikationen führen zu einem längeren Krankenhausaufenthalt und erhöhten Kosten. Darüber hinaus erfordert TFA eine Immobilisierung für mindestens 6 h, was das Unbehagen und die Unzufriedenheit der Patienten erhöht.
Transradial access (TRA) ist ein alternativer Ansatz, der seit mehr als zwei Jahrzehnten in der perkutanen koronaren Intervention (PCI) verwendet wird5,6. TRA PCI hat mehrere Vorteile: erhöhter Prozesskomfort, verminderte Zugangsblutungen, verminderte größere vaskuläre Komplikationen und verringerte Sterblichkeit7,8. Die radiale Arterie (RA) ist aufgrund ihrer oberflächlichen Lage7leicht zugänglich und punktioniert. Hemostasis ist nach interventioneinfach durchzuführen und es gibt keine strenge Immoblisierung9. Trotz ermutigender Beweise für TRA-Interventionen bei der Herzkatheterisierung wurden bisher nur wenige Studien TRA bei peripheren Krankheitsinterventionen verwendet. TRA-Interventionen bei bösartigen Lebertumoren sind noch seltener. Hierwird die klinische Machbarkeit und Sicherheit der TRA-Leberembolisierung analysiert. Die Erfahrungen einer Institution mit dem bereitgestellten Schritt-für-Schritt-TRA-Protokoll werden ebenfalls beschrieben.
Die interventionelle TRA-Therapie hat in den letzten Jahren weltweit deutlich zugenommen, insbesondere in diagnostischen und interventionellen kardiologischen Verfahren12. Darüber hinaus wurde die Aufmerksamkeit zunehmend auf periphere Gefäßerkrankungen aufmerksam. Ohne Kompromisse bei den verfahrenstechnischen Erfolgsraten kann TRA-zu-Herz-Intervention effektiv die Raten von Blutungen und Gefäßkomplikationen im Vergleich zu TFA13,14r…
