Transarteriële chemoembolisatie (TACE) is de standaardtherapie voor patiënten in het tussenstadium van hepatocellulair carcinoom en wordt meestal uitgevoerd via femurslagadertoegang. In vergelijking met transfemorale toegang kan transradiale toegang (TRA) de snelheid van bloedingscomplicaties verminderen en de tolerantie van de patiënt verbeteren. Hier wordt een methode gepresenteerd om transarteriële chemoembolisatie uit te voeren via radiale slagadertoegang.
Transarteriële chemoembolisatie (TACE) is de meest voorkomende modaliteit voor de behandeling van hepatocellulair carcinoom (HCC) in het tussenstadium. TACE wordt meestal uitgevoerd via transfemorale toegang (TFA). Transradiale toegang (TRA) heeft echter de voorkeur bij coronaire slagaderinterventies als gevolg van verminderde complicaties en mortaliteit. Of de voordelen van TRA kunnen worden toegepast op het vereiste onderzoek van TACE.
Patiënten die TRA TACE in één centrum kregen, werden met terugwerkende kracht ingeschreven voor onderzoek. Procedurele details, technisch succes, radiale slagader occlusie (RAO) snelheid, en toegang site-gerelateerde bloeding complicaties werden geëvalueerd. Van oktober 2017 tot oktober 2018 ondergingen 112 patiënten 160 TRA TACE-procedures. Het totale technische slagingspercentage bedroeg 95,0% (152/160). De oversteek van TRA naar TFA bedroeg 1,9%. Er werden in geen enkel geval toegangsplaatsgerelateerde bloedingscomplicaties gevonden. Asymptomatische RA occlusie trad op bij drie patiënten (2,7%). In vergelijking met TFA kan TRA de veiligheid en patiënttevredenheid verhogen en tegelijkertijd de complicaties van de toegangsplaatsgerelateerde bloedingen verminderen. Bovendien kunnen TRA-interventies patiënten met gevorderde leeftijd, obesitas of een hoog risico op bloedingscomplicaties ten goede komen.
Hepatocellulair carcinoom (HCC) is een veel voorkomende maligniteit, met de zesde hoogste incidentie wereldwijd. Het is ook de tweede belangrijkste oorzaak van kankersterfte over de hele wereld1. Omdat slechts 5%-20% van de patiënten curatieve therapie kan krijgen, is transarteriële chemoembolisatie (TACE) de meest populaire palliatieve behandeling voor patiënten met niet-reseceerbare HCC2. TACE is erkend als de meest gebruikte en effectieve behandelingsbenadering voor HCC-patiënten in het tussenstadium3. Transfemorale toegang (TFA) chemoembolisatie is de meest voorkomende aanpak voor TACE4. Er zijn echter risico’s verbonden aan TFA-interventie, waaronder bloedingen op de toegangslocatie en ernstige vasculairecomplicaties 5. Deze complicaties leiden tot langdurige ziekenhuisopname en hogere kosten. Bovendien vereist TFA immobilisatie gedurende ten minste 6 uur, wat ongemak en ontevredenheid voor de patiënten verhoogt.
Transradiale toegang (TRA) is een alternatieve benadering die al meer dan twee decennia wordt gebruikt bij percutane coronaire interventie (PCI)5,6. TRA PCI heeft verschillende voordelen: verhoogd procedurecomfort, verminderde toegang site-gerelateerde bloedingen, verminderde ernstige vasculaire complicaties, en verminderdemortaliteit 7,8. De radiale slagader (RA) is gemakkelijk toegankelijk en doorboord vanwege de oppervlakkige locatie7. Hemostase is gemakkelijk uit te voeren na interventie en er is geen strikte immoblisatie9. Ondanks bemoedigend bewijs voor TRA-interventie bij cardiale katheterisatie, gebruikten tot op heden slechts enkele studies TRA bij perifere ziekteinterventie. TRA-interventies voor kwaadaardige levertumoren zijn nog zeldzamer. Hier wordt de klinische haalbaarheid en veiligheid van TRA-leverembolie geanalyseerd. De ervaring van één instelling met het stapsgewijze TRA-protocol wordt ook beschreven.
Tra interventionele therapie is de afgelopen jaren wereldwijd aanzienlijk gegroeid, met name in diagnostische en interventionele cardiologische procedures12. Bovendien is er steeds meer aandacht voor perifere vasculaire ziekte-interventie. Zonder de procedurele slagingspercentages in gevaar te brengen, kan TRA naar hartinterventie de bloedings – en vasculaire complicaties effectief verminderen in vergelijking met TFA13,14.
