La chimioembolisation transartérale (TACE) est la thérapie standard pour des patients dans le stade intermédiaire du carcinome hépatocellulaire et est typiquement exécutée par l’accès fémoral d’artère. Par rapport à l’accès transfémoral, l’accès transradial (TRA) peut diminuer le taux de complications hémorragiques et améliorer la tolérance des patients. Une méthode est présentée ici pour effectuer la chimioembolisation transartérale par l’intermédiaire de l’accès radial d’artère.
La chimioembolisation transartérale (TACE) est la modalité la plus commune pour le traitement du carcinome hépatocellulaire (HCC) au stade intermédiaire. TACE est généralement effectué par accès transfémoral (TFA). Cependant, l’accès transradial (TRA) est préféré dans les interventions d’artère coronaire dues aux complications et à la mortalité diminuées. La question de savoir si les avantages de la TRA peuvent être appliqués au TACE nécessitait une enquête.
Les patients recevant TRA TACE à un seul centre ont été rétrospectivement inscrits pour l’étude. Les détails procéduraux, le succès technique, le taux d’occlusion radiale d’artère (RAO) et les complications de saignement site-connexes d’accès ont été évalués. D’octobre 2017 à octobre 2018, 112 patients ont subi 160 interventions TRA TACE. Le taux de réussite technique global était de 95,0 % (152/160). Le taux de croisement entre tra et TFA était de 1,9%. Aucune complication de saignement liée au site d’accès n’a été trouvée dans n’importe quel cas. L’occlusion asymptomatique de RA s’est produite dans trois patients (2.7%). Par rapport à l’AFE, tra peut augmenter la sécurité et la satisfaction des patients tout en diminuant l’accès aux complications hémorraiques liées au site. En outre, les interventions de TRA peuvent bénéficier aux patients avec l’âge avancé, l’obésité, ou un risque élevé de complications de saignement.
Le carcinome hépatocellulaire (HCC) est une malignité très commune, avec le sixième taux d’incidence le plus élevé dans le monde entier. C’est aussi la deuxième cause de mortalité par cancer dans le monde1. Puisque seulement 5%-20% de patients peuvent recevoir la thérapie curative, la chimioembolization transarterial (TACE) est le traitement palliatif le plus populaire pour des patients présentant hcc2 nonrésécable. TACE a été reconnu comme l’approche de traitement la plus couramment utilisée et efficace pour les patients HCC à l’étapeintermédiaire 3. La chimioembolisation de l’accès transfémoral (AFE) est l’approche la plus courante pour TACE4. Cependant, il y a des risques liés à l’intervention de TFA, y compris le saignement au site d’accès et les complications vasculairesprincipales 5. Ces complications entraînent une hospitalisation prolongée et une augmentation des coûts. En outre, TFA exige l’immobilisation pendant au moins 6 h, qui augmente l’inconfort et l’insatisfaction pour les patients.
L’accès transradial (TRA) est une approche alternative qui a été utilisée dans l’intervention coronaire percutanée (PCI) depuis plus dedeux décennies 5,6. Tra PCI a plusieurs avantages : confort accru de procédure, saignement lié au site d’accès diminué, complications vasculaires importantes diminuées, et mortalitédiminuée 7,8. L’artère radiale (RA) est facile d’accès et de ponction en raison de son emplacement superficiel7. Hemostasis est facile à conduire après l’intervention et il n’y a pas d’immoblisationstricte 9. En dépit des preuves encourageantes pour l’intervention de TRA dans le cathétérisme cardiaque, jusqu’ici seulement quelques études ont employé TRA dans l’intervention périphérique de maladie. Les interventions de TRA pour des tumeurs malignes de foie sont encore plus rares. Ici, la faisabilité clinique et l’innocuité de l’embolie hépatique tra sont analysées. L’expérience d’une institution avec le protocole tra étape par étape fourni est également décrite.
La thérapie interventionnelle tra s’est considérablement développée dans le monde entier ces dernières années, particulièrement dans les procédures diagnostiques et interventionnellesde cardiologie 12.
