Este protocolo descreve a metodologia para rastreamento não invasivamente de células T geneticamente modificadas para expressar receptores de antígeno quimérico in vivo com uma plataforma clinicamente disponível.
Células T geneticamente modificadas para expressar receptores de antígeno quimérico (CAR) mostraram resultados sem precedentes em ensaios clínicos cruciais para pacientes com malignidades celulares B ou mieloma múltiplo (MM). No entanto, inúmeros obstáculos limitam a eficácia e proíbem o uso generalizado de terapias celulares CAR T devido ao mau tráfico e infiltração em locais tumorais, bem como à falta de persistência in vivo. Além disso, toxicidades que ameaçam a vida, como síndrome de liberação de citocinas ou neurotoxicidade, são grandes preocupações. Imagens eficientes e sensíveis e o rastreamento de células CAR T permitem a avaliação do tráfico de células T, expansão e caracterização in vivo e permite o desenvolvimento de estratégias para superar as limitações atuais da terapia celular CAR T. Este artigo descreve a metodologia de incorporação do isíndote de sódio (NIS) em células CAR T e para a imagem celular CAR T usando a tomografia de emissão de tetrafluoroborato-pósitron ([18F]TFB-PET) em modelos pré-clínicos. Os métodos descritos neste protocolo podem ser aplicados a outras construções car e genes-alvo, além dos utilizados neste estudo.
A terapia celular do receptor de antígeno quimérico T (CAR T) é uma abordagem rapidamente emergente e potencialmente curativa em malignidades hematológicas1,2,3,4,5,6. Desfechos clínicos extraordinários foram relatados após a terapia celular CAR T (CART19) ou B (BCMA) de maturação celular B (BCMA). Isso levou à aprovação da Food and Drug Administration (FDA) dos EUA das células CART19 para linfoma agressivo de células B (axicabtagene ciloleucel (Axi-Cel)4, tisagenlecleucel (Tisa-Cel)3, e lisocabtagene maraleucel)7, leucemia linfoblástica aguda (Tisa-Cel)5,8, linfoma de células de manto (brexucabtagene autoleuce)9, e linfoma folicular (Axi-Cel)10 . Mais recentemente, a FDA aprovou a terapia celular CAR T dirigida pelo BCMA em pacientes com mieloma múltiplo (MM) (idecabtagene vicleucel)11. Além disso, a terapia celular CAR T para leucemia linfocítica crônica (LLC) está em desenvolvimento clínico em estágio final e deverá receber aprovação da FDA nos próximos três anos11.
Apesar dos resultados inéditos da terapia celular CAR T, seu uso generalizado é limitado por 1) insuficiente na expansão celular CAR T ou mau tráfico para locais tumorais, o que leva a menores taxas de resposta durável12,13 e 2) o desenvolvimento de eventos adversos com risco de vida, incluindo síndrome de liberação de citocinas (CRS)14,15 . As marcas do CRS incluem não apenas ativação imunológica resultando em níveis elevados de citocinas inflamatórias/quimioterapias, mas também proliferação maciça de células T após infusão de células CAR T15,16. Assim, o desenvolvimento de uma estratégia validada de grau clínico para imagem de células CAR T in vivo permitiria 1) O rastreamento de células CAR T em tempo real in vivo monitoraria seu tráfico para locais tumorais e descobriria mecanismos potenciais de resistência, e 2) monitoramento da expansão de células CAR T e potencialmente prever suas toxicidades, como o desenvolvimento de CRS.
As características clínicas de SS leve são febre alta, fadiga, dor de cabeça, erupção cutânea, diarreia, artalgia, mialgia e mal-estar. Em CRS mais grave, os pacientes podem desenvolver taquicardia/hipotensão, vazamento capilar, disfunção cardíaca, insuficiência renal/hepática e coagulação intravascular disseminada17,18. Em geral, o grau de elevação das citocinas, incluindo interferon-gama, fator estimulante da colônia granulocito-macrófago, interleucina (IL)-10 e IL-6, mostrou-se correlacionar com a gravidade dos sintomas clínicos17,19. No entanto, a ampla aplicação do monitoramento de citocinas séricos “em tempo real” para prever o CRS é difícil devido ao alto custo e disponibilidade limitada. Para explorar as características benéficas da terapia celular CAR T, imagens não invasivas de células T adotivas podem ser potencialmente utilizadas para prever a eficácia, toxicidades e recaídas após a infusão de células CAR T.
Vários pesquisadores desenvolveram estratégias para usar imagens baseadas em radionuclídeos com tomografia de emissão de pósitrons (PET) ou tomografia computadorizada por emissão de fótons (SPECT), que fornece alta resolução e alta sensibilidade20,21,22,23,24,25,26,27,28,29,30 para o visualização in vivo e monitoramento do tráfico de células CAR T. Entre essas estratégias de imagem baseadas em radionuclídeos, o isíndonte de iodeto de sódio (NIS) foi desenvolvido como uma modalidade sensível às células de imagem e vírus usando pet scans31,32. A imagem celular NIS+CAR T com [18F]TFB-PET é uma tecnologia sensível, eficiente e conveniente para avaliar e diagnosticar a expansão, o tráfico e a toxicidade das células CAR T. Este protocolo descreve 1) o desenvolvimento de células NIS+CAR T através de transdução dupla com alta eficácia e 2) uma metodologia para imagens de células NIS+CAR T com [18F]TFB-PET scan. BCMA-CAR T células para MM são usadas como um modelo de prova de conceito para descrever NIS como repórter para imagens de células CAR T.
