Dit protocol beschrijft de methodologie voor het niet-invasief volgen van T-cellen die genetisch gemanipuleerd zijn om chimere antigeenreceptoren in vivo tot expressie te brengen met een klinisch beschikbaar platform.
T-cellen die genetisch gemanipuleerd zijn om chimere antigeenreceptoren (CAR) tot expressie te brengen, hebben ongekende resultaten laten zien in cruciale klinische onderzoeken voor patiënten met B-cel maligniteiten of multipel myeloom (MM). Talrijke obstakels beperken echter de werkzaamheid en verbieden het wijdverbreide gebruik van CAR T-celtherapieën als gevolg van slechte handel en infiltratie in tumorlocaties en gebrek aan persistentie in vivo. Bovendien zijn levensbedreigende toxiciteiten, zoals cytokine release syndrome of neurotoxiciteit, grote zorgen. Efficiënte en gevoelige beeldvorming en tracking van CAR T-cellen maakt de evaluatie van T-celhandel, uitbreiding en in vivo karakterisering mogelijk en maakt de ontwikkeling van strategieën mogelijk om de huidige beperkingen van CAR T-celtherapie te overwinnen. Dit artikel beschrijft de methodologie voor het opnemen van de natriumjodidesymporter (NIS) in CAR T-cellen en voor CAR T-celbeeldvorming met behulp van [18F]tetrafluoroboraat-positronemissietomografie ([18F]TFB-PET) in preklinische modellen. De methoden die in dit protocol worden beschreven, kunnen worden toegepast op andere CAR-constructies en doelgenen naast de methoden die voor deze studie worden gebruikt.
Chimere antigeenreceptor T (CAR T) celtherapie is een snel opkomende en potentieel curatieve benadering bij hematologische maligniteiten1,2,3,4,5,6. Buitengewone klinische resultaten werden gerapporteerd na CD19-gerichte CAR T (CART19) of B-celrijpingsantigeen (BCMA) CAR T-celtherapie2. Dit leidde tot de goedkeuring door de Amerikaanse Food and Drug Administration (FDA) van CART19-cellen voor agressief B-cellymfoom (axicabtagene ciloleucel (Axi-Cel)4, tisagenlecleucel (Tisa-Cel)3 en lisocabtagene maraleucel)7, acute lymfoblastische leukemie (Tisa-Cel)5,8, mantelcellymfoom (brexucabtagene autoleuce)9 en folliculair lymfoom (Axi-Cel)10 . Onlangs keurde de FDA BCMA-gerichte CAR T-celtherapie goed bij patiënten met multipel myeloom (MM) (idecabtagene vicleucel)11. Bovendien bevindt CAR T-celtherapie voor chronische lymfatische leukemie (CLL) zich in een laat stadium van klinische ontwikkeling en zal naar verwachting binnen de komende drie jaar goedkeuring van de FDA krijgen1.
Ondanks de ongekende resultaten van CAR T-celtherapie, wordt het wijdverbreide gebruik ervan beperkt door 1) onvoldoende in vivo CAR T-celuitbreiding of slechte handel naar tumorlocaties, wat leidt tot lagere percentages van duurzame respons12,13 en 2) de ontwikkeling van levensbedreigende bijwerkingen, waaronder cytokine release syndrome (CRS)14,15 . De kenmerken van CRS omvatten niet alleen immuunactivatie die resulteert in verhoogde niveaus van inflammatoire cytokines / chemokines, maar ook massale T-celproliferatie na CAR T-celinfusie15,16. De ontwikkeling van een gevalideerde strategie van klinische kwaliteit om CAR T-cellen in vivo in beeld te brengen, zou dus 1) CAR T-celtracking in realtime in vivo mogelijk maken om hun handel naar tumorlocaties te volgen en potentiële mechanismen van resistentie te ontdekken, en 2) monitoring van CAR T-celuitbreiding en mogelijk hun toxiciteiten voorspellen, zoals de ontwikkeling van CRS.
Klinische kenmerken van mild CRS zijn hoge koorts, vermoeidheid, hoofdpijn, huiduitslag, diarree, artralgie, myalgie en malaise. Bij ernstiger CRS kunnen patiënten tachycardie/hypotensie, capillair lek, hartdisfunctie, nier-/leverfalen en gedissemineerde intravasculaire stolling ontwikkelen17,18. Over het algemeen is aangetoond dat de mate van verhoging van cytokines, waaronder interferon-gamma, granulocyt-macrofaag koloniestimulerende factor, interleukine (IL)-10 en IL-6, correleert met de ernst van klinische symptomen17,19. De uitgebreide toepassing van “real-time” serumcytokinemonitoring om CRS te voorspellen is echter moeilijk vanwege de hoge kosten en beperkte beschikbaarheid. Om de gunstige eigenschappen van CAR T-celtherapie te benutten, kan niet-invasieve beeldvorming van adoptieve T-cellen mogelijk worden gebruikt om de werkzaamheid, toxiciteit en terugval na CAR T-celinfusie te voorspellen.
Verschillende onderzoekers hebben strategieën ontwikkeld om op radionuclide gebaseerde beeldvorming te gebruiken met positronemissietomografie (PET) of single-photon emission computed tomography (SPECT), die een hoge resolutie en hoge gevoeligheid biedt20,21,22,23,24,25,26,27,28,29,30 voor de in vivo visualisatie en monitoring van CAR T-celhandel. Onder die op radionucliden gebaseerde beeldvormingsstrategieën is de natriumjodidesymporter (NIS) ontwikkeld als een gevoelige modaliteit voor beeldcellen en virussen met behulp van PET-scans31,32. NIS+CAR T-celbeeldvorming met [18F]TFB-PET is een gevoelige, efficiënte en handige technologie om CAR T-celuitbreiding, -handel en -toxiciteit te beoordelen en te diagnosticeren30. Dit protocol beschrijft 1) de ontwikkeling van NIS+CAR T-cellen door middel van dubbele transductie met hoge werkzaamheid en 2) een methodologie voor het in beeld brengen van NIS+CAR T-cellen met [18F]TFB-PET-scan. BCMA-CAR T-cellen voor MM worden gebruikt als een proof-of-concept model om NIS te beschrijven als een reporter voor CAR T-celbeeldvorming. Deze methoden kunnen echter worden toegepast op elke andere CAR T-celtherapie.
Dit artikel beschrijft een methodologie voor het opnemen van NIS in CAR T-cellen en beeldvorming geïnfundeerde CAR T-cellen in vivo via [18F] TFB-PET. Als proof of concept werden NIS+BCMA-CAR T-cellen gegenereerd via dubbele transductie.
