Questo protocollo descrive la metodologia per il tracciamento non invasivo delle cellule T geneticamente modificate per esprimere i recettori dell’antigene chimerico in vivo con una piattaforma clinicamente disponibile.
Le cellule T geneticamente modificate per esprimere i recettori dell’antigene chimerico (CAR) hanno mostrato risultati senza precedenti in studi clinici registrativi per pazienti con tumori maligni delle cellule B o mieloma multiplo (MM). Tuttavia, numerosi ostacoli limitano l’efficacia e vietano l’uso diffuso di terapie con cellule T CAR a causa dello scarso traffico e infiltrazione nei siti tumorali e della mancanza di persistenza in vivo. Inoltre, le tossicità potenzialmente letali, come la sindrome da rilascio di citochine o la neurotossicità, sono le principali preoccupazioni. L’imaging e il tracciamento efficienti e sensibili delle cellule T CAR consentono la valutazione del traffico, dell’espansione e della caratterizzazione in vivo delle cellule T e consentono lo sviluppo di strategie per superare gli attuali limiti della terapia con cellule T CAR. Questo documento descrive la metodologia per incorporare il symporter di ioduro di sodio (NIS) nelle cellule T CAR e per l’imaging delle cellule T CAR utilizzando la tomografia ad emissione di tetrafluoroborato-positrone [18F] tetrafluoroborato ([18F] TFB-PET) in modelli preclinici. I metodi descritti in questo protocollo possono essere applicati ad altri costrutti CAR e geni bersaglio oltre a quelli utilizzati per questo studio.
La terapia cellulare del recettore dell’antigene chimerico T (CAR T) è un approccio rapidamente emergente e potenzialmente curativo nelle neoplasie ematologiche1,2,3,4,5,6. Risultati clinici straordinari sono stati riportati dopo la terapia con cellule T CAR T (CART19) o CON l’antigene di maturazione delle cellule B (BCMA) CAR T2. Ciò ha portato all’approvazione da parte della Food and Drug Administration (FDA) degli Stati Uniti delle cellule CART19 per il linfoma aggressivo a cellule B (axicabtagene ciloleucel (Axi-Cel)4, tisagenlecleucel (Tisa-Cel)3 e lisocabtagene maraleucel)7, leucemia linfoblastica acuta (Tisa-Cel)5,8, linfoma a cellule del mantello (brexucabtagene autoleuce)9 e linfoma follicolare (Axi-Cel)10 . Più recentemente, la FDA ha approvato la terapia con cellule T CAR diretta da BCMA in pazienti con mieloma multiplo (MM) (idecabtagene vicleucel)11. Inoltre, la terapia con cellule T CAR per la leucemia linfocitica cronica (LLC) è in fase avanzata di sviluppo clinico e dovrebbe ricevere l’approvazione della FDA entro i prossimi tre anni1.
Nonostante i risultati senza precedenti della terapia con cellule T CAR, il suo uso diffuso è limitato da 1) insufficiente espansione in vivo delle cellule T CAR o scarso traffico verso i siti tumorali, che porta a tassi più bassi di risposta duratura12,13 e 2) lo sviluppo di eventi avversi potenzialmente letali, tra cui la sindrome da rilascio di citochine (CRS)14,15 . I segni distintivi della CRS includono non solo l’attivazione immunitaria con conseguente livelli elevati di citochine/chemochine infiammatorie, ma anche la massiccia proliferazione delle cellule T dopo infusione di cellule T CAR15,16. Pertanto, lo sviluppo di una strategia convalidata di livello clinico per l’immagine delle cellule T CAR in vivo consentirebbe 1) il tracciamento delle cellule T CAR in tempo reale in vivo per monitorare il loro traffico verso i siti tumorali e scoprire potenziali meccanismi di resistenza e 2) il monitoraggio dell’espansione delle cellule T CAR e potenzialmente prevedere le loro tossicità come lo sviluppo di CRS.
Le caratteristiche cliniche della CRS lieve sono febbre alta, affaticamento, mal di testa, eruzione cutanea, diarrea, artralgia, mialgia e malessere. Nella CRS più grave, i pazienti possono sviluppare tachicardia/ipotensione, perdita capillare, disfunzione cardiaca, insufficienza renale/epatica e coagulazione intravascolare disseminata17,18. In generale, il grado di elevazione delle citochine, tra cui interferone-gamma, fattore stimolante le colonie di granulociti-macrofagi, interleuchina (IL)-10 e IL-6, ha dimostrato di essere correlato alla gravità dei sintomi clinici17,19. Tuttavia, l’ampia applicazione del monitoraggio delle citochine sieriche “in tempo reale” per prevedere la CRS è difficile a causa dell’elevato costo e della disponibilità limitata. Per sfruttare le caratteristiche benefiche della terapia con cellule T CAR, l’imaging non invasivo delle cellule T adottive può essere potenzialmente utilizzato per prevedere l’efficacia, la tossicità e la ricaduta dopo l’infusione di cellule T CAR.
Diversi ricercatori hanno sviluppato strategie per utilizzare l’imaging a base di radionuclidi con tomografia ad emissione di positroni (PET) o tomografia computerizzata a emissione di singolo fotone (SPECT), che fornisce alta risoluzione e alta sensibilità20,21,22,23,24,25,26,27,28,29,30 per il visualizzazione e monitoraggio in vivo del traffico di cellule T CAR. Tra queste strategie di imaging basate su radionuclidi, il symporter di ioduro di sodio (NIS) è stato sviluppato come modalità sensibile per l’immagine di cellule e virus utilizzando scansioni PET31,32. L’imaging delle cellule T NIS+CAR con [18F]TFB-PET è una tecnologia sensibile, efficiente e conveniente per valutare e diagnosticare l’espansione, il traffico e la tossicità delle cellule T CAR30. Questo protocollo descrive 1) lo sviluppo di cellule T NIS+CAR attraverso la doppia trasduzione ad alta efficacia e 2) una metodologia per l’imaging di cellule T NIS+CAR con scansione [18F]TFB-PET. Le cellule T BCMA-CAR per MM sono utilizzate come modello proof-of-concept per descrivere NIS come reporter per l’imaging delle cellule T CAR. Tuttavia, queste metodologie possono essere applicate a qualsiasi altra terapia con cellule T CAR.
Questo documento descrive una metodologia per incorporare NIS nelle cellule T CAR e l’imaging infuso di cellule T CAR in vivo attraverso [18F] TFB-PET.
