Een uitgebreid laboratoriumprotocol en analyseworkflow worden beschreven voor een snelle, kosteneffectieve en eenvoudige colorimetrische celgebaseerde test om neutraliserende elementen tegen AAV6 te detecteren.
Recombinante adeno-geassocieerde virussen (rAAV) hebben bewezen een veilige en succesvolle vector te zijn voor het overbrengen van genetisch materiaal om verschillende gezondheidsproblemen in zowel het laboratorium als de kliniek te behandelen. Reeds bestaande neutraliserende antilichamen (NAbs) tegen AAV-capsiden vormen echter een voortdurende uitdaging voor de succesvolle toediening van gentherapieën in zowel grote dierexperimentele modellen als menselijke populaties. Voorlopige screening op gastheerimmuniteit tegen AAV is noodzakelijk om de werkzaamheid van op AAV gebaseerde gentherapieën te waarborgen als zowel een onderzoeksinstrument als een klinisch levensvatbaar therapeutisch middel. Dit protocol beschrijft een colorimetrische in vitro test om neutraliserende factoren tegen AAV serotype 6 (AAV6) te detecteren. De test maakt gebruik van de reactie tussen een AAV die codeert voor een alkalisch fosfatase (AP) reporter gen en het substraat NBT / BCIP, dat een onoplosbare kwantificeerbare paarse vlek genereert bij combinatie.
In dit protocol worden serummonsters gecombineerd met een AAV die AP tot expressie brengt en geïncubeerd om potentiële neutraliserende activiteit mogelijk te maken. Virusserummengsel wordt vervolgens toegevoegd aan cellen om virale transductie mogelijk te maken van eventuele AAV’s die niet zijn geneutraliseerd. Het NBT/BCIP-substraat wordt toegevoegd en ondergaat een chromogene reactie, die overeenkomt met virale transductie en neutraliserende activiteit. Het aandeel van het gekleurde gebied wordt gekwantificeerd met behulp van een gratis softwaretool om neutraliserende titers te genereren. Deze test toont een sterke positieve correlatie tussen kleuring en virale concentratie. Beoordeling van serummonsters van schapen voor en na toediening van een recombinant AAV6 leidde tot een dramatische toename van de neutraliserende activiteit (125 tot >10.000-voudige toename). De test vertoonde voldoende gevoeligheid om neutraliserende activiteit te detecteren in >1:32.000 serumverdunningen. Deze test biedt een eenvoudige, snelle en kosteneffectieve methode om NAbs tegen AAV’s te detecteren.
Adeno-geassocieerde virussen (AAV) worden steeds vaker gebruikt als vectoren voor de levering van gentherapieën aan proefbehandelingen voor verschillende gezondheidsproblemen die van invloed zijn op het cardiovasculaire, pulmonale, circulatoire, oculaire en centrale zenuwstelsel1,2,3,4,5. De populariteit van AAV-vectoren als toonaangevend gentherapieplatform komt voort uit hun positieve veiligheidsprofiel, transgenexpressie op lange termijn en brede weefselspecifieke tropismen1,6. Succesvolle resultaten in dierstudies hebben de weg vrijgemaakt voor meer dan vijftig klinische onderzoeken naar AAV-gentherapie die met succes hun werkzaamheidseindpunten hebben bereikt7, evenals de release van het eerste commercieel beschikbare AAV-gentherapiegeneesmiddel dat is goedgekeurd door de Amerikaanse Food and Drug Administration8. Na de eerste successen is AAV blijven tractie krijgen in de basis- en klinische onderzoekssectoren als een vector bij uitstek en is het momenteel de enige in vivo gentherapie die is goedgekeurd voor klinisch gebruik in de VS en Europa9. Niettemin blijft de aanwezigheid van reeds bestaande neutraliserende antilichamen (NAbs) tegen AAV-vectorcapsiden een belemmering voor zowel preklinisch onderzoek als de werkzaamheid van klinische onderzoeken. NAbs zijn aanwezig in zowel naïeve menselijke als dierlijke populaties en remmen gentransductie na in vivo toediening van een AAV-vector1. AAV-seropositiviteit is een uitsluitingscriterium voor de meeste gentherapiestudies en daarom is voorlopige screening op gastheerimmuniteit cruciaal in zowel het laboratorium als de kliniek. Het opzetten van een test die de aanwezigheid van NAbs tegen AAV kan detecteren, is een essentiële stap in de pijplijn van elk op AAV-gentherapie gebaseerd onderzoeksproject. Dit rapport richt zich op AAV6 dat van belang is geweest voor onderzoekers vanwege de efficiënte en selectieve transductie in dwarsgestreepte spieren (hart- en skeletspieren)1,10,11,12. Gentherapie wordt beschouwd als een veelbelovende strategie voor het richten op het hart, omdat het moeilijk is om specifiek op het hart te richten zonder invasieve openhartprocedures.
Neutraliserende activiteit wordt meestal bepaald met behulp van een celgebaseerde in vitro of in vivo transductie remmingstest. In vivo NAb-assays omvatten meestal het toedienen van serum van een proefpersoon (bijv. Menselijk of groot dier) aan muizen, gevolgd door een AAV met een reporter-gen, gevolgd door testen op de expressie van het reporter-gen of het bijbehorende antigeen. In vitro assays bepalen NAb-titers door serum of plasma van een mens of groot dier in seriële verdunningen te incuberen met een recombinant AAV (rAAV) dat een reporter-gen tot expressie brengt. Cellen zijn geïnfecteerd met het serum/virusmengsel en de mate waarin de genexpressie van de reporter wordt geremd, wordt beoordeeld in vergelijking met controles. In vitro assays worden veel gebruikt voor NAb-screening vanwege hun relatief lagere kosten, snelheid bij het testen en grotere capaciteit voor standaardisatie en validatie13,14 in vergelijking met in vivo assays. Van in vivo assays wordt vaak gemeld dat ze een grotere gevoeligheid hebben15,16, maar dezelfde claim is gemaakt met betrekking tot in vitro assays14,17.
Tot op heden hebben in vitro NAb-assays voornamelijk luminescentie (luciferase) gebruikt als het reporter-gen om neutralisatie te detecteren. Hoewel een op licht gebaseerde methode in veel contexten verdienstelijk is, kan een colorimetrische / chromogene NAb-test in sommige omstandigheden voordelig zijn. Colorimetrische assays om neutralisatie te beoordelen zijn met succes gebruikt voor andere virussen zoals influenza en adenovirus18,19. Hun aantrekkelijkheid komt voort uit hun eenvoud, lagere kosten en de behoefte aan alleen alledaagse laboratoriumapparatuur en gereedschappen20. NAb-assays die een op luminescentie gebaseerd reporter-gen gebruiken, vereisen dure substraatkits, een luminometer en bijbehorende software voor analyse21. Deze colorimetrische test heeft het voordeel dat alleen een lichtmicroscoop en een zeer goedkoop substraat nodig zijn. Rapportage van de gevoeligheid van colorimetrische versus luminescente assays heeft tegenstrijdige resultaten opgeleverd. Eén studie suggereerde dat op luminescentie gebaseerde ELISA-assays een grotere gevoeligheid en vergelijkbare reproduceerbaarheid vertonen als colorimetrische assays22, terwijl een andere vond dat op colorimetrische gebaseerde ELISA-assays een grotere gevoeligheid verlenen23. Hier wordt een gedetailleerd protocol gegeven voor een in vitro NAb-test tegen AAV die gebruik maakt van de chromogene reactie tussen een AAV-codering van een alkalisch fosfatase (AP) reportergen en een nitroblauw tetrazolium / 5-broom-4-chloor-3-indolylfosfaat (NBT / BCIP) substraat. Dit stapsgewijze protocol is ontwikkeld op basis van een eerder rapport dat een hPLAP (human placental alkaline phosphatase) reporter gen (AAV6-hPLAP) gebruikte om neutraliserende activiteit tegen AAV24 te detecteren. Deze test is kosteneffectief, tijdbesparend, eenvoudig in te stellen en vereist minimale technische vaardigheden, laboratoriumapparatuur en reagentia. Bovendien geeft de eenvoud van deze test het potentieel om te worden geoptimaliseerd voor brede toepassingen in verschillende soorten cellen, weefsels of virale serotypen.
Dit rapport beschrijft een colorimetrische test die de mate van AAV-neutralisatie in een bepaald serummonster beoordeelt door een chromogene reactie te evalueren die overeenkomt met de mate van in vitro virale transductie.
