Hier beschrijven we een protocol waarin een acuut lymfoblastische leukemiepatiënt-afgeleid xenograftmodel wordt gebruikt als een strategie om CD19-gerichte chimere antigeenreceptor T-cel-geassocieerde toxiciteiten te beoordelen en te monitoren.
Chimere antigeenreceptor T (CART) celtherapie is naar voren gekomen als een krachtig hulpmiddel voor de behandeling van meerdere soorten CD19 + maligniteiten, wat heeft geleid tot de recente FDA-goedkeuring van verschillende CD19-gerichte CART (CART19) celtherapieën. CART-celtherapie wordt echter geassocieerd met een unieke reeks toxiciteiten die hun eigen morbiditeit en mortaliteit met zich meebrengen. Dit omvat cytokine release syndrome (CRS) en neuro-inflammatie (NI). Het gebruik van preklinische muismodellen is cruciaal geweest in het onderzoek en de ontwikkeling van CART-technologie voor het beoordelen van zowel CART-werkzaamheid als CART-toxiciteit. De beschikbare preklinische modellen om deze adoptieve cellulaire immunotherapie te testen omvatten syngene, xenograft, transgene en gehumaniseerde muismodellen. Er is geen enkel model dat naadloos het menselijk immuunsysteem weerspiegelt, en elk model heeft sterke en zwakke punten. Dit methodedocument heeft tot doel een van de patiënt afgeleid xenograftmodel te beschrijven met behulp van leukemische blasten van patiënten met acute lymfatische leukemie als een strategie om CART19-geassocieerde toxiciteiten, CRS en NI te beoordelen. Van dit model is aangetoond dat het CART19-geassocieerde toxiciteiten en therapeutische werkzaamheid zoals gezien in de kliniek samenvat.
Chimere antigeenreceptor T (CART) celtherapie heeft een revolutie teweeggebracht op het gebied van kankerimmunotherapie. Het heeft bewezen succesvol te zijn bij de behandeling van recidiverende / refractaire acute lymfatische leukemie (ALL), grootcellig B-cellymfoom, mantelcellymfoom, folliculair lymfoom en multipel myeloom 1,2,3,4,5,6,7, wat leidt tot recente FDA-goedkeuringen. Ondanks het aanvankelijke succes in klinische onderzoeken, resulteert behandeling met CART-celtherapie in toxiciteiten die vaak ernstig en soms dodelijk zijn. De meest voorkomende toxiciteiten na CART-celtherapie zijn de ontwikkeling van CRS en NI, ook wel immuuneffectorcelgeassocieerd neurotoxiciteitssyndroom (ICANS) genoemd8,9. CRS wordt veroorzaakt door de overactivatie en massale expansie van CART-cellen in vivo, wat leidt tot de daaropvolgende secretie van meerdere inflammatoire cytokines, waaronder interferon-γ, tumornecrosefactor-α, granulocyt-macrofaag koloniestimulerende factor (GM-CSF) en interleukine-6 (IL-6). Dit resulteert in hypotensie, hoge koorts, capillair leksyndroom, respiratoire insufficiëntie, multi-orgaanfalen en in sommige gevallen de dood10,11. CRS ontwikkelt zich in 50-100% van de gevallen na CART19 celtherapie11,12,13. ICANS is een andere unieke bijwerking geassocieerd met CART-celtherapie en wordt gekenmerkt door gegeneraliseerd hersenoedeem, verwarring, obtundatie, afasie, motorische zwakte en af en toe aanvallen 9,14. Elke graad van ICANS komt voor bij maximaal 70% van de patiënten, en graad 3-4 wordt gemeld bij 20-30% van de patiënten 5,10,15,16. Over het algemeen komen CRS en ICANS vaak voor en kunnen ze fataal zijn.
Het beheer van ICANS na CART-celtherapie is een uitdaging. De meeste patiënten met ICANS ervaren ook CRS17, dat vaak kan worden behandeld met de IL-6-receptorantagonist tocilizumab of steroïden18. Een eerder rapport toonde aan dat vroege interventie met tocilizumab de snelheid van ernstige CRS verminderde, maar geen invloed had op de incidentie of ernst van ICANS19. Momenteel is er geen effectieve behandeling of profylactisch middel voor ICANS, en het is cruciaal om preventieve strategieën te onderzoeken20.
Myeloïde cellen en bijbehorende cytokines/chemokines worden beschouwd als de belangrijkste aanjagers van de ontwikkeling van CRS en ICANS21. Terwijl CRS direct gerelateerd is aan de extreme verhoging van cytokines en T-celexpansie, is de pathofysiologie van ICANS grotendeels onbekend22,23. Daarom is het noodzakelijk om een muismodel op te stellen dat deze toxiciteiten na CART-celtherapie samenvat om de mechanismen te bestuderen en preventieve strategieën te ontwikkelen.
Er zijn meerdere preklinische diermodellen die momenteel worden gebruikt om de werkzaamheid van CART-cellen te bestuderen, te optimaliseren en te valideren, en om hun bijbehorende toxiciteiten te controleren. Deze omvatten syngene, xenograft, immunocompetente transgene, gehumaniseerde transgene en patiënt-afgeleide xenograftmuizen, naast primatenmodellen. Elk van deze modellen heeft echter nadelen en sommige weerspiegelen niet de werkelijke werkzaamheid of veiligheidsproblemen van CART-cellen24,25. Daarom is het noodzakelijk om zorgvuldig het beste model te kiezen voor de beoogde doelen van het onderzoek.
Dit artikel beoogt de methodologie te beschrijven die wordt gebruikt om CART-cel-geassocieerde toxiciteiten, CRS en NI te beoordelen, met behulp van een ALL patient-derived xenograft (PDX) in vivo model (Figuur 1). In het bijzonder worden in de hier beschreven methoden CART19-cellen gebruikt die in het laboratorium van de auteurs zijn gegenereerd volgens eerder beschreven protocollen. Kortom, menselijke T-cellen worden geïsoleerd uit gezonde donor perifere bloed mononucleaire cellen (PBMC’s) via een dichtheidsgradiënttechniek, gestimuleerd met CD3 / CD28-kralen op dag 0 en lentiviraal getransduceerd op dag 1 met CARs samengesteld uit een CD19-gericht enkelvoudig keten variabel fragment gefuseerd met 4-1BB- en CD3ζ-signaleringsdomeinen. Deze CART-cellen worden vervolgens uitgebreid, ont-beaded op dag 6 en gecryopreserveerd op dag 8 26,27,28,29,30. Zoals eerder beschreven, worden muizen onderworpen aan een lymfodepleterende behandeling, gevolgd door de toediening van van de patiënt afgeleide leukemische blasten (ALL)28. Eerst wordt tumortransplantatie geverifieerd via submandibulaire bloedafname. Na het vaststellen van een geschikte tumorlast worden CART19-cellen toegediend aan de muizen. Vervolgens worden de muizen dagelijks gewogen om het welzijn te beoordelen. Beeldvorming van magnetische resonantie van kleine dieren (MRI) wordt uitgevoerd om NI te beoordelen, samen met staartbloedingen om T-celexpansie en cytokine / chemokineproductie te beoordelen. De hieronder beschreven technieken worden ten zeerste aanbevolen om te worden gebruikt als een model om CART-cel-geassocieerde toxiciteiten in een PDX-model te bestuderen.
In dit rapport is een methodologie beschreven om CART-cel-geassocieerde toxiciteiten te beoordelen met behulp van een ALL PDX-model. Meer specifiek probeert dit model twee levensbedreigende toxiciteiten na te bootsen, CRS en NI, die patiënten vaak ervaren na de infusie van CART-cellen.
