Het doel van dit protocol is het samenstellen en verwerken van menselijke chirurgische monsters voor meerdere downstream toepassingen in vestibulaire schwannoma- en Schwann-celonderzoek.
Vestibulaire schwannomen zijn de meest voorkomende neoplasma's van de cerebellopontinehoek, die 6-8% van alle intracraniale groeizen vormen. Hoewel deze tumoren sensorineuraal gehoorverlies veroorzaken in 95% van de getroffen individuen, blijven de moleculaire mechanismen die tot dit gehoorverlies leiden, onduidelijk. Dit artikel schetst de stappen in ons laboratorium om de inzameling en verwerking van diverse primaire menselijke weefselmonsters te vergemakkelijken voor downstream onderzoeksapplicaties die integraal zijn bij de studie van vestibulaire schwannomen. Specifiek beschrijft dit werk een verenigd methodologisch kader voor de verzameling, verwerking en cultuur van Schwann- en schwannomacellen uit chirurgische monsters. Dit is geïntegreerd met parallelle verwerkingsstappen die nu essentieel zijn voor huidig onderzoek: de verzameling van tumor- en zenuwafscheidingen, het behoud van RNA en de extractie van eiwitten uit verzamelde weefsels, de fixatie van weefsel voor de bereiding van secties, eenD de blootstelling van primaire humane cellen aan adeno-geassocieerde virussen voor toepassing op gentherapie. Daarnaast benadrukt dit werk de translabyrinthine chirurgische benadering om deze tumor te verzamelen als een unieke kans om menselijk zintuiglijk epitheel van het binnenoor en periliem te verkrijgen. Tips voor het verbeteren van de experimentele kwaliteit worden verstrekt en gemeenschappelijke valkuilen worden gemarkeerd.
Vestibulaire schwannomen (VS's) zijn de meest voorkomende neoplasma's van de cerebellopontinehoek, gediagnosticeerd bij 1 op elke 100.000 individuen. Hoewel niet-metastatische, beïnvloeden deze tumoren een levenskwaliteit van een patiënt 1 , 2 , 3 , 4 , 5 , 6 ernstig. Geïnteresseerden leven meestal met gehoorverlies, tinnitus en een gevoel van gehoorvervulling. Symptomen worden steeds meer belemmerend aangezien de tumor groeit, waardoor balansproblemen, gelaatsverlamming en vermindering van andere craniale zenuwfuncties ontstaan. Levensbedreigende complicaties als gevolg van hersenstam compressie kunnen ook 7 opleveren.
Beheersopties voor VS zijn in wezen beperkt tot waakzaam wachten op statische tumoren en stereotactische radiotherapie of chirurgische resectie voor groeiende tumoren <sUp class = "xref"> 8. De chirurgische verwijdering van deze tumoren in onderzoeksgebonden ziekenhuizen biedt de mogelijkheid om vers tumorweefsel te verzamelen en te analyseren die tijdens de patiëntenoperaties worden verzameld. Een specifieke chirurgische benadering van VS, de translabyrinthine resectie, kan zelfs toegang bieden tot waardevolle menselijke zintuiglijke epithelium uit het binnenoor en periliem.
Omdat VS's voortkomen uit een perifere sensorische zenuw ( dwz de vestibulaire zenuw), is het belangrijk om VS-geassocieerde waarnemingen te vergelijken met die afgeleid van een passende controlevenuwe, zoals de menselijke grote auriculaire zenuw (GAN). Gezonde GAN's worden regelmatig geofferd tijdens parotidectomies of nekdissecties en kunnen gebruikt worden als robuuste modellen voor gezonde Schwann-celfysiologie 9 .
Omdat er geen FDA-goedgekeurde geneesmiddelen zijn voor de behandeling of preventie van sporadische VS, is het essentieel dat onderzoekers de onderliggende moleculaire mecha verduidelijkenNismen van de ziekte ter identificatie van therapeutische doelen. Proteïnen die zijn aangetoond dat ze een rol spelen in de VS-pathogenese, omvatten merlin, vasculaire endotheliale groeifactor (VEGF), cyclooxygenase 2 (COX-2), nucleaire factor kappa B (NF-KB), tumornekrosefactor alfa (TNF-alpha) , Epidermale groeifactorreceptor (EGFR) en verwante signaleringsmoleculen 10 , 11 , 12 , 13 , 14 , 15 , 16 , 17 .
Recente vooruitgang heeft uitgebreid en verbeterde protocollen voor het verzamelen, verwerken, culturen en stroomafwaarts onderzoek van primaire humane vestibulaire schwannomen en gezonde zenuwweefsels 18 , 19 . De meeste bestaande protocollen zijn echter ontworpen om de voorbereiding op te nemenAtion van dergelijke weefsels voor een enkele stroomafwaartse onderzoeksaanvraag ( dwz alleen celcultuur). Dit artikel bevat een uniforme methodologische kader voor de gelijktijdige verwerking van een enkelvoudig primair menselijk VS- of GAN-monster voor meerdere downstream toepassingen: celtype-specifieke cultuur, eiwit-extractie, RNA-behoud, insameling van tumorenecretie en weefselfixatie. Dit werk bevat ook details over de chirurgische insameling en verwerking van menselijke cerebrospinale vloeistof (CSF) en periliem tijdens transplantabyrinthine VS resectie, aangezien deze nauw verwante weefsels kunnen dienen als belangrijke bronnen van biomarkers voor VS. Tenslotte presenteert dit protocol stappen voor de virale transductie van primaire menselijke VS-cellen in cultuur als een nieuwe toepassing van dit weefsel voor gebruik bij gentherapie.
Dit manuscript beschrijft een verenigd methodologisch kader voor VS-onderzoek, waarin de gelijktijdige verwerking van menselijke VS- en GAN-monsters wordt beschreven voor downstream onderzoeksoepassingen.
