Se describe un protocolo para medir el tiempo de formación de vórtice, un índice de eficiencia de llenado ventricular izquierdo, usando técnicas de ecocardiografía de transesophageal estándar en pacientes sometidos a cirugía cardíaca. Aplicamos esta técnica para analizar el tiempo de formación de vórtice en varios grupos de pacientes con diferentes patologías cardíacas.
Flujo sanguíneo trans-mitral produce un cuerpo rotacional tridimensional de líquido, conocido como un anillo de vórtice, que mejora la eficiencia del ventrículo izquierdo (LV) relleno en comparación con un chorro lineal continuo. Desarrollo del anillo de vórtice se cuantifica más a menudo con el tiempo de formación de vórtice (VFT), un parámetro sin dimensiones basado en la expulsión de líquido de un tubo rígido. Nuestro grupo está interesado en los factores que afectan LV eficiencia de llenado durante la cirugía cardíaca. En este informe, describimos el uso estándar bidimensional (2D) y Doppler la ecocardiografía transesophageal (te) para derivar de forma no invasiva las variables necesarias para calcular VFT. Calculamos la fracción de llenado auricular (β) de las integrales de velocidad-tiempo de trans-mitral temprano relleno de LV y sístole atrial sangre flujo velocidad formas de onda medidos en el punto de vista TEE cuatro-compartimiento mediados de-del esófago. Volumen sistólico (SV) se calcula como el producto del diámetro de la pista de salida de LV medido en el punto de vista de eje largo del mediados de-esófago TEE y la integral velocidad tiempo del flujo de sangre a través de la pista de salida determinado en la vista de transgastric profunda mediante ondas de pulso Doppler. VFT se calcula entonces como 4 × del × (1-β) SV / (πD3). Hemos utilizado esta técnica para analizar VFT en varios grupos de pacientes con anormalidades cardiacas diferentes. Discutir la aplicación de esta técnica y sus posibles limitaciones y también revisar nuestros resultados hasta la fecha. Medición no invasiva de VFT utilizando TEE es sencilla en pacientes anestesiados, sometidos a cirugía cardíaca. La técnica puede permitir que cirujanos y anestesiólogos cardiacos evaluar el impacto de las condiciones patológicas y las intervenciones quirúrgicas en LV relleno eficiencia en tiempo real.
Mecánica de fluidos es un determinante crítico pero a menudo infravalorado de relleno ventricular izquierdo de (LV). Un cuerpo de rotación tridimensional de líquido, conocido como un anillo de vórtice, se genera siempre que un fluido atraviesa un orificio1,2,3. Este anillo de vórtice mejora la eficiencia de transporte de fluidos en comparación con un chorro lineal continuo4. Movimiento de la sangre a través de la válvula mitral durante el llenado del LV temprana causa un anillo de vórtice formar5,6,7,8 y facilita su propagación en la cámara mediante la conservación de ímpetu fluido y energía cinética9. Estas acciones mejoran LV relleno eficiencia4,10,11,12,13. El anillo no sólo inhibe la estasis del flujo de sangre en el ápice del LV14,15,16,17 sino también dirige el flujo preferencial bajo el prospecto mitral anterior7, 18, efectos que disminuyen el riesgo de formación de trombo apical y facilitan el llenado de la salida de LV pista19, respectivamente. Ecocardiografía de contraste17, vector Doppler flujo asignación6,20,21, proyección de imagen de resonancia magnética7y partícula imaging velocimetry9,22 ,23,24 se han utilizado para demostrar el aspecto y comportamiento de anillos de vórtice trans-mitral bajo condiciones normales y patológicas. El gradiente de presión de LV atrial izquierdo, el grado de excursión anular mitral diastólica, la presión de LV mínima alcanzada durante la diástole y la velocidad y extensión de la relajación LV son los cuatro principales determinantes de la duración, tamaño, intensidad de la corriente y posición de el anillo mitral trans2,12,25,26,27,28,29.
Desarrollo del anillo de vórtice más a menudo se cuantifica con un parámetro adimensional (tiempo de formación de vórtice; VFT) basada en la expulsión de líquido de un tubo rígido3, donde VFT se define como el producto de la velocidad del fluido tiempo promedio y la duración de la eyección dividida por el diámetro del orificio. El tamaño óptimo de un anillo de vórtice se logra cuando VFT es 4 en vitro ya que se arrastra chorros y energéticas limitaciones le impiden alcanzar un mayor tamaño3,4. Válvula mitral VFT ha sido aproximada clínicamente mediante ecocardiografía transtorácica8,30,31. Basado en el análisis de la velocidad de flujo de la sangre trans-mitral y diámetro de la válvula mitral (D), puede ser fácilmente demostrado8 eso VFT = 4 × (1-β) × EF × α3, donde β = fracción de llenado auricular, fe = fracción de eyección del LV y α = EDV1/3/D, donde EDV = volumen telediastólico. Fracción de eyección es la proporción de volumen de movimiento (SV) y EDV, permitiendo esta ecuación simplificarse a VFT = 4 × del × (1-β) SV / (πD3). Porque VFT es adimensional (volumen/volumen), este índice permite la comparación directa entre pacientes de diferente tamaño sin ajuste para peso o cuerpo superficie8. Óptima oscila VFT entre 3,3 y 5,5 en sujetos sanos8y los resultados es consistente con los obtenidos en los modelos de dinámica de fluidos3,32. VFT fue demostrado para ser ≤ 2.0 en pacientes con función sistólica del VI deprimida, resultados que también son compatibles con las predicciones teóricas8. Reducciones en VFT independientemente predecían morbilidad y mortalidad en pacientes con insuficiencia cardíaca30. Elevada poscarga33de LV, la enfermedad de Alzheimer34, función diastólica anormal19y reemplazo de la válvula nativa mitral con una prótesis35 también han demostrado disminuir VFT. Medida de VFT también puede ser útil para identificar la estasis del flujo de sangre o trombosis en pacientes con infarto agudo de miocardio36,37.
Nuestro grupo está interesado en los factores que afectan LV eficiencia de llenado durante cirugía cardiaca38,39,40,41. Utilizamos el estándar bidimensional y Doppler transesofágica (ETE) para derivar de forma no invasiva las variables requeridas para calcular VFT. En este informe, describimos a esta metodología en detalle y revisión de nuestros resultados hasta la fecha.
Los resultados actuales ilustran VFT puede ser medido con fiabilidad durante la cirugía cardíaca utilizando las técnicas de t que se describe aquí.
