Les auteurs décrivent un protocole permettant de mesurer les temps de formation de vortex, un indice d’efficacité de remplissage du ventricule gauche, à l’aide de techniques d’échocardiographie transœsophagienne standard chez les patients subissant une chirurgie cardiaque. Nous appliquons cette technique pour analyser les temps de formation de vortex dans plusieurs groupes de patients atteints de différentes pathologies cardiaques.
Flux sanguin TRANS-mitrale produit un corps tridimensionnel de rotation du fluide, connu comme un anneau de vortex, qui améliore l’efficacité du ventricule gauche (VG) de remplissage par rapport à un jet continu linéaire. Développement de bague Vortex est plus souvent quantifiée avec temps de formation de vortex (VFT), un paramètre sans dimension issu d’ejection de fluide d’un tube rigide. Notre groupe s’intéresse à des facteurs qui influent sur les LV remplissage d’efficacité au cours de la chirurgie cardiaque. Dans ce rapport, nous décrivons comment utiliser standard à deux dimensions (2D) et Doppler échocardiographie transoesophagienne (ETO) de façon non invasive dériver les variables nécessaires au calcul de VFT. Nous calculons la fraction de remplissage auriculaire (β) de primitives de vitesse-temps de systole auriculaire sang écoulement vitesse formes d’ondes mesurés dans la vue TEE-oesophagien quatre chambres et trans-mitrale remplissage précoce de LV. Le volume systolique (SV) est calculé comme le produit du diamètre de la voie de sortie LV mesuré dans la vue TEE-oesophagien axe le plus long et l’intégrale de vitesse-temps de la circulation sanguine par l’intermédiaire de la voie de sortie déterminé selon la transgastric profonde à l’aide de la vague d’impulsion Doppler. Enfin, diamètre de la valve mitrale (D) correspond à la moyenne de l’axe majeur et mineur longueurs mesurées en bicommissural-oesophagien orthogonaux et plans d’imagerie axe long, respectivement. VFT est alors calculé comme 4 × × (1-β) SV / (πD3). Nous avons utilisé cette technique pour analyser la VFT dans plusieurs groupes de patients atteints d’anomalies cardiaques différentes. Nous examinons notre application de cette technique et ses limites possibles et également vérifier nos résultats à ce jour. Mesure non invasive de VFT utilisant TEE est simple chez les patients anesthésiés devant subir une chirurgie cardiaque. La technique peut permettre cardiaques anesthésistes et chirurgiens évaluer l’impact des pathologies et des interventions chirurgicales sur LV remplissant l’efficacité en temps réel.
Mécanique des fluides est un déterminant critique pourtant souvent sous-estimé de remplissage du ventricule gauche (VG). Un corps de rotation en trois dimensions du fluide, connu comme un anneau de vortex, est générée chaque fois qu’un fluide traverse un orifice1,2,3. Cet anneau de vortex améliore l’efficacité du transport des fluides en comparaison avec un jet linéaire continu4. Mouvement du sang par l’intermédiaire de la valve mitrale lors du remplissage de LV début provoque un anneau de vortex former5,6,7,8 et facilite sa propagation dans la chambre en préservant la dynamique fluide et énergie cinétique9. Ces actions renforcent LV remplissage efficacité4,10,11,12,13. L’anneau non seulement inhibe la stase du sang écoulement dans le LV apex14,15,16,17 mais aussi acheminent préférentiellement sous le feuillet mitral antérieur7, 18, effets qui diminuent le risque de formation de thrombus apical et de facilitent le remplissage des sorties LV piste19, respectivement. Échocardiographie de contraste17, Doppler vecteur flux cartographie6,20,21, imagerie par résonance magnétique7et particle imaging velocimetry9,22 ,23,,24 ont été utilisés pour démontrer l’apparence et le comportement des anneaux vortex trans-mitrale dans des conditions normales et pathologiques. Le gradient de pression auriculaire-LV gauche, le degré de diastolique excursion annulaire mitrale, la pression minimale de LV obtenue au cours de la diastole et le rythme et l’étendue de relaxation LV sont les quatre principaux déterminants de la durée, taille, intensité du courant et la position de l’anneau mitral-trans2,12,25,26,27,28,29.
Développement de bague Vortex est plus souvent quantifié avec un paramètre sans dimension (temps de formation de vortex ; VFT) issu d’ejection de fluide d’un tube rigide3, où VFT est défini comme le produit de la vitesse du fluide moyennée dans le temps et la durée d’éjection divisée par le diamètre de l’orifice. La taille optimale d’un anneau de vortex est atteint lorsque VFT est 4 in vitro car fuite jets et limitations énergiques il empêchent d’atteindre une plus grande taille3,4. Valve mitrale VFT a été approchée cliniquement à l’aide d’échocardiographie transthoracique8,30,31. Après analyse de la vitesse d’écoulement de sang trans-mitrale et la valve mitrale diamètre (D), il peut être facilement montré8 que VFT = 4 × (1-β) × EF × α3, où β = fraction de remplissage auriculaire, EF = fraction d’éjection du LV et α = EDV1/3/d, où EDV = volume télédiastolique. Fraction d’éjection est le rapport entre le volume systolique (SV) et EDV, permettant à cette équation à simplifier VFT = 4 × × (1-β) SV / (πD3). VFT étant sans dimension (volume/volume), cet indice permet une comparaison directe entre les patients de taille variable, sans réglage de poids ou corps superficie8. Optimal VFT oscille entre 3,3 et 5,5 en sujets sains8et les résultats concordent avec ceux obtenus dans la dynamique des fluides modèles3,32. VFT s’est avéré être ≤ 2.0 chez les patients atteints déprimée fonction systolique ventriculaire, conclusions qui sont également soutenues par les prédictions théoriques8. Réductions VFT prédisent indépendamment de morbidité et mortalité chez les patients atteints d’insuffisance cardiaque30. Surélevée LV postcharge33,34de la maladie d’Alzheimer, anormaux de la fonction diastolique19et remplacement de la valve mitrale native avec une prothèse35 ont aussi démontrés que diminuer VFT. Mesure de VFT peut également être utile pour identifier la stasis de flux de sang ou de thrombose chez les patients avec infarctus aigu du myocarde36,37.
Notre groupe s’intéresse à des facteurs qui influent sur les LV remplissage d’efficacité au cours de la chirurgie cardiaque38,39,40,41. Standard à deux dimensions et Doppler transœsophagienne (TEE) nous permet de dériver non-invasive les variables nécessaires pour calculer VFT. Dans ce rapport, nous décrire cette méthode en détail et revoir nos résultats à ce jour.
Les résultats actuels montrent que VFT peut être mesurée avec fiabilité au cours de la chirurgie cardiaque en utilisant les techniques TEE décrites ici. Les descriptions antérieures de VFT utilisé échocardiographie transthoracique chez les sujets conscients, mais cette approche ne peuvent pas être utilisée lorsque le coffre est ouvert.
