Hier presenteren we een protocol om orthotopische hepatocellulaire carcinoom xenografts met en zonder ligatie van de leverslagader te creëren en niet-invasieve positronemissietomografie (PET) beeldvorming van tumorhypoxie uit te voeren met behulp van [18 F] Fluoromisonidazol ([18 F] FMISO) en [18 F] Fluorodeoxyglucose ([18F] FDG).
Preklinische experimentele modellen van hepatocellulair carcinoom (HCC) die menselijke ziekten samenvatten, vormen een belangrijk hulpmiddel om tumorigenese te bestuderen en nieuwe therapeutische benaderingen te evalueren. Niet-invasieve beeldvorming van het hele lichaam met behulp van positronemissietomografie (PET) biedt kritische inzichten in de in vivo kenmerken van weefsels op moleculair niveau in realtime. We presenteren hier een protocol voor orthotopische HCC xenograft creatie met en zonder hepatische arterie ligatie (HAL) om tumorhypoxie te induceren en de beoordeling van hun tumormetabolisme in vivo met behulp van [18 F] Fluoromisonidazol ([18 F] FMISO) en [18 F] Fluorodeoxyglucose ([18F] FDG) PET / magnetische resonantie (MR) beeldvorming. Tumorhypoxie kon gemakkelijk worden gevisualiseerd met behulp van de hypoxiemarker [18 F] FMISO, en het bleek dat de [18 F] FMISO-opname hoger was bij HCC-muizen die HAL ondergingen dan in de niet-HAL-groep, terwijl [18F] FDG geen tumorhypoxie tussen de twee groepen kon onderscheiden. HAL-tumoren vertoonden ook een hoger niveau van hypoxie-induceerbare factor (HIF)-1α-expressie als reactie op hypoxie. Kwantificering van HAL-tumoren toonde een 2,3-voudige toename in [18F] FMISO-opname op basis van de gestandaardiseerde waardeopname (SUV) -benadering.
Hepatocellulair carcinoom (HCC) is de zesde meest gediagnosticeerde kanker en de derde meest voorkomende doodsoorzaak van kanker wereldwijd, met meer dan 900.000 nieuwe gevallen en 800.000 sterfgevallen in 20201. De belangrijkste risicofactor is cirrose, die optreedt als gevolg van virale infecties (hepatitis B- en C-virussen), alcoholmisbruik, diabetes en niet-alcoholische steatohepatitis2. Het beheer van HCC is vrij complex en er zijn verschillende behandelingsopties beschikbaar, waaronder chirurgische resectie, thermische of chemische ablatie, transplantatie, transarteriële chemo-embolisatie, bestraling en chemotherapie, afhankelijk van de stadiëring van de ziekte 2,3. HCC is een chemotherapie-refractaire tumor met een recidief van de ziekte bij maximaal 70% van de patiënten na curatieve behandeling2.
Ondanks de hoge mate van tumorheterogeniteit is HCC geassocieerd met twee veel voorkomende uitkomsten: (i) HCC is zeer hypoxisch en (ii) tumorhypoxie is gekoppeld aan grotere tumoragressiviteit en behandelingsfalen. De ongecontroleerde proliferatie van HCC-cellen resulteert in een hoog zuurstofverbruik dat voorafgaat aan vascularisatie, waardoor een hypoxische micro-omgeving ontstaat. Lage intra-tumorale zuurstofniveaus veroorzaken vervolgens een reeks biologische reacties die de agressiviteit van de tumor en de behandelingsrespons beïnvloeden. Hypoxie-induceerbare factoren (HIF’s) worden vaak erkend als de essentiële transcriptionele regulatoren in de respons op hypoxie 2,3. Daarom is het vermogen om hypoxie te detecteren cruciaal om neoplastische weefsels te visualiseren en de ontoegankelijke plaatsen te identificeren, die invasieve procedures vereisen. Het helpt ook om de moleculaire veranderingen die leiden tot tumoragressiviteit beter te begrijpen en de behandelingsresultaten van de patiënt te verbeteren.
Moleculaire beeldvorming met behulp van positronemissietomografie (PET) wordt vaak gebruikt bij de diagnose en stadiëring van veel kankers, waaronder HCC. Met name het gecombineerde gebruik van dual-tracer PET-beeldvorming met [18 F]Fluorodeoxyglucose ([18F]FDG) en [11C]Acetaat kan de algehele gevoeligheid bij HCC-diagnose aanzienlijk verhogen 4,5. Beeldvorming van hypoxie, aan de andere kant, kan worden bereikt met behulp van de veelgebruikte hypoxische marker [18 F] Fluoromisonidazol ([18F] FMISO). In de klinische praktijk is de niet-invasieve beoordeling van hypoxie belangrijk om onderscheid te maken tussen verschillende soorten tumoren en regio’s voor radiotherapieplanning6.
Preklinische beeldvorming is een onmisbaar hulpmiddel geworden voor de niet-invasieve en longitudinale evaluatie van muismodellen voor verschillende ziekten. Een robuust en zeer reproduceerbaar HCC-model vormt een belangrijk platform voor preklinisch en translationeel onderzoek naar de pathofysiologie van menselijke HCC en de beoordeling van nieuwe therapieën. Samen met PET-beeldvorming kunnen in vivo gedragingen worden opgehelderd om belangrijke inzichten op moleculair niveau te bieden voor een bepaald tijdspunt. Hier beschrijven we een protocol voor het genereren van hepatische arterieligatie (HAL) orthotopische HCC-xenografts en analyse van hun in vivo tumormetabolisme met behulp van [18 F] FMISO en [18F] FDG PET / MR. De integratie van HAL maakt een geschikt model van transgene of chemisch geïnduceerde HCC-muizen xenografts om tumorhypoxie in vivo te bestuderen, omdat HAL de arteriële bloedtoevoer effectief kan blokkeren om intratumorale hypoxie te induceren 7,8. Bovendien kunnen, in tegenstelling tot ex vivo immunohistochemische kleuring met pimonidazol, veranderingen in het tumormetabolisme als gevolg van hypoxie gemakkelijk worden gevisualiseerd en nauwkeurig niet-invasief worden gekwantificeerd met behulp van PET-beeldvorming, waardoor longitudinale beoordeling van de behandelingsrespons of meting van het ontstaan van resistentie mogelijk is 3,7,8 . Onze hier getoonde methode maakt het mogelijk om een robuust hypoxisch HCC-model te maken, samen met niet-invasieve monitoring van tumorhypoxie met behulp van PET / MR-beeldvorming om HCC-biologie in vivo te bestuderen.
In deze studie beschreven we de procedures om HAL uit te voeren op leverorthotopische HCC-xenografts met behulp van subcutane tumoren, samen met methoden voor de niet-invasieve monitoring van tumorhypoxie in orthotopische xenografts met behulp van [18 F] FMISO en [18F] FDG PET / MR.
