Um die Ausbreitung antimikrobiell resistenter Infektionen zu verlangsamen und zu verhindern, ist es dringend erforderlich, die antimikrobiellen Wirkungen auf Krankheitserreger quantitativ zu bestimmen. Es dauert in der Regel Tage, um die AST mit herkömmlichen Methoden abzuschließen, die auf der Langzeitkultur basieren, und sie funktionieren nicht direkt für klinische Proben. Hier berichten wir über eine schnelle AST-Methode, die durch stimulierte Raman-Streuung (SRS) -Bildgebung der Deuteriumoxid (D2O) metabolischen Inkorporation ermöglicht wird. Der metabolische Einbau vonD2Oin Biomasse und die Hemmung der Stoffwechselaktivität bei Exposition gegenüber Antibiotika auf Einzelbakterienebene werden durch SRS-Bildgebung überwacht. Die Einzelzellstoffwechsel-Inaktivierungskonzentration (SC-MIC) von Bakterien bei Exposition gegenüber Antibiotika kann nach insgesamt 2,5 Stunden Probenvorbereitung und -nachweis ermittelt werden. Darüber hinaus ist diese schnelle AST-Methode direkt auf Bakterienproben in komplexen biologischen Umgebungen wie Urin oder Vollblut anwendbar. Die SRS-metabolische Bildgebung der Deuteriuminkorporation ist transformativ für eine schnelle einzellige phänotypische AST in der Klinik.
Antimikrobielle Resistenz (AMR) ist eine wachsende globale Bedrohung für die wirksame Behandlung von Infektionskrankheiten1. Es wird prognostiziert, dass AMR bis 2050 zusätzliche 10 Millionen Todesfälle pro Jahr und einen globalen BIP-Verlust von 100 Billionen US-Dollar verursachen wird, wenn keine Maßnahmen zur Bekämpfung antibiotikaresistenter Bakterien ergriffen werden 1,2. Dies unterstreicht die dringende Notwendigkeit schneller und innovativer Diagnosemethoden für Antibiotika-Empfindlichkeitstests (AST) von infektiösen Bakterien, um das Auftreten antibiotikaresistenter Bakterien zu verlangsamen und die damit verbundene Sterblichkeitsrate zu senken3. Um das bestmögliche klinische Ergebnis zu gewährleisten, ist es entscheidend, innerhalb von 24 Stunden eine wirksame Therapie einzuleiten. Die aktuelle Goldstandardmethode, wie die Scheibendiffusions- oder Brüheverdünnungsmethode, erfordert jedoch in der Regel mindestens 24 Stunden für das Vorinkubationsverfahren für klinische Proben und zusätzliche 16-24 Stunden, um die Ergebnisse der minimalen Hemmkonzentration (MIC) zu erhalten. Insgesamt sind diese Methoden zu zeitaufwendig, um eine sofortige Entscheidung für die Behandlung von Infektionskrankheiten in der Klinik zu treffen, was zur Entstehung und Ausbreitung von Antibiotikaresistenzen führt4.
Genotypische AST-Methoden, wie Polymerase-Kettenreaktion (PCR)-basierte Techniken5, wurden für den schnellen Nachweis entwickelt. Solche Techniken messen die spezifischen Resistenz-Gensequenzen, um schnelle AST-Ergebnisse zu liefern. Sie sind nicht auf zeitaufwändige Zellkulturen angewiesen; Es werden jedoch nur bestimmte bekannte genetische Sequenzen mit Resistenz getestet. Daher ist seine Anwendung auf verschiedene Bakterienarten oder unterschiedliche Resistenzmechanismen beschränkt. Sie können auch keine MIC-Ergebnisse für Therapieentscheidungen liefern 6,7. Darüber hinaus werden neuartige phänotypische Methoden für schnelle AST entwickelt, um diese Einschränkungen zu überwinden8, einschließlich mikrofluidischer Geräte 9,10,11,12,13, optischer Geräte14,15,16, phänotypischer AST zur Quantifizierung der Nukleinsäure-Kopienzahl17,18 und Raman-spektroskopischer Methoden 19. 20,21,22,23,24. Diese Methoden verkürzen die Zeit, um AST-Ergebnisse zu steuern, aber die meisten von ihnen sind nur auf Bakterienisolate anwendbar, nicht direkt auf klinische Proben, und erfordern immer noch eine lange Vorinkubation.
In dieser Arbeit stellen wir eine Methode zur schnellen Bestimmung der Empfindlichkeit von Bakterien im Urin und Vollblut durch Überwachung der zellulären Stoffwechselaktivität mittels SRS-Bildgebung vor. Wasser (H2O) ist an der überwiegenden Mehrheit der wesentlichen biomolekularen Syntheseprozesse in lebenden Zellen beteiligt. Als Isotopologe von Wasser kann Deuterium durch enzymkatalysierte H/D-Austauschreaktion zwischen dem redoxaktiven Wasserstoffatom in NADPH und demD-Atom in D2Oin Biomasse innerhalb einer Zelle25,26 eingebaut werden. Eine deuterierte Fettsäuresynthesereaktion wird durch das mit Deuterium markierte NADPH vermittelt. DerD2O-Einbauin Reaktionen von Aminosäuren (AAs) führt zur deuterierten Proteinproduktion26 (Abbildung 1). Auf diese Weise können die neu synthetisierten C-D-Bindungs-haltigen Biomoleküle in einzelnen mikrobiellen Zellen als allgemeiner metabolischer Aktivitätsmarker verwendet werden, um nachgewiesen zu werden. Um de novo synthetisierte C-D-Bindungen auszulesen, wird die Raman-Spektroskopie, ein vielseitiges Analysewerkzeug, das spezifische und quantitative chemische Informationen über Biomoleküle liefert, häufig verwendet, um die antimikrobielle Empfindlichkeit zu bestimmen und die Testzeit auf wenige Stunden deutlich zu reduzieren27,28,29,30 . Aufgrund der inhärenten geringen Effizienz des Raman-Streuprozesses ist die spontane Raman-Spektroskopie jedoch von geringer Detektionsempfindlichkeit. Daher ist es schwierig, Echtzeit-Bildergebnisse mittels spontaner Raman-Spektroskopie zu erhalten. Die kohärente Raman-Streuung (CRS), einschließlich der kohärenten Anti-Stokes-Raman-Streuung (CARS) und der stimulierten Raman-Streuung (SRS), hat aufgrund des kohärenten Lichtfeldes eine hohe Detektionsempfindlichkeit erreicht, die um Größenordnungen größer ist als die spontane Raman-Spektroskopie, wodurch eine schnelle, spezifische und quantitative chemische Bildgebung auf Einzelzellebene ermöglichtwird 31,32,33,34,35 ,36,37,38,39.
Hier, basierend auf unserer jüngsten Arbeit40, präsentieren wir ein Protokoll zur schnellen Bestimmung der metabolischen Aktivität und antimikrobiellen Suszeptibilität durch Femtosekunden-SRS-C-D-Bildgebung vonD2O-Einbauvon Bakterien in das normale Medium, Urin und Vollblutumgebung auf Einzelzellebene. Die Femtosekunden-SRS-Bildgebung ermöglicht die Überwachung der Inaktivierungskonzentration des Einzelzellstoffwechsels (SC-MIC) gegen Antibiotika auf Einzelbakteriumebene innerhalb von 2,5 h. Die SC-MIC-Ergebnisse werden durch Standard-MIC-Test mittels Brühe-Mikrodilution validiert. Unsere Methode ist anwendbar zur Bestimmung der antimikrobiellen Anfälligkeit von bakteriellen Harnwegsinfektionen (UTI) und Blutbahninfektionen (BSI) Erregern mit einer viel kürzeren Assay-Zeit im Vergleich zur konventionellen Methode, was die Möglichkeit für eine schnelle phänotypische AST in der Klinik auf Einzelzellebene eröffnet.
Eine schnelle AST kann durch die Beurteilung der Reaktion der bakteriellen Stoffwechselaktivität auf die Antibiotikabehandlung unter Verwendung von Einzelzell-SRS-Stoffwechselbildgebung innerhalb von 2,5 Stunden von der Probe bis zu SC-MIC-Ergebnissen erreicht werden.
