Dit protocol presenteert een snelle antimicrobiële gevoeligheidstest (ASAT) binnen 2,5 uur door eencellige gestimuleerde Raman-verstrooiingsbeeldvorming van het D2O-metabolisme. Deze methode is van toepassing op bacteriën in de urine of volbloedomgeving, die transformerend is voor snelle eencellige fenotypische ASAT in de kliniek.
Om de verspreiding van antimicrobieel resistente infecties te vertragen en te voorkomen, is het dringend noodzakelijk om de antimicrobiële effecten op pathogenen kwantitatief te bepalen. Het duurt meestal dagen om de AST te voltooien met conventionele methoden op basis van de langdurige cultuur, en ze werken niet direct voor klinische monsters. Hier rapporteren we een snelle AST-methode die mogelijk wordt gemaakt door gestimuleerde Raman-verstrooiing (SRS) beeldvorming van deuteriumoxide (D2O) metabole opname. Metabole opname van D2O in biomassa en de metabole activiteitsremming bij blootstelling aan antibiotica op het niveau van één bacterie worden gemonitord door SRS-beeldvorming. De single-cell metabolism inactivation concentration (SC-MIC) van bacteriën bij blootstelling aan antibiotica kan worden verkregen na een totaal van 2,5 uur monstervoorbereiding en -detectie. Bovendien is deze snelle AST-methode direct toepasbaar op bacteriële monsters in complexe biologische omgevingen, zoals urine of volbloed. SRS metabole beeldvorming van deuteriumincorporatie is transformerend voor snelle eencellige fenotypische ASAT in de kliniek.
Antimicrobiële resistentie (AMR) is een groeiende wereldwijde bedreiging voor de effectieve behandeling van infectieziekten1. Er wordt voorspeld dat AMR tegen 2050 nog eens 10 miljoen doden per jaar en $ 100 biljoen wereldwijd bbp-verlies zal veroorzaken als er geen actie wordt ondernomen voor de bestrijding van antibioticaresistente bacteriën 1,2. Dit benadrukt de dringende behoefte aan snelle en innovatieve diagnostische methoden voor antibioticagevoeligheidstests (ASAT) van infectieuze bacteriën om de opkomst van antibioticaresistente bacteriën te vertragen en het gerelateerde sterftecijfer te verminderen3. Om het best mogelijke klinische resultaat te garanderen, is het cruciaal om binnen 24 uur effectieve therapie te introduceren. De huidige gouden standaardmethode, zoals schijfdiffusie of bouillonverdunningsmethode, vereist echter meestal ten minste 24 uur voor de pre-cubatieprocedure voor klinische monsters en een extra 16-24 uur om de resultaten van de minimale remmende concentratie (MIC) te verkrijgen. Over het algemeen zijn deze methoden te tijdrovend om een onmiddellijke beslissing voor infectieziektebehandeling in de kliniek te begeleiden, wat leidt tot het ontstaan en de verspreiding van antimicrobiële resistentie4.
Genotypische AST-methoden, zoals polymerasekettingreactie (PCR)-gebaseerde technieken5, zijn ontwikkeld voor snelle detectie. Dergelijke technieken meten de specifieke resistentie genetische sequenties om snelle AST-resultaten te verkrijgen. Ze zijn niet afhankelijk van tijdrovende celkweek; alleen specifieke bekende genetische sequenties met resistentie worden echter getest. Daarom is de toepassing ervan beperkt tot verschillende bacteriesoorten of verschillende resistentiemechanismen. Ook kunnen ze geen MIC-resultaten leveren voor therapiebeslissingen 6,7. Bovendien zijn er nieuwe fenotypische methoden voor snelle ASAT in ontwikkeling om deze beperkingen te overwinnen8, waaronder microfluïdische apparaten 9,10,11,12,13, optische apparaten 14,15,16, fenotypische AST die de nucleïnezuren kwantificeert kopie nummer 17,18, en Raman spectroscopische methoden19, 20,21,22,23,24. Deze methoden verkorten de tijd om AST-resultaten te begeleiden, maar de meeste zijn alleen van toepassing op bacteriële isolaten, niet rechtstreeks op klinische monsters, en vereisen nog steeds langdurige pre-incubatie.
In dit werk presenteren we een methode voor snelle bepaling van de gevoeligheid van bacteriën in de urine en volbloed via monitoring van de cellulaire metabole activiteit door SRS-beeldvorming. Water (H2O) neemt deel aan de overgrote meerderheid van essentiële biomoleculaire syntheseprocessen in levende cellen. Als een isotopoloog van water, door middel van een enzymgekatalyseerde H/D-uitwisselingsreactie tussen het redox-actieve waterstofatoom in NADPH en het D-atoom in D2O, kan deuterium worden opgenomen in biomassa in een cel25,26. Een gedeutereerde vetzuursynthesereactie wordt gemedieerd door het deuterium gelabelde NADPH. De D2O-opname in reacties van aminozuren (AA’s) resulteert in de gedeutereerde eiwitproductie26 (figuur 1). Op deze manier kunnen de nieuw gesynthetiseerde C-D-bindingsbevattende biomoleculen in enkele microbiële cellen worden gebruikt als een algemene metabole activiteitsmarker die moet worden gedetecteerd. Om de novo gesynthetiseerde C-D-bindingen uit te lezen, wordt Raman-spectroscopie, een veelzijdig analytisch hulpmiddel dat specifieke en kwantitatieve chemische informatie over biomoleculen biedt, veel gebruikt om antimicrobiële gevoeligheid te bepalen en de testtijd aanzienlijk te verkorten tot een paar uur 27,28,29,30 . Vanwege de inherente lage efficiëntie van het Raman-verstrooiingsproces is de spontane Raman-spectroscopie echter van lage detectiegevoeligheid. Daarom is het een uitdaging om real-time beeldresultaten te verkrijgen met behulp van spontane Raman-spectroscopie. Coherente Raman-verstrooiing (CRS), inclusief coherente anti-Stokes Raman-verstrooiing (CARS) en gestimuleerde Raman-verstrooiing (SRS), heeft een hoge detectiegevoeligheid bereikt vanwege het coherente lichtveld om ordes van grootte te genereren die groter zijn dan die van spontane Raman-spectroscopie, waardoor snelle, specifieke en kwantitatieve chemische beeldvorming op het niveau van één cel 31,32,33,34,35 wordt weergegeven ,36,37,38,39.
Hier, op basis van ons meest recente werk40, presenteren we een protocol voor snelle bepaling van de metabole activiteit en antimicrobiële gevoeligheid door femtoseconde SRS C-D beeldvorming van D2O opname van bacteriën in het normale medium, urine en volbloedomgeving op het niveau van één cel. Femtoseconde SRS-beeldvorming vergemakkelijkt het monitoren van de single cell metabolism inactivation concentration (SC-MIC) tegen antibiotica op het niveau van de enkele bacterie binnen 2,5 uur. De SC-MIC-resultaten worden gevalideerd door standaard MIC-test via bouillonmicroverdunning. Onze methode is toepasbaar voor het bepalen van antimicrobiële gevoeligheid van bacteriën urineweginfectie (UTI) en bloedbaaninfectie (BSI) pathogenen met een veel kortere testtijd in vergelijking met de conventionele methode, die de mogelijkheid opent voor snelle fenotypische AST in de kliniek op het niveau van één cel.
Snelle ASAT kan worden verkregen door de respons van bacteriële metabole activiteit op antibioticabehandeling te beoordelen met behulp van eencellige SRS metabole beeldvorming binnen 2,5 uur van het monster tot SC-MIC-resultaten. De respons van bacteriële metabole activiteit en antimicrobiële gevoeligheid kan worden gedetecteerd door de metabole opname van D2O voor biomolecuulsynthese te monitoren met behulp van SRS-beeldvorming van C-D-bindingen.
