Dit protocol onderzoekt het gebruik van extracellulair vesikel (EV)-rijk plasma als indicator van het stollingsvermogen van EV. EV-rijk plasma wordt verkregen door een proces van differentiële centrifugatie en daaropvolgende recalcificatie.
De rol van extracellulaire blaasjes (EV) bij verschillende ziekten krijgt steeds meer aandacht, met name vanwege hun krachtige procoagulerende activiteit. Er is echter dringend behoefte aan een bedtest om de procoagulerende activiteit van EV in klinische omgevingen te beoordelen. Deze studie stelt het gebruik van trombine-activeringstijd van EV-rijk plasma voor als een maat voor de procoagulerende activiteit van EV. Gestandaardiseerde procedures werden gebruikt om natriumgecitreerd volbloed te verkrijgen, gevolgd door differentiële centrifugatie om EV-rijk plasma te verkrijgen. Het EV-rijke plasma en calciumchloride werden aan de testbeker toegevoegd en de veranderingen in visco-elasticiteit werden in realtime gevolgd met behulp van een analysator. De natuurlijke stollingstijd van EV-rijk plasma, aangeduid als EV-ACT, werd bepaald. De resultaten toonden een significante toename van EV-ACT wanneer EV werd verwijderd uit plasma verkregen van gezonde vrijwilligers, terwijl het significant afnam wanneer EV werd verrijkt. Bovendien werd EV-ACT aanzienlijk verkort in menselijke monsters van pre-eclampsie, heupfracturen en longkanker, wat wijst op verhoogde niveaus van plasma-EV en bevordering van hypercoagulatie in het bloed. Met zijn eenvoudige en snelle procedure is EV-ACT veelbelovend als test aan het bed voor het evalueren van de stollingsfunctie bij patiënten met hoge plasma-EV-spiegels.
Trombose, die wordt veroorzaakt door hypercoagulabiliteit, speelt een belangrijke rol bij verschillende ziekten, waaronder hersentrauma1, pre-eclampsie2, tumoren3 en fractuurpatiënten4. Het mechanisme dat ten grondslag ligt aan hypercoagulabiliteit is complex en onlangs is de nadruk gelegd op de rol van extracellulaire blaasjes (EV) bij stollingsstoornissen. EV’s zijn blaasjesachtige lichamen met een dubbellaagse structuur die loskomen van het celmembraan, variërend in diameter van 10 nm tot 1000 nm. Ze worden in verband gebracht met een verscheidenheid aan ziekteprocessen, met name stollingsstoornissen5. Verschillende onderzoeken hebben EV’s geïdentificeerd als een veelbelovende voorspeller van het risico op trombose 6,7. De procoagulerende activiteit van EV’s hangt af van de expressie van stollingsfactoren, voornamelijk weefselfactor (TF) en fosfatidylserine (PS). EV’s met een robuuste procoagulerende activiteit verbeteren de katalytische efficiëntie van tenase en protrombinecomplex aanzienlijk, waardoor trombine-gemedieerd fibrinogeen en lokale trombose worden bevorderd8. Verhoogde niveaus van EV’s en hun causale verband met hypercoagulabiliteit zijn waargenomen bij tal vanziekten9. Bijgevolg is het standaardiseren van de detectie van EV’s en het rapporteren van hun procoagulerende activiteit een belangrijk onderzoeksgebied10.
Tot op heden zijn er slechts enkele commerciële kits beschikbaar voor het detecteren van de procoagulerende activiteit van EV’s. De MP-Activity-test en de MP-TF-assay, geproduceerd door een commercieel bedrijf, zijn functionele tests die worden gebruikt om de procoagulerende activiteit van EV in plasma11 te meten. Deze tests maken gebruik van een principe dat vergelijkbaar is met dat van enzymgekoppelde immunosorbenttests om PS en TF op EV’s te detecteren. Deze kits zijn echter duur en beperkt tot een paar onderzoeksinstellingen op hoog niveau. Het proces is complex en tijdrovend, waardoor het een uitdaging is om ze in klinische omgevingen te implementeren. Bovendien mengt een commercieel ontwikkelde procoagulerende fosfolipide (PPL)-assay PS-vrij plasma met testplasma, waarbij de stollingstijd wordt gemeten om kwantitatief niveaus van PS-positieve EV’s te detecteren12. Deze tests richten zich echter voornamelijk op PS en TF op EV’s, waarbij andere stollingsroutes over het hoofd worden gezien waarbij circulerende EV’s betrokken kunnen zijn12.
Het plasmastollingssysteem is ingewikkeld en omvat zowel “onzichtbare” als “zichtbare” componenten, waaronder stollingsmiddelen, anticoagulantia, fibrinolytische systemen en EV’s die in het plasma zijn gesuspendeerd. Fysiologisch zorgen deze componenten voor een dynamisch evenwicht. Bij pathologische aandoeningen dragen aanzienlijk verhoogde EV’s in de circulatie bij aan hypercoagulabiliteit, met name bij patiënten met hersentrauma, pre-eclampsie, fracturen en verschillende soorten kanker13. Momenteel omvat de evaluatie van de stollingsstatus in klinische laboratoria voornamelijk het beoordelen van het stollingssysteem, het antistollingssysteem en fibrinolyse 14,15,16,17. Protrombinetijd, geactiveerde partiële tromboplastinetijd, trombinetijd en internationale genormaliseerde ratio worden vaak gebruikt om stollingsfactorniveaus in het stollingssysteem te evalueren18. Recente studies hebben echter aangetoond dat deze tests de hypercoagulabiliteit van bepaalde ziekten niet volledig weerspiegelen19. Andere testmethoden, zoals trombo-elastometrie (TEG), rotatie-TEG en sonoclotanalyse, meten visco-elastische veranderingen in volbloed20,21. Aangezien volbloedmonsters veel bloedcellen en bloedplaatjes bevatten, is de kans groter dat deze tests de stollingsstatus van het monster als geheel aangeven. Sommige onderzoekers hebben gerapporteerd over de rol van bloedcellen en bloedplaatjes bij procoagulantia-activiteit22,23. Een recente studie ontdekte ook dat eerdere stollingsfunctietests moeilijkheden ondervinden bij het detecteren van veranderingen in de procoagulerende activiteit van microdeeltjes24. Daarom is een hypothese geopperd dat de procoagulerende functie van EV’s kan worden geëvalueerd door visco-elastische metingen van de geactiveerde stollingstijd (ACT) in EV-rijk plasma.
In deze studie werd de bereiding van EV-rijk plasma beschreven en werd de rationaliteit van de methode geverifieerd met behulp van flowcytometrie. Vervolgens werden de opnieuw verkalkte plasmamonsters geanalyseerd op ACT-tijd met behulp van een stolselanalysator op basis van visco-elasticiteitsprincipes24. Zoals te zien is in figuur 3A, bleek de concentratie EV’s verkregen door ultracentrifugatie de EV-ACT-tijd te verkorten, terwijl het supernatans na ultracentrifugat…
