Point-of-care echografie evaluatie van de inferieure vena cava (IVC) wordt vaak gebruikt om onder andere de volumestatus te identificeren. Beeldvorming moet systematisch worden uitgevoerd om herhaalbaarheid te garanderen. Dit manuscript bespreekt de methoden en valkuilen van echografisch IVC-onderzoek.
In de afgelopen decennia hebben clinici verschillende toepassingen van diagnostische point-of-care echografie (POCUS) opgenomen in de medische besluitvorming. Onder de toepassingen van POCUS wordt beeldvorming van de inferieure vena cava (IVC) beoefend door een breed scala aan specialiteiten, zoals nefrologie, spoedeisende geneeskunde, interne geneeskunde, kritieke zorg, anesthesiologie, pulmonologie en cardiologie. Hoewel elke specialiteit IVC-gegevens op enigszins verschillende manieren gebruikt, proberen de meeste medische specialiteiten op zijn minst IVC-gegevens te gebruiken om voorspellingen te doen over de intravasculaire volumestatus. Hoewel de relatie tussen IVC-echografische gegevens en intravasculaire volumestatus complex en zeer contextafhankelijk is, moeten alle clinici de echografische gegevens op gestandaardiseerde manieren verzamelen om herhaalbaarheid te garanderen. Dit artikel beschrijft gestandaardiseerde IVC-beeldacquisitie, inclusief patiëntpositionering, transducerselectie, sondeplaatsing, beeldoptimalisatie en de valkuilen en beperkingen van IVC-echografische beeldvorming. Dit artikel beschrijft ook de vaak uitgevoerde anterieure IVC-lange-asweergave en drie andere weergaven van de IVC die elk nuttige diagnostische informatie kunnen bieden wanneer de voorste lange-asweergave moeilijk te verkrijgen of te interpreteren is.
In de afgelopen decennia is de toegankelijkheid van point-of-care echografie (POCUS) dramatisch toegenomen. Zorgverleners in verschillende medische disciplines kunnen POCUS nu integreren in hun examens aan het bed en gemakkelijker belangrijke bijdragers aan de aandoeningen van patiënten identificeren1. In acute zorgomgevingen is een van de belangrijkste aandachtsgebieden bijvoorbeeld de beoordeling en het beheer van volumestatus2. Onvoldoende vloeistofreanimatie kan leiden tot weefselhypoperfusie, eindorgaandisfunctie en ernstige zuur-baseafwijkingen. Overijverige vloeistoftoediening is echter geassocieerd met verergerde mortaliteit3. De bepaling van de volumestatus is voornamelijk bereikt met behulp van de combinatie van bevindingen van lichamelijk onderzoek en dynamische hemodynamische metingen, waaronder pulsdrukvariatie, centrale veneuze druk en / of vloeistofuitdagingen via passieve beenverhogingstests of intraveneuze vloeistofbolussen4. Met de groeiende beschikbaarheid van POCUS-apparaten proberen sommige providers echografie te gebruiken om deze maatregelen aan te vullen5. De echografische beoordeling van de anterieure-naar-posterieure dimensie van de IVC en de respirofasische verandering in die dimensie kan helpen bij de beoordeling van de rechter atriale druk en mogelijk de intravasculaire volumestatus 6,7,8,9.
Met name is echter de relatie tussen IVC-parameters (d.w.z. grootte en respirofasische verandering) en volumeresponsiviteit vervormd in veel voorkomende situaties, waaronder, maar niet beperkt tot, het volgende: (1) passief beademde patiënten die ofwel hoge positieve eind-expiratoire druk (PEEP) of lage getijdenvolumes ontvangen; (2) spontaan ademende patiënten die een kleine of grote ademhalingsinspanning leveren; (3) longhyperinflatie; (4) aandoeningen die de veneuze terugkeer belemmeren (bijv. rechterventrikeldisfunctie, spanningspneumothorax, harttamponade, enz.); en (5) verhoogde abdominale compartdruk10.
Hoewel het nut van IVC-echografie als een op zichzelf staande maat voor het beoordelen van de intravasculaire volumestatus wordt besproken 5,10,11,12, is er geen discussie over het feit dat het gebruik ervan als diagnostisch hulpmiddel beeldvorming op gestandaardiseerde manieren vereist en het vermogen om alternatieve opvattingen te gebruiken wanneer een enkel gezichtspunt ontoereikend blijkt te zijn 2 . Daartoe definieert dit manuscript de vier echografische weergaven van het IVC, illustreert het veelvoorkomende sonografische valkuilen en hoe deze te vermijden, en biedt het voorbeelden van zowel typische als extreme IVC-echografische toestanden. Er zijn vier weergaven waarin de IVC adequaat kan worden gevisualiseerd door transabdominale echografie: voorste korte as, voorste lange as, rechter laterale lange as en rechter laterale korte as. Het onderstaande protocol beschrijft een gestandaardiseerde methode van beeldacquisitie.
Zelfs wanneer het goed in beeld wordt gebracht, mag informatie die uit het IVC wordt verzameld niet het enige gegevenspunt zijn dat wordt gebruikt voor het begeleiden van de behandeling. Exact dezelfde IVC-grootte en respirofasische veranderingen kunnen worden gezien in zowel normale toestanden als in pathologische omstandigheden.
