Door de mens geïnduceerde pluripotente van stamcellen afgeleide cardiomyocyten (hiPSC-CM’s) zijn naar voren gekomen als een veelbelovend in vitro model voor door geneesmiddelen geïnduceerde cardiotoxiciteitsscreening en ziektemodellering. Hier beschrijven we een protocol voor het meten van de contractiliteit en elektrofysiologie van hiPSC-CM’s.
Geneesmiddel-geïnduceerde cardiotoxiciteit is de belangrijkste oorzaak van drugsverloop en terugtrekking uit de markt. Daarom is het gebruik van geschikte preklinische cardiale veiligheidsbeoordelingsmodellen een cruciale stap tijdens de ontwikkeling van geneesmiddelen. Momenteel is de beoordeling van de cardiale veiligheid nog steeds sterk afhankelijk van dierstudies. Diermodellen worden echter geplaagd door een slechte translationele specificiteit voor mensen als gevolg van soortspecifieke verschillen, met name in termen van cardiale elektrofysiologische kenmerken. Er is dus dringend behoefte aan de ontwikkeling van een betrouwbaar, efficiënt en op mensen gebaseerd model voor preklinische cardiale veiligheidsbeoordeling. Door de mens geïnduceerde pluripotente van stamcellen afgeleide cardiomyocyten (hiPSC-CMs) zijn naar voren gekomen als een waardevol in vitro model voor door geneesmiddelen geïnduceerde cardiotoxiciteitsscreening en ziektemodellering. hiPSC-CMs kunnen worden verkregen van personen met verschillende genetische achtergronden en verschillende zieke aandoeningen, waardoor ze een ideaal surrogaat zijn om door geneesmiddelen geïnduceerde cardiotoxiciteit individueel te beoordelen. Daarom moeten methodologieën worden vastgesteld om de functionele kenmerken van hiPSC-CMs uitgebreid te onderzoeken. In dit protocol beschrijven we verschillende functionele assays die kunnen worden beoordeeld op hiPSC-CM’s, waaronder de meting van contractiliteit, veldpotentiaal, actiepotentiaal en calciumbehandeling. Over het algemeen heeft de integratie van hiPSC-CM’s in preklinische cardiale veiligheidsbeoordeling het potentieel om een revolutie teweeg te brengen in de ontwikkeling van geneesmiddelen.
De ontwikkeling van geneesmiddelen is een lang en duur proces. Een studie van nieuwe therapeutische geneesmiddelen goedgekeurd door de Amerikaanse Food and Drug Administration (FDA) tussen 2009 en 2018 meldde dat de geschatte mediane kosten van gekapitaliseerd onderzoek en klinische proeven $ 985 miljoen per productbedroegen 1. Geneesmiddel-geïnduceerde cardiotoxiciteit is de belangrijkste oorzaak van drugsverloop en terugtrekking uit de markt2. Met name cardiotoxiciteit wordt gemeld onder meerdere klassen van therapeutische geneesmiddelen3. Daarom is cardiale veiligheidsbeoordeling een cruciaal onderdeel tijdens het ontwikkelingsproces van geneesmiddelen. Het huidige paradigma voor cardiale veiligheidsbeoordeling is nog steeds sterk afhankelijk van diermodellen. Soortverschillen met het gebruik van diermodellen worden echter steeds meer erkend als een primaire oorzaak van onnauwkeurige voorspellingen voor door geneesmiddelen geïnduceerde cardiotoxiciteit bij menselijke patiënten4. De morfologie van cardiale actiepotentiaal verschilt bijvoorbeeld aanzienlijk tussen mensen en muizen vanwege de bijdragen van verschillende repolariserende stromen5. Bovendien zijn differentiële isovormen van cardiale myosine en circulaire RNA’s die de hartfysiologie kunnen beïnvloeden goed gedocumenteerd bij soort 6,7. Om deze lacunes te overbruggen, is het noodzakelijk om een betrouwbaar, efficiënt en op mensen gebaseerd model op te stellen voor preklinische cardiale veiligheidsbeoordeling.
De baanbrekende uitvinding van geïnduceerde pluripotente stamceltechnologie (iPSC) heeft ongekende platforms voor geneesmiddelenscreening en ziektemodellering gegenereerd. In het afgelopen decennium zijn methoden om door de mens geïnduceerde pluripotente van stamcellen afgeleide cardiomyocyten (hiPSC-CMs) te genereren, goed ingeburgerdgeworden 8,9. hiPSC-CMs hebben grote belangstelling getrokken voor hun potentiële toepassingen in ziektemodellering, door geneesmiddelen geïnduceerde cardiotoxiciteitsscreening en precisiegeneeskunde. HiPSC-CM’s zijn bijvoorbeeld gebruikt om de pathologische fenotypen van hartaandoeningen veroorzaakt door genetische overerving te modelleren, zoals lange QT-syndroom10, hypertrofische cardiomyopathie11,12 en gedilateerde cardiomyopathie 13,14,15. Bijgevolg zijn belangrijke signaalroutes geïdentificeerd die betrokken zijn bij de pathogenese van hartaandoeningen, die licht kunnen werpen op potentiële therapeutische strategieën voor een effectieve behandeling. Bovendien zijn hiPSC-CM’s gebruikt om door geneesmiddelen geïnduceerde cardiotoxiciteit geassocieerd met middelen tegen kanker te screenen, waaronder doxorubicine, trastuzumab en tyrosinekinaseremmers 16,17,18; strategieën om de resulterende cardiotoxiciteit te verminderen worden onderzocht. Ten slotte maakt de genetische informatie die wordt bewaard in hiPSC-CMs de screening en voorspelling van door geneesmiddelen geïnduceerde cardiotoxiciteit mogelijk op zowel individueel als populatieniveau19,20. Gezamenlijk hebben hiPSC-CMs bewezen een waardevol hulpmiddel te zijn voor gepersonaliseerde cardiale veiligheidsvoorspelling.
Het algemene doel van dit protocol is om methodologieën vast te stellen om de functionele kenmerken van hiPSC-CMs uitgebreid en efficiënt te onderzoeken, die van groot belang zijn bij het toepassen van hiPSC-CM’s voor ziektemodellering, door geneesmiddelen geïnduceerde cardiotoxiciteitsscreening en precisiegeneeskunde. Hier beschrijven we een reeks functionele testen om de functionele eigenschappen van hiPSC-CM’s te beoordelen, inclusief de meting van contractiliteit, veldpotentiaal, actiepotentiaal en calcium (Ca2 +) handling (figuur 1).
Menselijke iPSC-technologie is naar voren gekomen als een krachtig platform voor ziektemodellering en medicijnscreening. Hier beschrijven we een gedetailleerd protocol voor het meten van hiPSC-CM contractiliteit, veldpotentiaal, actiepotentiaal en Ca2+ transiënt. Dit protocol biedt een uitgebreide karakterisering van hiPSC-CM contractiliteit en elektrofysiologie.
