Het maagdarmkanaal is een van de meest gevoelige organen voor letsel bij radiotherapeutische kankerbehandelingen. Het is tegelijkertijd een orgaansysteem met een van de hoogste regeneratieve capaciteiten na dergelijke beledigingen. Het gepresenteerde protocol beschrijft een efficiënte methode om het regeneratieve vermogen van het darmepitheel te bestuderen.
Het darmepitheel bestaat uit een enkele laag cellen, maar bevat meerdere soorten terminaal gedifferentieerde cellen, die worden gegenereerd door de actieve proliferatie van darmstamcellen op de bodem van darmcrypten. Tijdens gebeurtenissen van acuut darmletsel ondergaan deze actieve darmstamcellen echter celdood. Gammabestraling is een veel gebruikte colorectale kankerbehandeling, die, hoewel therapeutisch effectief, de bijwerking heeft van het uitputten van de actieve stamcelpool. Inderdaad, patiënten ervaren vaak gastro-intestinaal stralingssyndroom tijdens het ondergaan van radiotherapie, deels als gevolg van actieve stamceluitputting. Het verlies van actieve intestinale stamcellen in darmcrypten activeert een pool van typisch rustige reserve darmstamcellen en induceert dedifferentiatie van secretoire en enterocytvoorlopercellen. Zonder deze cellen zou het darmepitheel niet in staat zijn om te herstellen van radiotherapie en andere dergelijke belangrijke weefselbeledigingen. Nieuwe ontwikkelingen in lineage-tracing-technologieën maken het mogelijk om de activering, differentiatie en migratie van cellen tijdens regeneratie te volgen en zijn met succes gebruikt om dit in de darm te bestuderen. Deze studie heeft tot doel een methode weer te geven voor de analyse van cellen in het darmepitheel van de muis na stralingsletsel.
Het menselijke darmepitheel zou ongeveer het oppervlak van een half badmintonveld bedekken als hetvolledig vlak 1 zou worden geplaatst. In plaats daarvan wordt deze enkele cellaag die mensen scheidt van de inhoud van hun darmen samengeperst in een reeks vingerachtige projecties, villi en inkepingen, crypten die het oppervlak van de darmen maximaliseren. De cellen van het epitheel differentiëren langs een crypte-villusas. De villus bestaat voornamelijk uit voedingsabsorberende enterocyten, slijmafscheidende bokaalcellen en de hormoonproducerende entero-endocriene cellen, terwijl de crypten voornamelijk bestaan uit defensine-producerende Paneth-cellen, actieve en reservestamcellen en voorlopercellen 2,3,4,5. Bovendien genereert de bidirectionele communicatie die deze cellen hebben met de stromale en immuuncellen van het onderliggende mesenchymale compartiment en de microbiota van het lumen een complex netwerk van interacties dat de darmhomeostase handhaaft en van cruciaal belang is voor herstel na letsel 6,7,8.
Het darmepitheel is het snelst zelfvernieuwende weefsel in het menselijk lichaam, met een omloopsnelheid van 2-6 dagen 9,10,11. Tijdens homeostase delen actieve stamcellen aan de basis van darmcrypten (crypt base columnar cells), gekenmerkt door de expressie van leucine-rijke repeat-bevattende G-eiwit gekoppelde receptor 5 (LGR5), zich snel en leveren voorlopercellen die differentiëren in alle andere intestinale epitheellijnen. Vanwege hun hoge mitotische snelheid zijn actieve stamcellen en hun directe voorlopers echter bijzonder gevoelig voor gammastralingsschade en ondergaan ze apoptose na bestraling 5,12,13,14. Na hun verlies ondergaan reservestamcellen en niet-stamcellen (subpopulatie van voorlopers en sommige terminaal gedifferentieerde cellen) in darmcrypten activering en vullen ze het basale cryptecompartiment aan, dat vervolgens celpopulaties van de villi kan reconstitueren en zo het darmepitheel kan regenereren15. Met behulp van afstammingstechnieken hebben meerdere onderzoeksgroepen aangetoond dat reserve (rustige) stamcellen in staat zijn om regeneratie te ondersteunen bij het verlies van actieve stamcellen 13,16,17,18,19,20,21,22. Deze cellen worden gekenmerkt door de aanwezigheid van polycomb complex protein 1 oncogen (Bmi1), muis telomerase reverse transcriptase gen (mTert), Hop homeobox (Hopx), en leucine-rijke repeat protein 1 gen (Lrig1). Bovendien is aangetoond dat niet-stamcellen in staat zijn om darmcrypten aan te vullen bij letsel 23,24,25,26,27,28,29,30,31. In het bijzonder is aangetoond dat voorlopers van secretoire cellen en enterocyten dedifferentiatie ondergaan bij letsel, terugkeren naar stamachtige cellen en de regeneratie van het darmepitheel ondersteunen. Recente studies hebben cellen geïdentificeerd die meerdere markers tot expressie brengen die de capaciteit bezitten om stamachtige kenmerken te verwerven bij letsel (zoals DLL +, ATOH1 +, PROX1 +, MIST1 +, DCLK1 +) 32,33,34,35,36. Verrassend genoeg toonden Yu et al. aan dat zelfs volwassen Paneth-cellen (LYZ +) kunnen bijdragen aan darmregeneratie37. Bovendien, naast het veroorzaken van apoptose van intestinale epitheelcellen en het verstoren van de epitheelbarrièrefunctie, resulteert bestraling in dysbiose van de darmflora, immuuncelactivering en het initiëren van een pro-inflammatoire reactie, en de activering van mesenchymale en stromale cellen38,39.
Gammastraling is een waardevol therapeutisch hulpmiddel bij de behandeling van kanker, vooral voor colorectale tumoren40. Bestraling beïnvloedt echter de intestinale homeostase aanzienlijk door schade aan de cellen te veroorzaken, wat leidt tot apoptose. Blootstelling aan straling veroorzaakt meerdere verstoringen die het herstel van een patiënt vertragen en wordt gekenmerkt door slijmvliesletsel en ontsteking in de acute fase en diarree, incontinentie, bloedingen en buikpijn op lange termijn. Dit arsenaal aan manifestaties wordt gastro-intestinale stralingstoxiciteit genoemd. Bovendien kan door straling geïnduceerde progressie van transmurale fibrose en/of vasculaire sclerose zich pas jaren na de behandeling manifesteren38,41. Gelijktijdig met het letsel zelf induceert straling een herstelreactie in darmcellen die signaalroutes activeert die verantwoordelijk zijn voor het initiëren en orkestreren van regeneratie42. Door straling geïnduceerde dunne darmziekte kan afkomstig zijn van bekken- of abdominale radiotherapie die wordt toegediend aan andere organen (zoals baarmoederhals, prostaat, pancreas, rectum)41,43,44,45,46. Intestinale bestralingsschade is dus een belangrijk klinisch probleem en een beter begrip van de resulterende pathofysiologie zal waarschijnlijk de ontwikkeling van interventies bevorderen om de gastro-intestinale complicaties geassocieerd met radiotherapie te verlichten. Er zijn andere technieken die het mogelijk maken om het regeneratieve doel van het darmepitheel te onderzoeken, afgezien van straling. Transgene en chemische muizenmodellen om ontstekingen en de regeneratie daarna te bestuderen zijnontwikkeld 47. Dextran natriumsulfaat (DSS) induceert ontsteking in de darm en leidt tot de ontwikkeling van kenmerken die vergelijkbaar zijn met die van inflammatoire darmziekte48. Een combinatie van DSS-behandeling met de pro-carcinogene verbinding azoxymethaan (AOM) kan leiden tot de ontwikkeling van colitis-geassocieerde kanker48,49. Ischemie reperfusie-geïnduceerde schade is een andere methode die wordt gebruikt om het regeneratieve potentieel van het darmepitheel te bestuderen. Deze techniek vereist ervaring en chirurgische kennis50. Bovendien veroorzaken de bovengenoemde technieken andere soorten letsel dan straling en kunnen ze leiden tot de betrokkenheid van verschillende regeneratiemechanismen. Bovendien zijn deze modellen tijdrovend, terwijl de stralingstechniek vrij kort is. Onlangs zijn in vitro methoden met behulp van enteroïden en colonoïden gegenereerd uit de darm en dikke darm gebruikt in combinatie met stralingsletsel om de mechanismen van darmregeneratie te bestuderen51,52. Deze technieken geven echter niet volledig de balans weer van het orgel dat ze modelleren53,54.
Het gepresenteerde protocol omvat de beschrijving van een muizenmodel van gammastralingsschade in combinatie met een genetisch model dat, na behandeling met tamoxifen, het traceren van afstammingslijnen afkomstig van de reservestamcelpopulatie (Bmi1-CreER; Rosa26eYFP). Dit model maakt gebruik van een 12 Gy totale lichaamsbestraling, die voldoende darmletsel veroorzaakt om reservestamcellen te activeren, terwijl het daaropvolgende onderzoek naar het regeneratieve vermogen van de darm binnen 7 dagen na letsel mogelijk is55.
Dit protocol beschrijft een robuust en reproduceerbaar stralingsschademodel. Het maakt de nauwkeurige analyse mogelijk van de veranderingen in het darmepitheel in de loop van 7 dagen na het letsel. Belangrijk is dat de geselecteerde tijdstippen cruciale stadia van letsel weerspiegelen en worden gekenmerkt door duidelijke veranderingen in de darm (letsel, apoptose, regeneratie en normalisatiefasen)60.
