Modelos de xenoenxerto derivados da paciente (PDX) e modelos transplantáveis de camundongos geneticamente modificados recapitulam fielmente a doença humana e são modelos preferidos para pesquisas básicas e translacionais sobre câncer de mama. Aqui, um método é descrito para transplantar ortotopicamente fragmentos de tumor de mama na almofada de gordura mamária para estudar biologia tumoral e avaliar a resposta medicamentosa.
Modelos pré-clínicos que recapitulam fielmente a heterogeneidade tumoral e a resposta terapêutica são fundamentais para a pesquisa translacional do câncer de mama. Linhas celulares imortalizadas são fáceis de cultivar e geneticamente modificadas para estudar mecanismos moleculares, mas a pressão seletiva da cultura celular muitas vezes leva a alterações genéticas e epigenéticas ao longo do tempo. Modelos de xenoenxerto derivados da paciente (PDX) recapitulam fielmente a heterogeneidade e a resposta medicamentosa de tumores mamários humanos. Os modelos PDX exibem uma latência relativamente curta após o transplante ortotópico que facilita a investigação da biologia do tumor de mama e da resposta a medicamentos. Os modelos transplantáveis de camundongos geneticamente modificados permitem o estudo da imunidade do tumor de mama. O protocolo atual descreve o método para transplantar ortotopicamente fragmentos de tumor de mama na almofada de gordura mamária seguido de tratamentos medicamentosos. Esses modelos pré-clínicos fornecem abordagens valiosas para investigar a biologia do tumor de mama, a resposta a medicamentos, a descoberta de biomarcadores e os mecanismos de resistência a medicamentos.
A maioria das mortes por câncer de mama pode ser atribuída a doenças recorrentes resistentes às terapias convencionais1,2. A heterogeneidade inter e intra-tumora dos cânceres de mama contribuem para a resistência terapêutica. Além disso, a heterogeneidade tumoral pode afetar o prognóstico preciso e desafiar o manejo da doença3,4. A identificação de biomarcadores preditivos de resposta melhorará significativamente os resultados clínicos de pacientes com câncer de mama. Embora a maioria dos tipos de câncer de mama sejam tumores imunologicamente ‘frios’ que provavelmente não são responder à imunoterapia, os inibidores de checkpoint imunológicos têm mostrado promessa nos ensaios clínicos2,5. Por exemplo, um estudo de fase III mostrou melhor sobrevida livre de doenças (DFS) e evidências preliminares de que o atezolizumabe (anticorpo monoclonal contra PD-L1) combinado com nab-paclitaxel pode fornecer um benefício global de sobrevivência em comparação com nab-paclitaxel sozinho em tumores com ≥1% de mancha PD-L16. O desenvolvimento de terapias que sensibilizem tumores mamários para a imunoterapia revolucionará os regimes de tratamento.
Modelos pré-clínicos que recapitulam fielmente a heterogeneidade do câncer de mama humano e a resposta a medicamentos são fundamentais para estudar a biologia tumoral e identificar potenciais biomarcadores para terapia direcionada. As linhas celulares imortalizadas são amplamente utilizadas para pesquisas sobre câncer de mama, uma vez que essas linhas celulares são fáceis de crescer e geneticamente modificadas para estudar mecanismos moleculares. No entanto, devido à pressão seletiva da cultura celular de longo prazo in vitro, a deriva genética pode ocorrer ao longo do tempo e as linhas de células cancerígenas de mama podem levar a alterações genômicas específicas da linha celular que são distintas das aberrações em tumores primários de mama7,8,9.
Os pedaços tumorais de xenoenxerto derivados do paciente (PDX) são capazes de recapitular a heterogeneidade da doença humana, e são histologicamente e imunohistoquesticamente semelhantes ao tumor de origem10,11,12,13,14,15,16,17,18,19,20,21,22 ,23,24,25,26,27,28,29. É importante ressaltar que os modelos PDX são fenotipicamente estáveis em múltiplos transplantes, evidenciados pela histologia, transcriptome, proteome e análise genômica10,11,12,13,14,15,16,17,18,19,20,21,22 ,23,24,25,26,27,28,29. Os modelos PDX mostram respostas de tratamento comparáveis às observadas clinicamente10,11,12,13,14,15,16,17,18,19,20,21,22,23,24, 25,26,27,28,29. Foram estabelecidos modelos PDX para os modelos PDX positivos para receptor de estrogênio (ER+),receptor de progesterona positivo (PR+),fator de crescimento epidérmico 2 positivo (ERBB2+, HER2+) e câncer de mama triplo negativo (TNBC) modelos PDX e fornecem uma excelente plataforma para testar terapias endócrinas, quimio e direcionadas. No entanto, uma das principais ressalvas dos modelos PDX no momento é a falta de um sistema imunológico funcional no mouse.
Os modelos de rato geneticamente modificados (GEMM), como trp53 homozigos nulos, cMyc, Wnt1, PyMT, ou modelos de superexpressão Her2, permitem o estudo da iniciação espontânea do tumor, progressão e metástase no contexto de um sistema imunológico intacto. No entanto, a latência tumoral é longa, o que dificulta a realização de ensaios pré-clínicos com múltiplos braços30,31. No entanto, o GEMM pode ser transplantado para hospedeiros síngênicos para gerar número suficiente de tumores que recapitulem de perto tumores humanos32,33,34,35,36,37,38,39,40,41,42,43,44,45, 46,47,48,49,50,51,52,53,54,55. Por exemplo, o epitélio mamário de um rato BALB/c p53-nulo foi transplantado nas almofadas de gordura limpas de camundongos receptores do tipo selvagem síndico para formar tumores primários, que podem ser transplantados ainda mais em hospedeiros síngênicos56,57. Os tumores p53-nulos recapitularam diferentes subtipos de tumores humanos.
A combinação de modelos PDX e GEMM transplantável fornece ferramentas pré-clínicos valiosas para investigar a biologia do tumor de mama, a resposta a medicamentos e a imunidade anti-tumor. No protocolo atual, é descrito um método de transplante ortotópico de fragmentos de tumor PDX e GEMM na almofada de gordura mamária do rato. Estes modelos são passíveis de passagens seriais e geralmente mantêm um fenótipo estável. Para mitigar o risco de deriva genética ou perda de heterogeneidade através de passagens ao longo do tempo, múltiplos fragmentos de tecido são criopreservados a cada passagem para transplante subsequente no caso de alterações biológicas ou morfológicas serem observadas ao longo do tempo29,58.
Para reduzir as variações no crescimento do tumor entre os animais, é fundamental cortar o tecido tumoral em fragmentos de 1 mm3 para transplante. Modelos que crescem tecido mole são mais difíceis de trabalhar e os fragmentos tumorais precisam ser cortados ligeiramente maiores (1−2 mm3). Ao colocar o tecido no bolso da almofada de gordura mamária tome cuidado para não dividir o tecido em vários pedaços, pois isso resultará em múltiplos tumores pequenos ou tumores de forma estranha.
