A reference measurement procedure for the absolute quantification of Aβ1-42 in human CSF based on solid-phase extraction and liquid chromatography tandem mass spectrometry is described.
Alzheimer’s disease (AD) is the most common neurodegenerative disease among the elderly and accounts for 60-80% of all cases of dementia. Currently, the diagnosis of AD is based on cognitive tests and mental state exams, but the peptide amyloid-beta (Aβ) in cerebrospinal fluid (CSF) is increasingly used in clinical trials and settings. As for most protein and peptide biomarkers, quantification is performed using antibody-based techniques, such as enzyme-linked immunosorbent assay (ELISA). However, intra- and inter-laboratory variability in these assays hamper its use as a diagnostic marker in clinical routine.
An antibody-independent Reference Measurement Procedure (RMP) was developed based on solid-phase extraction (SPE) and liquid chromatography (LC)-tandem mass spectrometry (MS/MS), where stable, isotope-labeled Aβ peptides were used as internal standards, enabling absolute quantification. A high-resolution quadrupole-orbitrap hybrid instrument was used for the measurements. The method allows for the quantification of CSF Aβ1-42 between 150-4,000 pg/mL.
Ziekte van Alzheimer (AD) is de meest voorkomende vorm van dementie en treft ongeveer 35 miljoen mensen wereldwijd 1. De neuropathologische kenmerken van de ziekte algemeen aangenomen in de kern van de pathogenese van AD te liggen zijn intracellulaire tangles van hypergefosforyleerd tau-eiwit 2 en extracellulaire plaques bestaande uit geaggregeerde amyloid-beta (Aß) peptiden 3. In lijn hiermee heeft de beoordeling van plaque pathologie in vivo door biomarkers onlangs opgenomen in het onderzoek diagnostische criteria voor AD 4. Voor CSF metingen van AB 1-42, verschillende immunoassays zijn beschikbaar en worden gebruikt in vele klinische laboratoria 5. De concentratie van Ap 1-42 in CSF is in AD patiënten ongeveer 50% lager dan in het cognitief normale ouderen, als gevolg van de afzetting van het peptide in plaques in de brain 6, 7.
Deze biomarkers zijn voornamelijk geanalyseerd met immunoassays (bijvoorbeeld op antilichaam gebaseerde technieken), maar deze assays worden beïnvloed door matrixeffecten 8. Het gebruik van immunoassays op verschillende technologische platforms en het gebrek aan standaardisatie assay 9, 10 maakt de invoering van globale cut-off concentraties moeilijk 11, 12. Een analytisch gevalideerd RMP zou de uniforme kalibratie van verschillende assay platforms toe te staan, idealiter resulteert in een betere vergelijkbaarheid tussen de analytische platforms en in een betere controle van de factoren die bijdragen aan de totale meting variabiliteit.
De absolute kwantificering van Aß 1-42 met behulp van de ontwikkelde LC-MS / MS methode overwint veel van de kwesties in verband met antilichamen gebaseerde techniques. De methode, vermeld als een RMP door het Gemengd Comité voor Traceerbaarheid in Laboratory Medicine (JCTLM databank identificatienummer C11RMP9), zullen worden gebruikt om de absolute concentratie van Aß 1-42 te bepalen in een Certified Reference Material (CRM) te harmoniseren CSF AB 1 -42 metingen over technieken en analyse-platforms. De beschreven workflow moet relevant zijn voor de ontwikkeling van kandidaat referentiemethoden voor peptiden en eiwitten in andere gebieden van de geneeskunde.
