La estimulación magnética transcraneal (TMS) es una herramienta no invasiva para obtener conocimientos sobre la fisiología y la función del sistema nervioso humano. A continuación, presentamos nuestras técnicas de TMS para estudiar la excitabilidad cortical de la extremidad superior y de la musculatura lumbar.
La estimulación magnética transcraneal (TMS) ha estado en uso durante más de 20 años 1, y ha crecido exponencialmente en popularidad durante la última década. Mientras que el uso de la EMT se ha ampliado para el estudio de muchos sistemas y procesos durante este tiempo, la solicitud original y tal vez uno de los usos más comunes de la EMT consiste en el estudio de la fisiología, la plasticidad y la función del sistema neuromuscular humanos. TMS solo pulso aplicado a la corteza motora excita las neuronas piramidales transsynaptically 2 (Figura 1) y los resultados en una respuesta medible electromiográfica que se pueden utilizar para estudiar y evaluar la integridad y la excitabilidad de las vías corticoespinal en los seres humanos 3. Además, los recientes avances en la estimulación magnética permite ahora la partición de la excitabilidad cortical en comparación con 4,5 espinal. Por ejemplo, el pulso pares-TMS se puede utilizar para evaluar intracortical propiedades de facilitación e inhibición mediante la combinación de una condiciónción de estímulo y un estímulo de prueba a intervalos interestímulo diferentes 3,4,6-8. En este artículo vamos a demostrar de vídeo de los aspectos metodológicos y técnicos de estas técnicas. En concreto, vamos a demostrar un solo pulso y pulso de dos a dos técnicas TMS aplicada a la flexor radial del carpo (FCR) del músculo, así como la musculatura erector de la columna (ES). Nuestro laboratorio estudia los músculos FCR, ya que es de interés para nuestra investigación sobre los efectos de la inmovilización con yeso de la muñeca en el rendimiento muscular reducido 6,9, y se estudian los músculos ES debido a estos músculos relevancia clínica, ya que se relaciona con el dolor de espalda baja 8. Con esto dicho, hay que señalar que la EMT se ha utilizado para estudiar muchos músculos de las manos, brazos y piernas, y debe reiterar que nuestras manifestaciones en el FCR y grupos ES musculares son sólo algunos ejemplos de la EMT se utilizan para estudiar el ser humano neuromuscular del sistema.
El objetivo general de este artículo es proporcionar a los científicos ya los médicos una cuenta visual de nuestros laboratorios de uso de la estimulación magnética transcraneal. Sin embargo, además de proporcionar una visualización de estos experimentos, a continuación se discuten los problemas básicos a considerar cuando se realiza TMS de esta manera, ofrecer un breve resumen de la fisiología de las respuestas de TMS, y también discutir el uso de la EMT en lo que respecta a la utilización de otros.
