Laseropname van microdissectie van orale submuceuze fibroseweefsels maakt nauwkeurige extractie van cellen mogelijk uit histologische regio’s die van belang zijn voor de analyse van multi-omics-gegevens met morfologische en ruimtelijke informatie.
Orale submuceuze fibrose (OSF) is een veel voorkomende vorm van potentieel kwaadaardige aandoening in de mondholte. De atrofie van epitheel en fibrose van de lamina propria en de submucosa worden vaak aangetroffen op histopathologische objectglaasjes. Er werd voorgesteld dat epitheliale dysplasie, epitheliale atrofie en senescente fibroblasten geassocieerd zijn met de kwaadaardige transformatie van OSF. Vanwege de heterogeniteit van potentieel kwaadaardige orale aandoeningen en oraal plaveiselcelcarcinoom, is het echter moeilijk om de specifieke moleculaire mechanismen van kwaadaardige transformatie in OSF te identificeren. Hier presenteren we een methode om een klein aantal epitheel- of mesenchymale cellen te verkrijgen die morfologische gegevens en ruimtelijke informatie dragen door middel van microdissectie met laseropname op in formaline gefixeerde paraffine-ingebedde weefselglaasjes. Met behulp van een microscoop kunnen we op microschaal (~500 cellen) dysplastisch of atrofisch epitheelweefsel en fibrotisch subepitheliaal weefsel nauwkeurig vastleggen. De geëxtraheerde cellen kunnen worden geëvalueerd door genoom- of transcriptoomsequencing om genomische en transcriptomische gegevens met morfologische en ruimtelijke informatie te verkrijgen. Deze aanpak elimineert de heterogeniteit van de sequentiebepaling van OSF-weefsel in bulk en de interferentie veroorzaakt door cellen in niet-laesionele gebieden, waardoor nauwkeurige spatial-omics-analyse van OSF-weefsel mogelijk wordt.
Orale submuceuze fibrose (OSF) is een chronische, verraderlijke ziekte die zich voornamelijk in het mondslijmvlies ontwikkelt en resulteert in een beperkte mondopening. Hoewel OSF een multifactoriële ziekte is, is het kauwen van arecanoten of betelnoten de belangrijkste oorzaak van OSF 2,3. Vanwege deze geografisch specifieke gewoonte is OSF voornamelijk geconcentreerd in populaties in Zuidoost- en Zuid-Azië3. De gemeenschappelijke histologische kenmerken van OSF zijn onder meer abnormale collageenafzetting in het bindweefsel onder het mondslijmvliesepitheel, vasculaire stenose en occlusie1. OSF-epitheelweefsel kan manifestaties van atrofie of hyperplasie en zelfs dysplasie vertonen wanneer het gelijktijdig wordt behandeld met orale leukoplakie 4,5.
OSF wordt door de Wereldgezondheidsorganisatie gedefinieerd als een veel voorkomende orale potentieel kwaadaardige aandoening (OPMD) die het potentieel vertoont om zich te ontwikkelen tot oraal plaveiselcelcarcinoom met een kwaadaardige transformatiesnelheid van 4%-6%6,7,8,9. Het mechanisme dat ten grondslag ligt aan de kwaadaardige transformatie van OSF is complex10. Abnormale groei van het epitheel, waaronder zowel dysplasie als atrofie, verhoogt de kans op carcinogenese, en senescente fibroblasten in het stroma kunnen betrokken zijn bij de kwaadaardige progressie van OSF door epitheliale-mesenchymale overgang (EMT) te induceren via reactieve zuurstofsoorten (ROS) en andere moleculen10.
Technologieën voor ruimtelijk-omische analyses genereerden multi-omische gegevens met morfologische en ruimtelijke informatie die inzicht hebben verschaft in kankermechanismen11,12,13. Hier presenteren we een protocol om morfologie-gerelateerde celpopulaties vast te leggen uit formaline-gefixeerd paraffine-ingebed OSF-weefsel door middel van lasermicrodissectie. Multi-omische analyses van deze monsters kunnen uitdagingen met heterogeniteit binnen het weefsel overwinnen en het begrip van de moleculaire pathologie en mechanismen van kwaadaardige transformatie in OSF14 vergroten.
Dit protocol rapporteerde een pijplijn om OSF-weefselmonsters vast te leggen met morfologische en ruimtelijke informatie voor verdere ruimtelijk-omische analyses door middel van lasermicrodissectie. Op basis van de representatieve resultaten identificeerden we verschillende CNA-patronen tussen verschillende morfologiegerelateerde monsters.
