Este protocolo apresenta um procedimento curativo de moxabustão no tratamento de pacientes com asma brônquica.
A asma brônquica é uma doença inflamatória crônica das vias aéreas que pode levar a sintomas como sibilância recorrente, falta de ar, aperto no peito ou tosse, como resultado do aumento da reatividade das vias aéreas. Com alta variação diurna, esses sintomas geralmente ocorrem ou pioram à noite ou pela manhã. Ao queimar e torrar a medicina chinesa acima dos acupontos humanos, a moxabustão é um tipo de tratamento que estimula a atividade dos meridianos humanos por meio de medicamentos e estímulos térmicos para prevenir e tratar doenças. De acordo com o princípio da diferenciação da síndrome e tratamento da medicina tradicional chinesa, os acupontos são selecionados nas partes correspondentes, o que tem um efeito definido. É considerada uma terapia característica da medicina tradicional chinesa para asma brônquica. Este protocolo elabora os métodos e etapas do manejo do paciente, preparo do material, seleção dos acupontos, operação e enfermagem pós-operatória para garantir o tratamento seguro e eficaz da moxabustão para melhorar significativamente os sintomas clínicos e a qualidade de vida dos pacientes com asma brônquica.
O protocolo aqui apresentado demonstra o funcionamento da moxabustão no tratamento da asma brônquica em termos de manejo do paciente, preparo do material, seleção do acuponto, operação e enfermagem pós-operatória. O objetivo deste protocolo é controlar os sintomas de pacientes com asma brônquica e melhorar a função pulmonar por meio da moxabustão.
A acupuntura e a moxabustão são importantes componentes da medicina tradicional chinesa (MTC). Em relação à asma brônquica, a acupuntura e a moxabustão têm acumulado vasta experiência clínica em tratamentos, e a moxabustão, em particular, tem mostrado vantagens únicas 1,2,3,4. Estudos modernos mostram que os efeitos do mecanismo biológico de promoção do aquecimento e suplementação da moxabustão são os seguintes: a moxabustão ativa os acupontos (início local), promove o movimento do qi e do sangue, regula a rede neuroendócrino-imune (via regulatória) e regula as funções das vísceras (resposta do órgão efetor)5,6,7.

Esses sintomas frequentemente ocorrem ou pioram à noite ou pela manhã8. A asma brônquica é uma das doenças pulmonares crônicas mais comuns no mundo, acometendo pessoas de todas as idades9. Nos últimos anos, a prevalência de asma brônquica na China vem aumentando anualmente. O último levantamento epidemiológico mostrou que a prevalência de asma em adultos acima de 20 anos é de 4,2%10. A doença levará ao remodelamento das vias aéreas e hiper-reatividade, o que afetará seriamente a função pulmonar dos pacientes em estágio tardio. Atualmente, os principais tratamentos clínicos da asma brônquica são os tratamentos anti-inflamatórios, antiespasmódicos e sintomáticos. No entanto, a alta taxa de recorrência e a intratabilidade delineiam as principais desvantagens, sem falar na baixa eficácia da droga em longo prazo e na falta de um esquema terapêutico ideal. Comparada com drogas inalatórias isoladamente, a moxabustão poderia aumentar a imunidade humoral e celular, que têm um efeito imunomodulador significativo durante o tratamento11,12,13. Ao queimar e torrar a medicina chinesa acima dos acupontos humanos, a moxabustão é um tipo de tratamento que funciona através de drogas e estimulação térmica. Tem efeitos como dissipar as sensações de frio aquecendo os meridianos, promovendo a circulação do qi para remover a obstrução meridiana e eliminando a estagnação para ativar os meridianos. É considerada uma terapia característica da MTC para asma brônquica14.
A inflamação das vias aéreas causada pela asma brônquica é uma resposta imune em cascata envolvendo várias células imunes, citocinas e mediadores inflamatórios19. Biópsias brônquicas de pacientes asmáticos mostram a presença de eosinófilos, mastócitos ativados e infiltração de células T Th2020.
