Het huidige protocol beschrijft een geoptimaliseerde hematopoietische stam- en voorlopercel (HSPC) kweekprocedure voor de robuuste engraftment van gen-bewerkte cellen in vivo.
CRISPR/Cas9 is een zeer veelzijdige en efficiënte genbewerkingstool die op grote schaal wordt gebruikt om verschillende genetische mutaties te corrigeren. De haalbaarheid van genmanipulatie van hematopoëtische stam- en voorlopercellen (HSPC’s) in vitro maakt HSCC’s een ideale doelcel voor gentherapie. HSPC’s verliezen echter matig hun engraftment- en multilineage herbevolkingspotentieel in ex vivo cultuur. In de huidige studie worden ideale kweekomstandigheden beschreven die de HSPC-engraftment verbeteren en in vivo een verhoogd aantal gen-gemodificeerde cellen genereren. Het huidige rapport toont geoptimaliseerde in vitro kweekomstandigheden, waaronder het type kweekmedia, unieke cocktailsuppletie met kleine moleculen, cytokineconcentratie, celkweekplaten en kweekdichtheid. Daarnaast wordt een geoptimaliseerde HSPC-genbewerkingsprocedure, samen met de validatie van de genbewerkingsgebeurtenissen, verstrekt. Voor in vivo validatie worden de gen-bewerkte HSPC-infusie en post-engraftmentanalyse bij muisontvangers weergegeven. De resultaten toonden aan dat het kweeksysteem de frequentie van functionele HSC’s in vitro verhoogde, wat resulteerde in robuuste engraftment van gen-bewerkte cellen in vivo.
De ontoegankelijkheid van menselijke leukocytenantigeen (HLA)-gematchte donoren in allogene transplantatie-instellingen en de snelle ontwikkeling van zeer veelzijdige en veilige genetische manipulatietools maken autologe hematopoëtische stamceltransplantatie (HSCT) een curatieve behandelingsstrategie voor de erfelijke bloedaandoeningen 1,2. Autologe hematopoietische stam- en voorlopercel (HSPC) gentherapie omvat het verzamelen van HSPC’s van patiënten, genetische manipulatie, correctie van ziekteveroorzakende mutaties en transplantatie van gengecorrigeerde HSPC’s in de patiënt 3,4. Het succesvolle resultaat van de gentherapie is echter afhankelijk van de kwaliteit van het transplanteerbare gen-gemodificeerde transplantaat. De genmanipulatiestappen en ex vivo kweek van HSPC’s beïnvloeden de kwaliteit van het transplantaat door de frequentie van langdurige hematopoietische stamcellen (LT-HSC’s) te verlagen, waardoor de infusie van grote doses gengemanipuleerde HSPC’s 2,5,6 noodzakelijk wordt.
Verschillende kleine moleculen, waaronder SR1 en UM171, worden momenteel gebruikt om navelstrengbloed HSPC’s robuust uit te breiden 7,8. Voor volwassen HSPC’s is, vanwege de hogere celopbrengst verkregen bij mobilisatie, robuuste expansie niet vereist. Het behoud van de stamheid van geïsoleerde HSPC’s in ex vivo cultuur is echter cruciaal voor de gentherapietoepassingen. Daarom wordt een aanpak ontwikkeld die zich richt op de kweekverrijking van hematopoëtische stamcellen (HSC’s) met behulp van een combinatie van kleine moleculen: Resveratrol, UM729 en SR1 (RUS)7. De geoptimaliseerde HSPC-kweekomstandigheden bevorderen de verrijking van HSC’s, wat resulteert in een verhoogde frequentie van gengemodificeerde HSC’s in vivo, en verminderen de behoefte aan genmanipulatie van grote doses HSCS, waardoor kosteneffectieve gentherapiebenaderingenworden vergemakkelijkt 8.
Hier wordt een uitgebreid protocol voor HSPC-cultuur beschreven, samen met de infusie en analyse van gen-bewerkte cellen in vivo.
Het succesvolle resultaat van HSPC-gentherapie is voornamelijk afhankelijk van de kwaliteit en kwantiteit van enttable HSC’s in het transplantaat. De functionele eigenschappen van HSC’s worden echter sterk beïnvloed tijdens de voorbereidende fase van gentherapieproducten, onder meer door in vitro kweek en toxiciteit geassocieerd met de genmanipulatieprocedure.
