Het protocol combineert een longkoeling snelle hersteltechniek met abdominale normotherme regionale perfusie voor abdominale transplantaatverkrijging bij gecontroleerde asystoliedonoren, wat een veilige en nuttige methode is om de donorpool uit te breiden.
Gecontroleerde donatie na circulatoire sterfte (cDCD) heeft bijgedragen aan het verhogen van het aantal donoren over de hele wereld. Ervaringen die in de afgelopen jaren zijn gepubliceerd, bevestigen dat de resultaten na longtransplantatie van cDCD vergelijkbaar zijn met die van donoren met hersendood; Het gebruik van longen van asystoliedonoren blijft echter laag. Verschillende redenen kunnen hierbij betrokken zijn: verschillende wettelijke kaders tussen landen en centra met verschillende premortale interventies, ontoereikende longdonorzorg vóór de verkrijging, of zelfs slechte ervaring met cDCD-procedures en -protocollen.
Aanvankelijk werd de snelle hersteltechniek vaak gebruikt voor de verwerving van thoracale en abdominale organen in cDCD, maar in het laatste decennium is abdominale normotherme regionale perfusie (ANRP) met extracorporale membraanoxygenatie-apparaten een nuttige methode geworden om de bloedtoevoer naar buikorganen te herstellen, waardoor hun kwaliteitsverbetering en hun functionele beoordeling voorafgaand aan transplantatie mogelijk zijn. Dit maakt de donatieprocedure complexer en roept twijfels op over letsel aan de grafts als gevolg van dubbele temperatuur.
Het doel van dit artikel is om een protocol te beschrijven op basis van een enkele centrumervaring met Maastricht III-donoren die longkoeling, snel herstel in de thorax en abdominale normotherme regionale perfusie combineren. Tips en tricks gericht op premorteminterventies en longverwervingsproceduretechnieken worden uitgelegd. Dit kan helpen om de terughoudendheid van professionals om deze gecombineerde techniek te gebruiken te minimaliseren en andere donorcentra aan te moedigen om het te gebruiken, ondanks de toegenomen complexiteit van de procedure.
Donatie na circulatoire sterfte (DCD) begon in Spanje met ongecontroleerde donoren. In 1996 werd het eerste nationale consensusdocument over DCD gepubliceerd als leidraad voor de praktijk van ongecontroleerde donatie na circulatoire sterfte1 (uDCD), waarbij ook een moratorium op gecontroleerde donatie na circulatoire sterfte (cDCD) werd ingesteld. In 2012 ontstond een nieuwe consensus waarin de basis en het wetgevingskader voor de praktijk van zowel uDCD als cDCD2 werden vastgelegd. Momenteel is Spanje een van de meest actieve landen in DCD en bereikt het hoogste percentage donoren na circulatoire sterfte ter wereld3. Dit type donor vertegenwoordigde bijna 35% van de totale donoren in 2021 in het land, met een duidelijke daling van uDCD en donoren die uitsluitend cDCD4 zijn.
Orgaanverwerving in cDCD wordt gewoonlijk uitgevoerd met behulp van de supersnelle hersteltechniek5. Na de verklaring van overlijden en wanneer de niet-aanraakperiode is verstreken, wordt een snelle sternotomie en laparotomie uitgevoerd. De abdominale aorta en longslagader worden gecannuleerd en gespoeld met koude perfusieoplossingen om de buik- en thoracale organen te behouden, plus topische koeling wordt uitgevoerd voordat ze worden opgehaald6. In deze situatie wordt cDCD gekenmerkt door de onvoorspelbare gevolgen van warme ischemie, na het staken van levensondersteunende therapie. De ischemische schade tijdens deze periode van agonische hypotensie en progressieve hypoxie, gevolgd door de non-touch periode na hartstilstand, wordt verder verergerd door de latere periode van koude ischemie7. Deze combinatie van warme en koude ischemie lijkt schadelijk te zijn, vooral voor buiktransplantaten 8,9,10, waardoor meer terughoudendheid bij professionals ontstaat bij het gebruik van deze organen van cDCD-donoren.
Om deze risico’s te minimaliseren, is met groeiende interesse een in situ conserveringsmodel ontwikkeld, gebaseerd op eerdere ervaringen van Spaanse teams die in uCDC11 werken. Het gebruik van extracorporale membraanoxygenatie (ECMO) -systemen om de bloedstroom na de dood en vóór het herstel van het transplantaat te herstellen, kan de metabole afwijkingen als gevolg van ischemie omkeren en de cellulaire fysiologie herstellen12. Abdominale normotherme regionale perfusie (ANRP) kan de kwaliteit van ischemisch beschadigde organen in cDCD13 verbeteren. De orgaanfunctie kan worden beoordeeld en verbeterd, waardoor een betere selectie van abdominale transplantaten voor transplantatie mogelijk is.
Recente internationale multicenter-ervaringen leveren bewijs dat ANRP versus de rapid recovery (RR) -techniek helpt bij het overwinnen van traditionele beperkingen in cDCD, het verminderen van het aantal galcomplicaties na transplantatie, het vergemakkelijken van succesvolle transplantatie van oudere levers en het verbeteren van de overleving van levertransplantaten14,15. In nieren lijkt het de kortetermijnresultaten te verbeteren met een lagere vertraagde transplantaatfunctie en hogere 1-jaarsoverlevingspercentages voor transplantaten16. Met dit bewijs heeft ANRP in cDCD voordelen gekregen ten opzichte van de snelle hersteltechniek voor abdominale transplantaatverkrijging en wordt nu toegepast in verschillende Europese landen en andere delen van de wereld17,18.
Het gebruik van longen van cDCD-donoren werd echter wereldwijd snel overgenomen. Een long functionele warme ischemische tijd van maximaal 60 min lijkt geen invloed te hebben op de overleving19. In het afgelopen decennium hebben verschillende centra en multi-institutionele ervaringen resultaten gemeld na longtransplantatie van cDCD vergelijkbaar met die van DBD20,21. De RR-techniek is de routinemethode voor longverkrijging: longen worden topisch gekoeld en verwijderd na te zijn gespoeld met koude conserveringsoplossing22.
De eerste ervaringen met het combineren van ANRP en RR van longen in cDCD werden gemeld door twee Britse groepen23,24. Jaren later werd een variant van deze techniek die premorteminterventies toevoegdegepubliceerd 25. De resultaten presenteren deze dubbele verkrijgingstechniek als veilig en effectief voor zowel abdominale als thoracale transplantaten26. Uiteraard wordt de donatieprocedure complexer. Het vereist technologische en menselijke hulpbronnen, voldoende organisatorische capaciteiten en heeft hogere economische kosten. Dit alles kan professionals ontmoedigen om een programma te starten. Het doel van deze studie is om een protocol te presenteren dat speciaal gericht is op premorteminterventies, cannulatie en plaatsing van aorta-occlusieballonnen, met tips en trucs die uit ervaring zijn geleerd, en commentaar te geven op de verschillende technische details waarmee rekening moet worden gehouden tijdens het ophalen van de longen wanneer ARNP wordt gebruikt. Op dit moment zijn cDCD-donoren in het centrum de belangrijkste bron van grafts voor thoracale en abdominale transplantatie geworden.
Hoewel het gebruik van gelijktijdige longkoude perfusie met ARNP in cDCD voor het eerst werd gepubliceerd in 2014, zijn er zeer weinig ervaringen beschreven voor deze25,26,29. Bovendien blijft het gebruik van cDCD-longen, ongeacht de gebruikte techniek, in de meeste landen laag.
