Метод флуоресцентной идентификации является практичным, быстрым и надежным подходом к дифференциации композитных смоляных реставраций от зубного вещества и облегчает минимально инвазивное и полное удаление композитных смоляных реставраций и композитных связанных травматических шин.
Обнаружение и удаление пломбированных материалов цвета зубов является серьезной проблемой для каждого стоматолога. Метод флуоресцентной идентификации (FIT) является неинвазивным инструментом для облегчения отличия композитного смоляного материала от вещества здорового зуба. По сравнению с обычным освещением, FIT является очень точным, надежным и быстрым методом диагностики. Когда композитная смола освещается с длиной волны приблизительно 398 ± 5 нм, некоторые флуоресцентные компоненты заставляют композитную смолу казаться ярче, чем структура зуба. Для этого метода может быть использован любой флуоресцентно-индуцирующий источник света с соответствующей длиной волны. Оптимально использовать данную технику без дополнительного естественного или искусственного освещения. Применение FIT может быть использовано в диагностических целях, например, стоматологических картах, а также дополнительно для полного и малоинвазивного удаления композитных смоляных реставраций, снятия брекетов и снятия травматологической шины. Оценка объемных изменений после удаления композита может быть обеспечена путем перекрывающегося пред- и послеоперационного сканирования и последующего расчета с использованием подходящего программного обеспечения.
Применение FIT облегчает отделение композитных смоляных материалов от звукового зубчатого вещества по сравнению с обычным освещением, например, лампойстоматологической установки 1,2. Флуоресценция возникает, когда материал излучает свет на более высокой длине волны, чем он был поглощен. В результате такого освещения материал кажется ярче, чемзубец 3. Максимальная флуоресценция композиционных смоляных материалов происходит при освещении длиной волны 398 ± 5 нанометров3. Флуоресценция в композиционных смоляных материалах появляется за счет редкоземельных оксидов, добавляемых в стеклянные наполнители, некоторые из основных компонентов композитных смол 4,5. Добавление этих флуоресцентных веществ направлено на адаптацию оптических свойств композитных смол к структуре зуба для улучшения эстетических свойств композитных смол 4,5. FIT применим ко многим композитным смоляным материалам, поскольку они демонстрируют эти флуоресцентные свойства3. Однако флуоресценция уменьшается с возрастом композиционных смолистых материалов 6,7,8,9.
Отличие композитных смоляных материалов от структуры зуба с обычным освещением является проблемой, поскольку современные композитные смоляные материалы почти идеально соответствуют оптическим свойствам вещества зуба10,11. Ошибочный диагноз композитной смолы приводит к неточным стоматологическим картам, ложной оценке риска кариеса и ненадлежащему планированию лечения11. Более того, эпидемиологические данные фальсифицированы12.
Композитная смола является предпочтительным материалом для прямых реставраций благодаря своей простоте в обращении, эстетическим свойствам и клиническим характеристикам13. Тем не менее, многие композитные реставрации должны быть возобновлены из-за вторичного кариеса, переломов или других причин14,15. Тем не менее, удаление остаточных композитных смоляных материалов может быть требовательным в обычных условиях освещения. Даже при нанесении увеличительного средства и использовании тактильных зондов или обширном высыхании зубов композитные остатки иногда трудно отличить от прочной структуры зуба. Остатки композитных остатков при удалении адгезивной реставрации снижают качество дальнейших реставраций и имеют эстетическое нарушение из-за возможного обесцвечивания краев 1,16,17,18,19,20,21,22 . Напротив, чрезмерная подготовка из-за неправильного диагноза композитной смолы по сравнению со структурой зуба может привести к ненужной потере вещества 1,2.
В стоматологической травматологии фиксация травмированных зубов с помощью травматических шин является частой и обязательной во многих случаях23. Травматические шины обычно фиксируются на зубах с помощью текучего композитного смоляного материала. Неполное удаление композитного смоляного материала в этом сценарии может привести к нарушениям, описанным выше. Поскольку стоматологическая травма происходит в основном в передних зубах, нарушение эстетики и достаточная адгезия дальнейших реконструкций имеют решающее значение. Поэтому целью статьи является демонстрация применения метода FIT как эффективного и простого подхода к обнаружению и удалению композиционных смоляных материалов.
