Dit artikel beschrijft een workflow van technieken die worden gebruikt voor het testen van nieuwe kandidaat-mediatoren van melanoommetastase en hun werkingsmechanisme (s).
Metastase is een complex proces waarbij cellen barrières moeten overwinnen die slechts onvolledig worden gemodelleerd door in vitro assays. Een systematische workflow werd opgezet met behulp van robuuste, reproduceerbare in vivo modellen en gestandaardiseerde methoden om nieuwe spelers in melanoommetastase te identificeren. Deze benadering maakt het mogelijk om gegevens in specifieke experimentele stadia te laten afleiden om de rol van een gen in metastase nauwkeurig te karakteriseren. Modellen worden vastgesteld door genetisch gemodificeerde melanoomcellen via intracardiale, intradermale of subcutane injecties in muizen te introduceren, gevolgd door monitoring met seriële in vivo beeldvorming. Zodra vooraf vastgestelde eindpunten zijn bereikt, worden primaire tumoren en/of metastasen-dragende organen geoogst en verwerkt voor verschillende analyses. Tumorcellen kunnen worden gesorteerd en onderworpen aan een van de verschillende ‘omics’-platforms, waaronder eencellige RNA-sequencing. Organen ondergaan beeldvorming en immunohistopathologische analyses om de totale last van metastasen te kwantificeren en hun specifieke anatomische locatie in kaart te brengen. Deze geoptimaliseerde pijplijn, inclusief gestandaardiseerde protocollen voor engraftment, monitoring, weefseloogst, verwerking en analyse, kan worden gebruikt voor patiënt-afgeleide, kortetermijnculturen en gevestigde menselijke en muizencellijnen van verschillende vaste kankertypen.
De hoge mortaliteit geassocieerd met gemetastaseerd melanoom in combinatie met een toenemende incidentie van melanoom wereldwijd1 (een geschatte toename van 7,86% tegen 2025) vraagt om nieuwe behandelingsbenaderingen. Vooruitgang in doelontdekking hangt af van reproduceerbare modellen van metastase, een zeer complex proces. Gedurende de stappen van de gemetastaseerde cascade moeten melanoomcellen talloze barrières overwinnen om ontwijking van het immuunsysteem en kolonisatie van verre weefsels te bereiken2. De veerkracht en het aanpassingsvermogen van melanoomcellen komen voort uit een veelheid aan factoren, waaronder hun hoge genetische mutatielast3 en hun neurale crest-oorsprong, die cruciale fenotypische plasticiteitverlenen 3,4,5. Bij elke stap laten transcriptionele programma’s metastaserende melanoomcellen van de ene toestand naar de andere overschakelen op basis van signalen van de kruisverwijzing met de micro-omgeving, bestaande uit het immuunsysteem6, het extracellulaire milieu 7,8 en de cellulaire architectuur van fysieke barrières9 waarmee ze in contact komen. Melanoomcellen ontsnappen bijvoorbeeld aan immuunsurveillance door de expressie van belangrijke immuun-priming tumor-uitgescheiden factoren te downreguleren6.
Studies beschrijven een “premetastatische niche”, waarbij melanoomcellen chemokines en cytokines afscheiden om het verre “doel” -orgaan voor te bereiden op metastase10. Deze bevindingen roepen belangrijke vragen op over het orgaantropisme van gemetastaseerde melanoomcellen en de anatomische route die ze nemen om toegang te krijgen tot verre weefsels. Na intravasatie is bekend dat melanoomcellen uitzaaien via lymfevaten (lymfestelsel) en bloedvaten (hematogene verspreiding)2,11. Terwijl de meeste patiënten zich presenteren met gelokaliseerde ziekte, presenteert een kleine subset van gevallen zich met gemetastaseerde ziekte op afstand en geen lymfatische verspreiding (negatieve betrokkenheid van lymfeklieren)11, wat wijst op het bestaan van alternatieve gemetastaseerde routes voor melanoom.
Wanneer ze een gemetastaseerde site koloniseren, ondergaan melanoomcellen epigenetische en metabole aanpassingen12,13. Om toegang te krijgen tot nieuwe compartimenten en deze binnen te dringen, gebruiken melanoomcellen proteasen14 en cytoskeletale modificaties 11,15, waardoor ze naar hun nieuwe locatie kunnen reizen en groeien. De moeilijkheid bij het richten op melanoomcellen ligt in de complexiteit en het aantal van dergelijke aanpassingen; daarom moet het veld zich inspannen om experimenteel zoveel mogelijk stappen en aanpassingen na te bootsen. Ondanks talrijke vooruitgang in in vitro assays zoals organoïden en 3D-culturen16,17, vatten deze modellen de in vivo metastatische cascade slechts onvolledig samen.
Murine-modellen hebben waarde aangetoond door een balans te vinden tussen reproduceerbaarheid, technische haalbaarheid en simulatie van menselijke ziekten. Intravasculair, orthotopisch en heterotopisch geïmplanteerde melanoomcellen uit patiënt-afgeleide xenografts of kortetermijnculturen in immuungecompromitteerde of gehumaniseerde muizen vormen de ruggengraat van doelontdekking bij gemetastaseerd melanoom. Deze systemen missen echter vaak een cruciale biologische beperking op metastase: het immuunsysteem. Syngeneïsche melanoommetastasemodellen die deze beperking bezitten, zijn relatief schaars in het veld. Deze systemen, ontwikkeld in immunocompetente muizen, waaronder B16-F1018, de YUMM-familie van cellijnen19, SM120, D4M321, RIM322 of meer recent, de RMS23 en M1 (Mel114433), M3 (HCmel1274), M4 (B2905) 24 melanoomcellijnen, vergemakkelijken het onderzoek naar de complexe rol van de immuunrespons van de gastheer bij de progressie van melanoom.
Hier wordt een pijplijn voor melanoommetastase doelidentificatie gepresenteerd. Met toenemende en grotere ‘omics’-datasets die worden gegenereerd uit melanoompatiëntencohorten, stellen we dat studies met de meeste klinische beloftes die zijn die voortkomen uit big data-integratie, wat leidt tot nauwgezette functionele en mechanistische ondervraging 25,26,27,28. Door muismodellen te gebruiken om potentiële doelen in het metastatische proces te bestuderen, kan men rekening houden met in vivo-specifieke gebeurtenissen en weefselinteracties, waardoor de kans op klinische vertaling toeneemt. Meerdere methoden om gemetastaseerde belasting te kwantificeren worden geschetst, die aanvullende gegevens over de resultaten van een bepaald experiment opleveren. Een protocol voor eencellige isolatie van tumoren in verschillende organen wordt beschreven om de onbevooroordeelde karakterisering van genexpressie in gemetastaseerde cellen te ondersteunen, die vooraf kunnen gaan aan single-cell of bulk RNA-sequencing.
Het doel van dit technisch rapport is om een gestandaardiseerde, top-to-bottom workflow te bieden voor het onderzoek naar potentiële actoren in melanoommetastase. Omdat in vivo experimenten kostbaar en tijdrovend kunnen zijn, zijn strategieën om de efficiëntie te maximaliseren en de waarde van de verkregen informatie te verhogen van het grootste belang.
