Deze studie beschrijft de directe meting van de leverglucoseproductie in een polycysteus ovariumsyndroom muismodel door gebruik te maken van een stabiele isotopische glucose tracer via staartader in zowel nuchtere als glucoserijke toestanden in tandem.
Polycysteus ovariumsyndroom (PCOS) is een veel voorkomende ziekte die resulteert in aandoeningen van het glucosemetabolisme, zoals insulineresistentie en glucose-intolerantie. Ontregeld glucosemetabolisme is een belangrijke manifestatie van de ziekte en is de sleutel tot de pathogenese. Daarom zijn studies met betrekking tot de evaluatie van het glucosemetabolisme in PCOS van het grootste belang. Zeer weinig studies hebben de productie van leverglucose rechtstreeks gekwantificeerd in PCOS-modellen met behulp van niet-radioactieve glucose tracers. In deze studie bespreken we stapsgewijze instructies voor de kwantificering van de snelheid van leverglucoseproductie in een PCOS-muismodel door M +2-verrijking van [6,6-2H2] glucose, een stabiele isotopische glucose tracer, te meten via gaschromatografie – massaspectrometrie (GCMS). Deze procedure omvat het creëren van stabiele isotopische glucose tracer oplossing, gebruik van staartader katheter plaatsing en infusie van de glucose tracer in zowel nuchtere als glucose-rijke toestanden in dezelfde muis in tandem. De verrijking van [6,6-2H2]glucose wordt gemeten met pentaacetaatderivaat in GCMS. Deze techniek kan worden toegepast op een breed scala aan studies met directe meting van de snelheid van de leverglucoseproductie.
Polycysteus ovariumsyndroom (PCOS) is een veel voorkomende aandoening die voorkomt bij 12%-20% van de vrouwen in de reproductieve leeftijd1,2. Het is een complexe ziekte die resulteert in variabele fenotypen met polycysteuze eierstokken, onregelmatige menstruatie en klinisch of laboratoriumbewijs van hyperandrogeenemie, en wordt meestal gediagnosticeerd wanneer een vrouw aan twee van de drie criteria voldoet3. Een overheersend aspect van PCOS, en een belangrijke factor in de pathogenese, is metabole verstoringen die worden aangetroffen bij vrouwen die de ziekte hebben. Vrouwen met PCOS hebben een hogere incidentie van insulineresistentie, glucose-intolerantie, obesitas en metabool syndroom3,4,5,6. Insulineresistentie is niet alleen een manifestatie van de ziekte, maar er wordt gedacht dat het bijdraagt aan de pathogenese door de werking van luteïniserend hormoon in de eierstok te versterken, wat leidt tot een verhoogde androgeenproductie7,8. Insulineresistentie wordt verondersteld verschillende mogelijke oorzaken te hebben, maar studies suggereren dat het te wijten kan zijn aan abnormale patronen van insulinereceptorsignalering9,10. Studies hebben insulineresistentie bij PCOS-patiënten geëvalueerd met behulp van de gouden standaardtechniek van hyperinsulinemisch-euglycemische clamp11,12,13,14,15. Vrouwen met PCOS, ongeacht de BMI, hebben hogere niveaus van insulineresistentie in vergelijking met controles. Insulinecontrole over glucoseproductie is verminderd bij aandoeningen van insulineresistentie die leiden tot overmatige glucoseproductie. Diabetespatiënten hebben bijvoorbeeld verhoogde gluconeogenese en verminderde onderdrukking van glycogenolyse16. Bovendien is een verminderde onderdrukking van de glucoseproductie waargenomen bij diabetische ratten17. Hoewel klemstudies een meting van insulineresistentie kunnen geven, richten weinig studies in PCOS zich op directe meting van glucoseproductie in nuchtere en gevoede toestanden. Dit vereist het gebruik van een niet-radioactieve isotopische glucose tracer infusie en meting via massaspectrometrie.
Diermodellen zijn op grote schaal gebruikt in PCOS-onderzoek. Zowel magere als zwaarlijvige PCOS-muizenmodellen zijn gemaakt door androgenen prenataal, prepubertaal of post-pubertaal toe te dienen18. Knaagdier PCOS-modellen tonen ook metabole verschillen in vergelijking met hun respectieve controles. Eerdere gegevens uit ons laboratorium toonden abnormale glucosetolerantietests (GTT) aan in PCOS-muismodellen (mager en zwaarlijvig), in overeenstemming met de menselijke PCOS-literatuur19. Het gebruik van een mager en zwaarlijvig diermodel maakt verder onderzoek naar metabole verschillen mogelijk. In het bijzonder maakt dit model het mogelijk om de snelheid van glucoseproductie rechtstreeks te evalueren met behulp van isotopische glucose tracers. Een van de meest gebruikte stabiele isotopische glucose tracers is [6,6-2H2]glucose. De [6,6-2H2]glucoseverrijking kan worden gemeten met behulp van een pentaacetaatderivaat zoals eerder beschreven20.
In deze studie was ons doel om de snelheid van hepatische glucoseproductie in nuchtere en glucoserijke toestand bij PCOS-muizen te meten met behulp van isotopische glucose-infusie. Deze technieken kunnen worden toegepast op een breed scala aan experimenten met glucosekinetiek.
Hyperglycemie en abnormaal glucosemetabolisme / homeostase zijn kenmerken van PCOS. De bloedsuikerspiegel wordt gehandhaafd door een combinatie van glucose uit voeding en glucoseproductie via glycogenolyse en gluconeogenese en glycogenese, onder controle van hormoon en enzymen. De productie van leverglucose wordt onderdrukt door de aanwezigheid van verhoogde circulerende glucosespiegels.
