Dit protocol presenteert een nieuw, robuust en reproduceerbaar kweeksysteem om driedimensionale sferoïden uit Caco2 colon adenocarcinoomcellen te genereren en te laten groeien. De resultaten vormen de eerste proof-of-concept voor de geschiktheid van deze aanpak om kankerstamcelbiologie te bestuderen, inclusief de respons op chemotherapie.
Colorectale kankers worden gekenmerkt door heterogeniteit en een hiërarchische organisatie bestaande uit een populatie van kankerstamcellen (CSC’s) die verantwoordelijk zijn voor tumorontwikkeling, onderhoud en resistentie tegen geneesmiddelen. Een beter begrip van CSC-eigenschappen voor hun specifieke targeting is daarom een vereiste voor effectieve therapie. Er is echter een tekort aan geschikte preklinische modellen voor diepgaand onderzoek. Hoewel in vitro tweedimensionale (2D) kankercellijnen waardevolle inzichten bieden in tumorbiologie, repliceren ze de fenotypische en genetische tumor heterogeniteit niet. Driedimensionale (3D)-modellen daarentegen pakken bijna-fysiologische kankercomplexiteit en cel heterogeniteit aan en reproduceren deze. Het doel van dit werk was om een robuust en reproduceerbaar 3D-cultuursysteem te ontwerpen om CSC-biologie te bestuderen. De huidige methodologie beschrijft de ontwikkeling en optimalisatie van omstandigheden om 3D-sferoïden, die homogeen van grootte zijn, te genereren uit Caco2 colon adenocarcinoomcellen, een model dat kan worden gebruikt voor langetermijncultuur. Belangrijk is dat binnen de sferoïden de cellen die waren georganiseerd rond lumenachtige structuren, werden gekenmerkt door differentiële celproliferatiepatronen en door de aanwezigheid van CSC’s die een paneel van markers uitdrukken. Deze resultaten vormen de eerste proof-of-concept voor de geschiktheid van deze 3D-benadering om cel heterogeniteit en CSC-biologie te bestuderen, inclusief de respons op chemotherapie.
Colorectale kanker (CRC) blijft de tweede belangrijkste oorzaak van kanker-geassocieerde sterfgevallen in de wereld1. De ontwikkeling van CRC is het resultaat van een progressieve verwerving en accumulatie van genetische mutaties en/of epigenetische veranderingen2,3, met inbegrip van de activering van oncogenen en inactivatie van tumoronderdrukkergenen3,4. Bovendien kunnen niet-genetische factoren (bv. het micromilieu) bijdragen tot en de oncogene transformatie bevorderen en zo deelnemen aan de ontwikkeling van CRC’s5. Belangrijk is dat CRC’s bestaan uit verschillende celpopulaties, waaronder ongedifferentieerde CSC’s en bulktumorcellen die enkele differentiatiekenmerken vertonen, die een hiërarchische structuur vormen die doet denken aan de organisatie van het epitheel in een normale coloncrypte 6,7.
CSC ‘s worden geacht verantwoordelijk te zijn voor het verschijnen vantumoren 8, het onderhoud en de groei ervan, de gemetastaseerde capaciteit en de resistentie tegen conventionele therapieën6,7. Binnen tumoren vertonen kankercellen, waaronder CSC’s, een hoog niveau van heterogeniteit en complexiteit in termen van hun verschillende mutatie- en epigenetische profielen, morfologische en fenotypische verschillen, genexpressie, metabolisme, proliferatiepercentages en gemetastaseerd potentieel9. Daarom, om kankerbiologie, tumorprogressie en het verwerven van resistentie tegen therapie en de vertaling ervan in effectieve behandelingen beter te begrijpen, zijn menselijke preklinische modellen die deze kanker heterogeniteit en hiërarchie vastleggen belangrijk10,11.
In vitro 2D-kankercellijnen worden al lange tijd gebruikt en bieden waardevolle inzichten in tumorontwikkeling en de mechanismen die ten grondslag liggen aan de werkzaamheid van therapeutische moleculen. Hun beperking met betrekking tot het ontbreken van de fenotypische en genetische heterogeniteit gevonden in de oorspronkelijke tumoren wordt nu echter algemeen erkend12. Bovendien worden nutriënten, zuurstof, pH-gradiënten en het micromilieu van de tumor niet gereproduceerd, waarbij het micromilieu vooral belangrijk is voor het behoud van verschillende celtypen, waaronder CSC’s11,12. Om deze belangrijkste nadelen te overwinnen, zijn verschillende 3D-modellen ontwikkeld om de complexiteit en heterogeniteit van kankers experimenteel aan te pakken en te reproduceren. In feite vatten deze modellen tumor cellulaire heterogeniteit, cel-cel interacties en ruimtelijke architectuur samen, vergelijkbaar met die waargenomen in vivo12,13,14. Primaire tumororganoïden die zijn vastgesteld op basis van verse tumoren, evenals van cellijn afgeleide sferoïden, worden grotendeels gebruikt15,16.
Sferoïden kunnen op een steigervrije of steigergebaseerde manier worden gekweekt om de cellen te dwingen zich te vormen en te groeien in celaggregaten. Steigervrije methoden zijn gebaseerd op de cultuur van cellen onder niet-aanhangende omstandigheden (bv. de ophangingsmethode of ultralage bevestigingsplaten), terwijl steigermodellen afhankelijk zijn van natuurlijke, synthetische of hybride biomaterialen voor kweekcellen12,13,14. Op steigers gebaseerde sferoïden hebben verschillende nadelen, omdat de uiteindelijke sferoïdevorming afhankelijk is van de aard en samenstelling van het gebruikte (bio)materiaal. Hoewel de tot nu toe beschikbare steigervrije sferoïdemethoden niet afhankelijk zijn van de aard van het substraat, genereren ze sferoïden die variëren in structuur en grootte17,18.
Dit werk was gericht op het ontwerpen van een robuust en reproduceerbaar 3D-kweeksysteem van sferoïden, die homogeen van grootte zijn, samengesteld uit Caco2 colon adenocarcinoomcellen om CSC-biologie te bestuderen. Caco2-cellen zijn van bijzonder belang vanwege hun vermogen om in de loop van de tijd19,20te differentiëren , wat sterk wijst op een stamachtig potentieel. Dienovereenkomstig onthulde de langetermijncultuur van de sferoïden de aanwezigheid van verschillende CSC-populaties met verschillende reacties op chemotherapie.
In vitro 3D-modellen overwinnen de belangrijkste experimentele nadelen van 2D-kankercelculturen, omdat ze betrouwbaarder lijken te zijn bij het samenvatten van typische tumorkenmerken, waaronder micromilieu en cel heterogeniteit. Veelgebruikte 3D-modellen van sferoïden zijn steigervrij (gekweekt in omstandigheden met weinig bevestiging) of steigergebaseerd (met behulp van biomaterialen om cellen te gekweekt).
