Il documento corrente descrive come valutare tre clinicamente rilevanti parametri di outcome primario di performance cardiaca e danni in un modello di infarto miocardico acuto maiale.
Mortality after acute myocardial infarction remains substantial and is associated with significant morbidity, like heart failure. Novel therapeutics are therefore required to confine cardiac damage, promote survival and reduce the disease burden of heart failure. Large animal experiments are an essential part in the translational process from experimental to clinical therapies. To optimize clinical translation, robust and representative outcome measures are mandatory. The present manuscript aims to address this need by describing the assessment of three clinically relevant outcome modalities in a pig acute myocardial infarction (AMI) model: infarct size in relation to area at risk (IS/AAR) staining, 3-dimensional transesophageal echocardiography (TEE) and admittance-based pressure-volume (PV) loops. Infarct size is the main determinant driving the transition from AMI to heart failure and can be quantified by IS/AAR staining. Echocardiography is a reliable and robust tool in the assessment of global and regional cardiac function in clinical cardiology. Here, a method for three-dimensional transesophageal echocardiography (3D-TEE) in pigs is provided. Extensive insight into cardiac performance can be obtained by admittance-based pressure-volume (PV) loops, including intrinsic parameters of myocardial function that are pre- and afterload independent. Combined with a clinically feasible experimental study protocol, these outcome measures provide researchers with essential information to determine whether novel therapeutic strategies could yield promising targets for future testing in clinical studies.
L'insufficienza cardiaca con ridotta frazione di eiezione (HFrEF) rappresenta circa il 50% di tutti i casi di insufficienza cardiaca, colpisce circa 1 – 2% delle persone nel mondo occidentale 1. La causa più comune è l'infarto miocardico acuto (IMA). Come mortalità acuta dopo IMA è diminuito in modo significativo a causa di una maggiore consapevolezza e migliori opzioni di trattamento, l'enfasi si è spostata verso la sua sequele croniche; l'essere più importante HFrEF 2,3. Insieme con l'aumento dei costi sanitari 4, la crescente epidemia di insufficienza cardiaca sottolinea la necessità di nuove diagnosi e terapie, che può essere studiato in un modello suino altamente traslazionale di rimodellamento avverso dopo IMA come descritto in precedenza 5.
Entrambi, determinanti (ad esempio, dimensione dell'infarto) e funzionali valutazioni (ad esempio, l'ecocardiografia) di rimodellamento avverso sono spesso utilizzati per le prove di efficacia di nuove terapie, che indica la necessità di relmetodi iAble e relativamente poco costoso. Lo scopo della carta corrente è affrontare questa esigenza introducendo importanti e affidabili outcome per prove di efficacia in un modello suino di infarto miocardico acuto. Questi includono la dimensione infartuale (IS) in relazione alla zona a rischio (AAR), 3D l'ecocardiografia transesofagea (3D-TEE) e l'acquisizione ciclo dettagliata pressione-volume ammissione a base di (PV).
Dimensione dell'infarto è il principale determinante di rimodellamento avverso e la sopravvivenza dopo IMA 6. Anche se tempestiva riperfusione del miocardio ischemico può salvare cardiomiociti reversibilmente feriti e limitare le dimensioni dell'infarto, riperfusione per sé provoca ulteriori danni attraverso la generazione di stress ossidativo e una risposta infiammatoria sproporzionata (danno da ischemia-riperfusione (IRI)) 7. Quindi, IRI è stato identificato come un obiettivo terapeutico promettente. La capacità di nuove terapie per diminuire la dimensione dell'infarto è quantificato valutando dimensioni dell'infarto in relazionealla zona a rischio (AAR). AAR quantificazione è obbligatoria per correggere variabilità inter-individuale in anatomia coronarica di modelli animali, come AAR grande porta ad una più ampia dimensione dell'infarto assoluto. Dal momento che la dimensione dell'infarto è direttamente correlata alla funzione cardiaca e della contrattilità miocardica, variazioni di AAR possono influenzare studiare misure di outcome a prescindere dalla modalità di trattamento 8.
Tridimensionale transesofagea (3D-TEE) è un metodo poco costoso sicuro, affidabile e, soprattutto, clinicamente applicabile per misurare la funzione cardiaca in modo non invasivo. Mentre ecocardiografia transtoracica (TTE) immagini sono limitati alle viste 2D lungo parasternale e asse corto nei suini 9, 3D-TEE può essere utilizzato per ottenere la completa immagini 3-dimensionali del ventricolo sinistro. Pertanto, non richiede approssimazioni matematiche di ventricolo sinistro (LV) volumi come regola modificata di Simpson 10. Quest'ultimo è a corto di correctly stima volumi LV dopo LV rimodellamento causa della mancanza di geometria cilindrica 11. Inoltre, 3D-TEE è preferibile rispetto ecocardiografia epicardico in quanto non richiede interventi chirurgici, che sono stati osservati per esercitare effetti cardioprotettivi del presente modello 12. Sebbene l'uso di 2D-TEE per la valutazione della funzione miocardica è stata descritta prima 13,14, limitazioni per quanto riguarda la geometria ventricolare sono simili a quelli osservati in 2D-TTE e dipendono dal grado di LV rimodellamento. Quindi, maggiore è l'infarto (e quindi maggiore è la probabilità di insufficienza cardiaca), le misurazioni 2D più probabili diventano viziata da ipotesi geometriche errate e maggiore è la necessità di tecniche 3D.
Tuttavia, la maggior parte delle modalità di imaging sono limitati nella loro capacità di valutare intrinseche proprietà funzionali del miocardio. PV loop fornire tali informazioni supplementari pertinenti e la loro acquisizione è pertantodescritto in dettaglio nel seguito.
