The flow mediated dilation (FMD) test is the most commonly utilized, non-invasive, ultrasound assessment of endothelial function in humans. Although the FMD test has been related with the prediction of future cardiovascular disease and events, it is a physiological assessment with many inherent confounding factors that need to be considered.
Cardiovasculaire ziekte is de belangrijkste doodsoorzaak en de belangrijkste oorzaak van invaliditeit wereldwijd. Het disfunctioneren van het vasculaire endotheel is een pathologische aandoening vooral gekenmerkt door een verstoring van het evenwicht tussen vaatverwijdende stoffen en vasoconstrictor en wordt voorgesteld een belangrijke rol spelen bij de ontwikkeling van atherosclerotische cardiovasculaire ziekte. Daarom is een nauwkeurige evaluatie van de functie van het endotheel bij de mens een belangrijk instrument dat beter zou kunnen helpen inzicht in de etiologie van verschillende cardio-centric pathologieën.
Gedurende de afgelopen vijfentwintig jaren hebben vele methodieken ontwikkeld om de beoordeling van de endotheelfunctie bij mensen te verschaffen. Geïntroduceerd in 1989, de MKZ-test is voorzien van een onderarm occlusie en de daaropvolgende reactieve hyperemie dat de productie van stikstofoxide en vaatverwijding van de arteria brachialis bevordert. De MKZ-test is nu de meest gebruikte, niet-invasieve, UltrAsonic beoordeling van de endotheelfunctie bij mensen en is in verband met toekomstige cardiovasculaire gebeurtenissen.
Hoewel de FMD proef klinische bruikbaarheid kan hebben, is een fysiologisch beoordelingsprocedure die meerdere verstorende factoren die moeten worden beschouwd geërfd. Dit artikel beschrijft een gestandaardiseerd protocol voor het bepalen FMD inclusief de aanbevolen methode om de fysiologische en technische problemen minimaliseren en de nauwkeurigheid en de reproduceerbaarheid van de beoordeling.
Cardiovasculaire ziekte is de belangrijkste oorzaak van morbiditeit en mortaliteit wereldwijd. Disfunctie van het vasculaire endotheel geeft een eerste fase naar de ontwikkeling van meervoudige-vasculaire ziekten 1. Dus een nauwkeurige vaststelling van de endotheelfunctie bij mensen is een belangrijke techniek die kan helpen bij het begrijpen van de etiologie van multiple cardiovasculaire pathologieën, met als uiteindelijk doel het verbeteren van de effectiviteit van de behandeling en preventie van ziekten.

Het endotheel is een monolaag van cellen die talrijke vasoactieve substanties, zoals stikstofoxide (NO), prostacycline, endothelinen, endotheelcelgroeifactor, interleukinen en plasminogeen inhibitoren 2 synthetiseert. Dergelijke factoren dragen bij aan de functie van het endotheel om bloed vloeibaarheid, vasculaire tonus, de aggregatie van bloedplaatjes, doorlaatbaarheid van plasma componenten en vaatwand infl regulerenammation 2-4. Bovendien NO speelt een belangrijke anti-atherogene rol bij het bevorderen vasodilatatie en onderhouden van endotheliale integriteit. NO reguleert vaattonus en de diameter tot het regelen van de balans tussen de afgifte van zuurstof naar de weefsels en de metabolische vraag 3,5. Er zijn meerdere endogene, exogene en mechanische stimulator factoren induceren endotheliale NO synthase (eNOS) die synthetiseert NO uit L-arginine 6,7. De meest opvallende mechanische stimulus is shear stress. Afschuifspanning bijdragen tot meer activering van eNOS, waardoor NO productie en daaropvolgende gladde spierrelaxatie 4. Daarom heeft de afname NO biobeschikbaarheid wordt vaak gebruikt als een maat voor endotheeldisfunctie 8.

De onbalans tussen vaatverwijdende en vasoconstrictor factoren leidt tot een disfunctionele endotheel 2. Bovendien, de release van ontstekingsmediatoren en veranderde lokale dwarskrachten kan de synthese van endotheelcellen afgeleid reactive oxygen species (ROS) te verbeteren. Deze opregulatie in redox signalering niet alleen wijzigt de integriteit van het endotheel en vermindert de synthese van NO 9 kan eNOS loskoppelen waardoor directe productie van extra vrije radicalen. De mate van disfunctioneren van het endotheel is geassocieerd met de ernst van pathologieën zoals hypertensie 10, 11 atherosclerose, ischemische beroerte 12, 13 diabetes, pre-eclampsie 14 of nierziekten 15 onder anderen. Daarom is er grote belangstelling om niet alleen veranderingen in endotheelfunctie tijd kan meten, maar ook na therapeutische interventies. Verschillende werkwijzen zijn gebruikt voorde klinische beoordeling van de endotheelfunctie zowel invasief (hartkatheterisatie en veneuze occlusie plethysmografie 3,16) en niet-invasief (stroom gemedieerde dilatatie, radiale slagader tonometrie en hartslag contouranalyse 4,17,18) in coronaire en perifere circulatie 19.
Flow gemedieerde dilatatie
Flow gemedieerde dilatatie (FMD) is een niet-invasieve ultrasone evaluatie van endotheelfunctie en is gecorreleerd met de ontwikkeling van vasculaire gezondheidsproblemen. Sinds haar oprichting in 1989 20, heeft MKZ is op grote schaal gebruikt als een betrouwbare, in vivo methode om voornamelijk NO-gemedieerde endotheliale functie bij de mens 19,21,22 evalueren. Inderdaad, de slagader MKZ-test in verband gebracht met andere invasieve technieken 23 en tal van onderzoeken hebben beschreven een sterke omgekeerde relatie tussen MKZ en cardiovasculaire verwondingen 24,25 zodanig dat indduals met meer vasculaire pathologie vertonen een lagere MKZ-25.
