The lymphodepletive and immunomodulatory effects of chemotherapy and radiation standard of care can be leveraged to enhance the antitumor efficacy of T cell immunotherapy. We outline a method for generating EGFRvIII-specific chimeric antigen receptor (CAR) T cells and administering them in the context of glioblastoma standard of care.
Adoptive T cell immunotherapy offers a promising strategy for specifically targeting and eliminating malignant gliomas. T cells can be engineered ex vivo to express chimeric antigen receptors specific for glioma antigens (CAR T cells). The expansion and function of adoptively transferred CAR T cells can be potentiated by the lymphodepletive and tumoricidal effects of standard of care chemotherapy and radiotherapy. We describe a method for generating CAR T cells targeting EGFRvIII, a glioma-specific antigen, and evaluating their efficacy when combined with a murine model of glioblastoma standard of care. T cells are engineered by transduction with a retroviral vector containing the anti-EGFRvIII CAR gene. Tumor-bearing animals are subjected to host conditioning by a course of temozolomide and whole brain irradiation at dose regimens designed to model clinical standard of care. CAR T cells are then delivered intravenously to primed hosts. This method can be used to evaluate the antitumor efficacy of CAR T cells in the context of standard of care.
Glioblastoom (GBM) is de meest voorkomende primaire kwaadaardige hersentumor en is altijd fataal. Chirurgische resectie gekoppeld aan aspecifieke zorgstandaard chemotherapie en radiotherapie niet volledig te elimineren kwaadaardige cellen, resulterend in een sombere prognose van minder dan 15 maanden bij patiënten met deze ziekte 1. Daarentegen immunotherapie biedt een nauwkeurige benadering specifiek richten op tumorcellen, en heeft dus de potentie om te dienen als een zeer effectieve behandeling platform met verminderd risico van toxiciteit zekerheden 2-4. T-cellen ontwikkeld ex vivo te uiten chimeer antigen receptoren (auto's) bieden een veelzijdige strategie voor tumor immunotherapie. Auto's worden gegenereerd door het fuseren van het extracellulaire variabele gebied van een antilichaam met één of meer intracellulaire T-cel signalerende moleculen (s), in plaats van een full-length major histocompatibility complex (MHC) -beperkte T-cel receptor 5. Deze wijze van antilichaam-achtige antigeen erkennOp maakt reactieve antigen-specifieke T-cellen te herkennen en tumorantigenen in afwezigheid van MHC en kan worden aangepast voor een virtueel oneindig antigeen repertoire.
CAR T-cellen gemanipuleerd tegen diverse tumor antigenen aangetoond preklinische werkzaamheid en uitstekende belofte in de kliniek 6-9. Met name in de context van GBM, auto T cel targeting epidermale groeifactorreceptor variant III (EGFRvIII), een tumor-specifieke mutatie expressie op het celoppervlak 10, bleek de overleving verlengen-glioom dragende muizen 11. Ondanks hun veelzijdigheid, maar de klinische voordelen van CAR adoptieve therapie is nog niet volledig gerealiseerd, mede als gevolg van tumor-geassocieerde immunosuppressie en het immuunsysteem belastingontduiking 12-16 evenals uitdagingen bij het opzetten en onderhouden van antigeen-specifieke T-cellen in vivo. Gebruik te maken van standaard van zorg (SOC) met immunotherapie kan mogelijk overwinnen een aantal van deze limitaties, resulterend in verhoogde werkzaamheid in zowel de preklinische en klinische setting.
SOC voor post-resectie GBM bestaat uit een hoge dosis temozolomide (TMZ), een DNA-alkylerende stof 17, en het hele brein bestraling (WBI) 1. Deze behandelingen worden geacht om synergie met tumor vaccins via opregulatie van tumor MHC expressie 18-20 en het loskomen van antigenen door dode tumorcellen 17,19,21,22. Inderdaad, de toevoeging van TMZ 20,23 of 18,24 WBI leidt tot verhoogde antitumorale werkzaamheid van immuun gebaseerde behandelingen in de preklinische omgeving. Bovendien, net als vele niet-specifieke cytotoxische chemotherapeutica, TMZ bekend systemische lymfopenie 25,26, die kan worden benut als middel gastheer-conditioning voor adoptieve therapie platforms 27-29 veroorzaken. TMZ-gemedieerde lymphodepletion is aangetoond dat de frequentie en de functie van de antigeen-specifieke T-cellen verbeteren, wat leidt tot verhoogde werkzaamheid van een Adoptieve therapie platform tegen intracraniële tumoren 30. In de context van CAR therapie lymphodepletion fungeert als middel gastheer-conditioning zowel vermindering van het aantal endogene suppressor-T-cellen 31 en induceren homeostatische proliferatie 32 via verminderde concurrentie voor cytokinen 33, waardoor antitumoractiviteit 11,34 verbeteren. Gezien de synergetische relatie tussen GBM SOC en immunotherapie platforms evalueren adoptieve nieuwe therapieën en vaccins platforms in de context van SOC is essentieel voor het trekken van zinvolle conclusies inzake werkzaamheid.
In dit protocol beschrijven we een werkwijze voor het genereren en intraveneuze toediening van muizen EGFRvIII specifieke CAR T-cellen naast TMZ en WBI in muizen met EGFRvIII-positieve intracraniale tumoren (zie Figuur 1 voor de behandeling tijdlijn). In het kort worden AUTO T-cellen gemaakt door retrovirale transductie. Menselijke embryonale nier (HEK) 293T cellen worden getransfecteerd met een DNA / lipide-complex (met de CAR vector en PCL-Eco plasmiden) virus, die vervolgens wordt gebruikt om geactiveerde muriene splenocyten die zijn geoogst en gekweekt in parallel transduceren produceren. Tijdens CAR generatie, zijn murine gastheren dragen EGFRvIII-positieve intracraniale tumoren toegediend gefractioneerde hele brein röntgenbestraling en systemische TMZ behandeling bij doses vergelijkbaar klinische SOC. AUTO T-cellen worden vervolgens intraveneus te lymphodepleted gastheren geleverd.
De volgende procedure wordt beschreven in zeven afzonderlijke fasen: (1) Toediening van temozolomide tumordragende muizen, (2) Whole Brain Bestraling van tumordragende muizen, (3) transfectie (4) Splenectomie en T cel bereiding, (5 ) Transduction, (6) AUTO T-cel Cultuur en Harvest, en (7) AUTO T-cel toediening aan muizen met een tumor. Deze fasen bestaan uit verschillende stappen die 6-7 dagen bestrijken en tegelijkertijd worden uitgevoerd.
De behandeling tijdlijn hier beschreven is ontworpen voor het modelleren klinische kwaliteit van de zorg en het benutten ervan bij de CAR adoptieve therapie. CAR T cel doses TMZ regimes en radiotherapie toediening kan worden gemodificeerd ter verbetering van in vivo T-cel activiteit lymphodepletion en tumor doden.
