Het protocol beschrijft een methode voor Tuina-interventie in een konijnenmodel van knieartrose.
Artrose van de knie (KOA) wordt voornamelijk gekenmerkt door degeneratieve veranderingen in het kraakbeen van het kniegewricht en de omliggende zachte weefsels. De werkzaamheid van Tuina bij de behandeling van KOA is bevestigd, maar het onderliggende mechanisme moet worden onderzocht. Deze studie heeft tot doel een wetenschappelijk haalbaar KOA-konijnenmodel op te stellen dat is behandeld met Tuina om de onderliggende mechanismen bloot te leggen. Hiervoor werden 18 mannelijke Nieuw-Zeelandse konijnen van 6 maanden oud willekeurig verdeeld in schijn-, model- en Tuina-groepen, met 6 konijnen in elke groep. Het KOA-model is tot stand gekomen door 4% papaïne-oplossing in de holte van het kniegewricht te injecteren. De Tuina-groep werd gedurende 4 weken geïntervenieerd met Tuina in combinatie met de kniegewricht-rotatiecorrectiemethode. Alleen het standaard grijpen en fixeren werd uitgevoerd in schijn- en modelgroepen. Aan het einde van de interventie van 1 week werd het bewegingsbereik (ROM) van het kniegewricht waargenomen en werd kraakbeen hematoxylin-eosine (HE) kleuring uitgevoerd. De studie toont aan dat Tuina de apoptose van chondrocyten kan remmen, kraakbeenweefsel kan herstellen en ROM van het kniegewricht kan herstellen. Concluderend toont deze studie de wetenschappelijke haalbaarheid aan van Tuina-behandeling voor KOA-modelkonijnen, waarbij de potentiële toepassing ervan in de studie van KOA en soortgelijke kniegewrichtgerelateerde aandoeningen wordt benadrukt.
Artrose van de knie (KOA) is een degeneratieve ziekte van het kniegewricht, die zich voornamelijk manifesteert door kniepijn, zwelling, vervorming en bewegingsbeperking, met een hoog invaliditeitspercentage en een hogere prevalentie bij vrouwen, met 527,81 miljoen patiënten met artrose wereldwijd in 2019 en de wereldwijde prevalentie ervan is goed voor 60,6% van de totale wereldwijde prevalentie van artrose1. Klinisch wordt de behandeling van KOA meestal onderverdeeld in niet-chirurgische en chirurgische therapieën. Niet-chirurgische therapieën omvatten fysiotherapie, farmacotherapie en bloedplaatjesrijke plasma-injectietherapie 2,3. Tuina is een veelvoorkomende, veilige, betrouwbare en effectieve behandelmethode in de Chinese geneeskunde. Deze studie maakt gebruik van Tuina in combinatie met de roterende correctiemethode van het kniegewricht om KOA te behandelen. Tuina-technieken zoals de roterende kneed- en persmethode kunnen spierweefsel in evenwicht brengen, pijn verminderen, ontstekingsfactorniveaus aanpassen, het weefselmetabolisme verbeteren en gewrichtskraakbeendegeneratie remmen 4,5. De roterende correctiemethode van het kniegewricht kan de uitlijning van de botten en gewrichten van de onderste ledematen aanpassen, de opening van het kniegewricht verbeteren, de normale krachtlijn herstellen en de biomechanica van de onderste ledematen in evenwicht brengen 6,7,8,9. Weerstandsoefeningen kunnen de spiermassa en -kracht vergroten en de vernieuwing van kraakbeenweefsel bevorderen10,11. Een voorlopige studie wees uit dat dit Tuina-protocol significant effectiever is dan orale glucosaminesulfaatcapsules bij de behandeling van KOA, met een sneller begin van de werking en een significante remming van chondrocytendegeneratie en herstel van beschadigd kraakbeenweefsel12. Bij de behandeling van KOA hebben niet-steroïde anti-inflammatoire geneesmiddelen, in vergelijking met Tuina-therapie, bijwerkingen en een onbevredigende werkzaamheid op lange termijn, relatief hoge chirurgische risico’s en kosten, en vereisen ze bepaalde indicaties voor chirurgische behandeling, met postoperatieve problemen en periprothetische complicaties13,14,15. In vergelijking met medicamenteuze therapie en chirurgie biedt de behandeling van Tuina voor KOA verschillende voordelen, waaronder minder bijwerkingen, lager risico, verbeterde veiligheid, kosteneffectiviteit en langdurige werkzaamheid. Bovendien kan het effectief pijn in het kniegewricht, zwelling, knallen en bewegingsbeperking verlichten 6,13,16,17.
Het mechanisme van Tuina voor de behandeling van KOA moet echter worden verduidelijkt, wat de verbetering en perfectie van het behandelprotocol voor KOA beperkt. Daarom is het bestuderen van het mechanisme van Tuina-interventie in KOA door middel van dierproeven een effectieve methode. Konijnen hebben, in vergelijking met ratten, een volgzaam temperament en grotere kniegewrichten. De anatomische structuur en de biochemische indexen van het kraakbeen zijn vergelijkbaar met die van mensen, dus het is een geschikt onderwerp voor het bestuderen van het mechanisme van kniegewrichtsaandoeningen door Tuina18. Het KOA-model dat tot stand is gekomen door papaïne in de holte van het kniegewricht van konijnen te injecteren, heeft de voordelen van een korte modelleringstijd, minder trauma, een hoog slagingspercentage, een hoog overlevingspercentage en een vergelijkbaar pathologisch mechanisme als KOA19. Deze studie heeft tot doel een wetenschappelijk haalbaar dierexperimenteel protocol voor Tuina-interventie in KOA vast te stellen en het mechanisme van Tuina te onderzoeken.
Het ontwerp van het experimentele protocol is met name belangrijk om het mechanisme van Tuina bij de behandeling van KOA te onderzoeken. KOA-modellering werd uitgevoerd op konijnen door middel van injectie met papaïne in Waixiyan.
