Geeignete klinische und Laborstrategien gewährleisten maximale Wirksamkeit, Effizienz und Sicherheit von Fruchtbarkeitserhaltungsbehandlungen.
Die Erhaltung der weiblichen Fruchtbarkeit ist in einem multifunktionalen Gesundheitssystem, das sich um die zukünftige Lebensqualität der Patienten kümmert, von entscheidender Bedeutung. Die Kryokonservierung von Eizellen wird von mehreren internationalen wissenschaftlichen Gesellschaften als Goldstandard für die Erhaltung der Fruchtbarkeit bei postpubertären Frauen anerkannt, sowohl für medizinische als auch für nicht-medizinische Indikationen. Die wichtigsten medizinischen Indikationen sind onkologische Erkrankungen, gynäkologische Erkrankungen wie schwere Endometriose, systemische Erkrankungen, die die Eierstockreserve beeinträchtigen, und genetische Erkrankungen mit vorzeitiger Menopause. Dieses Papier beschreibt die gesamte klinische und Laboraufarbeitung einer Fruchtbarkeitserhaltungsbehandlung, indem es Empfehlungen für eine objektive und evidenzbasierte Beratung skizziert. Darüber hinaus konzentriert es sich auf die Wirksamkeit des Verfahrens und beschreibt die am besten geeigneten Strategien, um die Eierstockreserve vollständig auszuschöpfen und die Anzahl der in kürzester Zeit entnommenen Eizellen zu maximieren. Die Bewertung der Eierstockreserve, die Definition eines idealen Stimulationsprotokolls sowie die Verfahren zur Entnahme, Denudation und Vitrifikation von Eizellen wurden zusammen mit Ansätzen zur Maximierung ihrer Wirksamkeit, Effizienz und Sicherheit detailliert beschrieben.
Die Entwicklung und Implementierung eines effizienten Kryokonservierungsprogramms für menschliche Eizellen war ein bedeutender Durchbruch in der Reproduktionsmedizin. Jüngsten Erkenntnissen zufolge ist die Vitrifikation die effektivste Strategie zur Kryokonservierung von Eizellen der Metaphase II (MII), da sie unabhängig von der Patientenpopulation (unfruchtbare Patientinnen oder Eizellspendeprogramm) zu statistisch höheren Überlebensraten im Vergleich zum langsamen Einfrieren führt1,2,3. Die bemerkenswerten Errungenschaften der Eizellvitrifikation veranlassten die Praxiskomitees der American Society for Reproductive Medicine (ASRM) und der Society for Assisted Reproductive Technology (SART), diese Technik als die effektivste für die elektive Fruchtbarkeitserhaltung bei postpubertären Frauen zu erklären, sowohl für medizinische als auch für nicht-medizinische Indikationen4,5,6. Medizinische Indikationen zur Erhaltung der Fruchtbarkeit umfassen (i) Krebs und Autoimmunerkrankungen, die Therapien7 wie Strahlentherapie, zytotoxische Chemotherapie und endokrine Therapie erfordern (deren schädliche Wirkung auf die Eierstockreserve mit dem Alter der Mutter sowie art und dosis der Behandlung verbunden ist); ii) Eierstockerkrankungen, die eine wiederholte oder radikale Operation erfordern (z. B. Endometriose)8; und (iii) genetische Erkrankungen (z. B. X-fragile) oder vorzeitiges Ovarialversagen. Darüber hinaus ist die Erhaltung der Fruchtbarkeit zu einer wertvollen Option für alle Frauen geworden, die ihr elterliches Ziel aus nicht-medizinischen Gründen nicht erreicht haben (auch bekannt als Social Freezing).
Unabhängig von der Indikation zur Erhaltung der Fruchtbarkeit und gemäß den wichtigsten internationalen Richtlinien zur Erhaltung der Fruchtbarkeit sollten alle Patientinnen, die bereit sind, ihre Eizellen zu vitrifizieren, eine angemessene Beratung erhalten, um über ihre realistischen Erfolgschancen, die Kosten, Risiken und Einschränkungen des Verfahrens informiert zu werden9,10,11,12,13. Am wichtigsten ist, dass klar sein sollte, dass die Vitrifizierung einer Kohorte von MII-Eizellen keine Schwangerschaft gewährleistet, sondern dass sie eine höhere Erfolgschance für die zukünftige In-vitro-Fertilisation (IVF) bietet, falls erforderlich14. In dieser Hinsicht ist das Alter der Frau zum Zeitpunkt der Vitrifizierung der Eizellen sicherlich der wichtigste limitierende Faktor15, da das fortgeschrittene mütterliche Alter (AMA; >35 Jahre) die Hauptursache für weibliche Unfruchtbarkeit ist16. Neben einer fortschreitenden Verringerung der Ovarialreserve ist AMA mit einer Beeinträchtigung der Eizellkompetenz aufgrund defekter physiologischer Wege wie Stoffwechsel, epigenetische Regulation, Zellzyklus-Checkpoints und meiotische Segregation verbunden17. Daher hängt die angemessene Anzahl der zu vitrifizierenden Eier hauptsächlich vom Alter der Mutter ab. Bei Frauen unter 36 Jahren sind mindestens 8-10 MII-Eizellen18 erforderlich, um die Erfolgschancen zu maximieren. Im Allgemeinen gilt: Je höher die Anzahl der vitrifizierten Eizellen, desto höher ist die Erfolgswahrscheinlichkeit. Daher ist die Anpassung der ovariellen Stimulation an ovariellen Reservemarkern wie Anti-Müller-Hormon (AMH) oder Antralfollikelzahl (AFC) entscheidend, um die Eierstockreserve in kürzester Zeit voll auszuschöpfen.
Die Sicherheit des gesamten Verfahrens ist das andere Schlüsselthema bei der Aufnahme von Patienten zur Erhaltung der Fruchtbarkeit. Kliniker sollten die besten Strategien anwenden, um die Risiken zu minimieren und (i) das ovarielle Hyperstimulationssyndrom (OHSS) zu verhindern, indem sie sichere Ansätze wie das Gonadotrophin-Releasing-Hormon (GnRH) -Antagonistenprotokoll verwenden, gefolgt von einem GnRH-Agonistenauslöser19 und (ii) den entfernten, aber möglichen Risiken von Peritonealblutungen, Verletzungen der Beckenstrukturen (Harnleiter, Darm, Blinddarm, Nerven) oder Beckeninfektionen während der Eizellentnahme. Schließlich (iii) traditionelle Stimulationsschemata sind mit supraphysiologischem Serumestradiol verbunden und werden daher bei östrogenempfindlichen Erkrankungen wie Brustkrebs nicht empfohlen. Protokolle mit Aromatasehemmern (wie Letrozol oder Tamoxifen) sind in diesen Fällen besser geeignet20,21. Im Labor ist das am weitesten verbreitete Protokoll zur Vitrifikation von Eizellen immer noch das von Kuwayama und Kollegen 2,23beschriebene Protokoll, das auseinemschrittweisen Verfahren besteht, bei dem Kryoprotektoren (CPAs) schrittweise hinzugefügt werden. In der ersten Phase (Gleichgewicht/Dehydratation) werden Eizellen in einer CPA-Lösung exponiert, die 7,5% v/v Ethylenglykol und 7,5% v/v Dimethylsulfoxid (DMSO) enthält, während in der zweiten Phase Eizellen in eine Vitrifikationslösung mit 15% v/v Ethylenglykol und 15% v/v DMSO plus 0,5 mol/L Saccharose bewegt werden. Nach einer kurzen Inkubation im Medium der Vitrifikation können die Eizellen in speziell entwickelte, offene Kryodevices gelegt und schließlich bei -196 °C in flüssigen Stickstoff getaucht werden, um bis zur Verwendung gelagert zu werden.
Hier wurde die gesamte klinische und Laboraufarbeitung einer Fruchtbarkeitserhaltungsbehandlung beschrieben, indem (i) Empfehlungen für eine objektive und evidenzbasierte Beratung skizziert wurden, (ii) die Kosteneffizienz des Verfahrens im Mittelpunkt stand und (iii) die am besten geeigneten Strategien beschrieben wurden, um die Eierstockreserve vollständig auszuschöpfen und die Anzahl der in kürzester Zeit entnommenen Eizellen zu maximieren. Die Bewertung der Eierstockreserve, die Definition eines idealen Stimulationsprotokolls sowie die Verfahren zur Entnahme, Denudation und Vitrifikation von Eizellen werden zusammen mit Ansätzen zur Maximierung ihrer Wirksamkeit, Effizienz und Sicherheit detailliert beschrieben. Da andere Protokolle oder Anpassungen dieses Protokolls in der Literatur existieren, gelten die repräsentativen Ergebnisse und die Diskussionsabschnitte dieses Manuskripts nur für dieses Verfahren.
Klinische Überlegungen
Obwohl neue Strategien wie die Kryokonservierung von Eierstockgewebe und die In-vitro-Reifung erforscht wurden, ist die Vitrifikation von Eizellen nach COS die Goldstandardtechnik zur Erhaltung der Fruchtbarkeit.
