Hier wordt een protocol gepresenteerd voor het niet-invasief monitoren van cerebrale hemodynamiek van neurokritische patiënten in realtime en aan het bed met behulp van diffuse optica. In het bijzonder maakt het voorgestelde protocol gebruik van een hybride diffuus optisch systeem om real-time informatie over cerebrale oxygenatie, cerebrale bloedstroom en cerebrale metabolisme te detecteren en weer te geven.
Neurofysiologische monitoring is een belangrijk doel bij de behandeling van neurokritische patiënten, omdat het secundaire schade kan voorkomen en rechtstreeks van invloed kan zijn op morbiditeit en sterftecijfers. Er is momenteel echter een gebrek aan geschikte niet-invasieve, real-time technologieën voor continue monitoring van de cerebrale fysiologie aan het bed. Diffuse optische technieken zijn voorgesteld als een potentieel hulpmiddel voor bedside metingen van cerebrale bloedstroom en cerebrale oxygenatie in het geval van neurokritische patiënten. Diffuse optische spectroscopieën zijn eerder onderzocht om patiënten te volgen in verschillende klinische scenario’s, variërend van neonatale monitoring tot cerebrovasculaire interventies bij volwassenen. De haalbaarheid van de techniek om clinici te helpen door real-time informatie aan het bed te verstrekken, blijft echter grotendeels onbesproken. Hier rapporteren we de vertaling van een diffuus optisch systeem voor continue real-time monitoring van de cerebrale bloedstroom, cerebrale oxygenatie en cerebrale zuurstofmetabolisme tijdens intensieve zorg. De real-time functie van het instrument zou behandelingsstrategieën mogelijk kunnen maken op basis van patiëntspecifieke cerebrale fysiologie in plaats van te vertrouwen op surrogaatstatistieken, zoals arteriële bloeddruk. Door real-time informatie te verstrekken over de cerebrale circulatie op verschillende tijdschalen met relatief goedkope en draagbare instrumentatie, kan deze aanpak vooral nuttig zijn in low-budget ziekenhuizen, in afgelegen gebieden en voor monitoring in open velden (bijv. Defensie en sport).
De meeste complicaties die leiden tot slechte resultaten voor ernstig zieke neurologische patiënten zijn gerelateerd aan secundaire verwondingen veroorzaakt door cerebrale hemodynamische stoornissen. Daarom kan het monitoren van de cerebrale fysiologie van deze patiënten rechtstreeks van invloed zijn op morbiditeits- en sterftecijfers 1,2,3,4,5,6,7. Momenteel is er echter geen gevestigde klinische tool voor de continue real-time niet-invasieve monitoring van cerebrale fysiologie bij neurokritische patiënten aan het bed. Onder de potentiële kandidaten zijn onlangs diffuse optische technieken voorgesteld als een veelbelovend hulpmiddel om deze leemteop te vullen 8,9,10,11. Door de langzame veranderingen (d.w.z. in de orde van tientallen tot honderden ms) van het diffuus verstrooide nabij-infrarode licht (~ 650-900 nm) van de hoofdhuid te meten, kan diffuse optische spectroscopie (DOS) concentraties van de belangrijkste chromoforen in de hersenen meten, zoals cerebrale oxy- (HbO) en deoxy-hemoglobine (HbR)12,13. Daarnaast is het mogelijk om de cerebrale bloedstroom (CBF) te meten met diffuse correlatiespectroscopie (DCS)10,14,15,16,17 door de snelle fluctuaties in lichtintensiteit (d.w.z. van enkele μs tot enkele ms) te kwantificeren. In combinatie kunnen DOS en DCS ook een schatting geven van de cerebrale stofwisseling van zuurstof (CMRO2)18,19,20.
De combinatie van DOS en DCS is onderzocht om patiënten in verschillende preklinische en klinische scenario’s te monitoren. Er is bijvoorbeeld aangetoond dat diffuse optica relevante klinische informatie biedt voor ernstig zieke pasgeborenen 21,22,23,24, inclusief tijdens hartoperaties om hartafwijkingen te behandelen 23,25,26,27,28 . Daarnaast hebben verschillende auteurs het gebruik van diffuse optica onderzocht om de cerebrale hemodynamiek te beoordelen tijdens verschillende cerebrovasculaire interventies, zoals carotis endarterectomie 29,30,31, trombolytische behandelingen voor beroerte 32, head-of-bed manipulaties 33,34,35, cardiopulmonale reanimatie 36, en anderen37,38, 39. Wanneer continue bloeddrukmonitoring ook beschikbaar is, kan diffuse optica worden gebruikt om cerebrale autoregulatie te controleren, zowel bij gezonde als bij ernstig zieke proefpersonen 11,40,41,42, evenals om de kritische sluitdruk van de cerebrale circulatie te beoordelen 43. Verschillende auteurs hebben CBF-metingen met DCS gevalideerd tegen verschillende gouden standaard CBF-metingen 18, terwijl CMRO2 gemeten met diffuse optica een nuttige parameter is gebleken voor neurokritische monitoring 8,18,23,24,28,43,44,45 . Bovendien hebben eerdere studies de optisch afgeleide cerebrale hemodynamische parameters gevalideerd voor langetermijnmonitoring van neurokritische patiënten 8,9,10,11, inclusief voor de voorspelling van hypoxische 46,47,48 en ischemische gebeurtenissen 8.
De betrouwbaarheid van de diffuse optische technieken om waardevolle real-time informatie te leveren tijdens longitudinale metingen en tijdens klinische interventies blijft grotendeels onbesproken. Het gebruik van een stand-alone DOS-systeem werd eerder vergeleken met invasieve zuurstofspanningsmonitoren van hersenweefsel en DOS werd geacht niet voldoende gevoelig te zijn om de invasieve monitoren te vervangen. Afgezien van het gebruik van relatief kleine populaties, kan de directe vergelijking van de invasieve en niet-invasieve monitoren echter misleidend zijn, omdat elke techniek verschillende volumes onderzoekt die verschillende delen van de cerebrale vasculatuur bevatten. Hoewel deze studies uiteindelijk concludeerden dat diffuse optica geen vervanging is voor de invasieve monitoren, bereikte DOS in beide studies een matige tot goede nauwkeurigheid, die voldoende kan zijn voor gevallen en/of plaatsen waar invasieve monitoren niet beschikbaar zijn.
Ten opzichte van andere benaderingen is het belangrijkste voordeel van diffuse optica het vermogen om tegelijkertijd de bloedstroom en weefselbloedoxygenatie niet-invasief (en continu) aan het bed te meten met behulp van draagbare instrumentatie. In vergelijking met Transcraniële Doppler-echografie (TCD) heeft DCS een bijkomend voordeel: het meet perfusie op weefselniveau, terwijl TCD de cerebrale bloedstroomsnelheid meet in grote slagaders aan de basis van de hersenen. Dit onderscheid kan met name van belang zijn bij de evaluatie van steno-occlusieve ziekten waarbij zowel proximale grote slagaderstroom als leptomeningeale collateralen bijdragen aan perfusie. Optische technieken hebben ook voordelen in vergelijking met andere traditionele beeldvormingsmodaliteiten, zoals positronemissietomografie (PET) en magnetische resonantiebeeldvorming (MRI). Naast gelijktijdige directe metingen van zowel CBF- als HbO/HbR-concentraties, wat niet mogelijk is met MRI of PET alleen, biedt optische monitoring ook een significant betere temporele resolutie, waardoor bijvoorbeeld de beoordeling van dynamische cerebrale autoregulatie40,41,42 en de beoordeling dynamisch evoluerende hemodynamische veranderingen mogelijk zijn. Bovendien is diffuse optische instrumentatie goedkoop en draagbaar in vergelijking met PET en MRI, wat een cruciaal voordeel is gezien de hoge last van vaatziekten in landen met lagere en middeninkomens.
Het hier voorgestelde protocol is een omgeving voor real-time neuromonitoring aan het bed van patiënten op de intensive care (ICU). Het protocol maakt gebruik van een hybride optisch apparaat samen met een klinisch-vriendelijke grafische gebruikersinterface (GUI) en aangepaste optische sensoren om de patiënten te onderzoeken (figuur 1). Het hybride systeem dat wordt gebruikt voor de presentatie van dit protocol combineert twee diffuse optische spectroscopieën van onafhankelijke modules: een commerciële frequentiedomein (FD-) DOS-module en een zelfgemaakte DCS-module (figuur 1A). De FD-DOS module49,50 bestaat uit 4 fotomultiplicatorbuizen (PMT’s) en 32 laserdiodes die uitzenden op vier verschillende golflengten (690, 704, 750 en 850 nm). De DCS-module bestaat uit een lange-coherentie laser die uitzendt bij 785 nm, 16 single-photon tellers als detectoren en een correlatorkaart. De bemonsteringsfrequentie voor de FD-DOS-module is 10 Hz en de maximale bemonsteringsfrequentie voor de DCS-module is 3 Hz. Om de FD-DOS- en DCS-modules te integreren, werd in onze besturingssoftware een microcontroller geprogrammeerd om automatisch tussen elke module te schakelen. De microcontroller is verantwoordelijk voor het in- en uitschakelen van de FD-DOS- en DCS-lasers, evenals de FD-DOS-detectoren om interleaved-metingen van elke module mogelijk te maken. In totaal kan het voorgestelde systeem elke 0,5 tot 5s één gecombineerd FD-DOS- en DCS-monster verzamelen, afhankelijk van de signaal-ruisverhouding (SNR) -vereisten (langere verzameltijden leiden tot een betere SNR). Om het licht aan het voorhoofd te koppelen, ontwikkelden we een 3D-geprinte optische sonde die voor elke patiënt kan worden aangepast (figuur 1B), met brondetectorscheidingen variërend tussen 0,8 en 4,0 cm. De standaard bron-detectorscheidingen die in de hier gepresenteerde voorbeelden worden gebruikt, zijn 2,5 cm voor DCS en 1,5, 2,0, 2,5 en 3,0 cm voor FD-DOS.
Het belangrijkste kenmerk van het protocol dat in deze studie wordt gepresenteerd, is de ontwikkeling van een real-time interface die zowel de hardware met een vriendelijke GUI kan besturen als de belangrijkste cerebrale fysiologieparameters in realtime kan weergeven onder verschillende temporele vensters (figuur 1C). De real-time analysepijplijn die binnen de voorgestelde GUI is ontwikkeld, is snel en duurt minder dan 50 ms om de optische parameters te berekenen (zie het aanvullende materiaal voor meer informatie). De GUI is geïnspireerd op de huidige klinische instrumenten die al beschikbaar zijn op de neuro-ICU, en het werd aangepast door uitgebreide feedback van klinische gebruikers tijdens de vertaling van het systeem naar de neuro-ICU. Bijgevolg kan de real-time GUI de adoptie van het optische systeem door regulier ziekenhuispersoneel, zoals neuro-intensivisten en verpleegkundigen, vergemakkelijken. De brede acceptatie van diffuse optica als een klinisch onderzoeksinstrument heeft het potentieel om het vermogen om fysiologisch zinvolle gegevens te monitoren te verbeteren en kan uiteindelijk aantonen dat diffuse optica een goede optie is voor niet-invasieve monitoring van neurokritische patiënten in realtime.
Dit artikel presenteerde een hybride optisch systeem dat real-time informatie kan bieden over de cerebrale bloedstroom, cerebrale oxygenatie en cerebrale zuurstofmetabolisme van neurokritische patiënten aan de zijkant.
