This work demonstrates the feasibility of an in vivo phosphorus-31 magnetic resonance spectroscopy (31PMRS) technique to quantify mitochondrial oxidative phosphorylation (OXPHOS) capacity in human skeletal muscle.
Skeletal muscle mitochondrial oxidative phosphorylation (OXPHOS) capacity, which is critically important in health and disease, can be measured in vivo and noninvasively in humans via phosphorus-31 magnetic resonance spectroscopy (31PMRS). However, the approach has not been widely adopted in translational and clinical research, with variations in methodology and limited guidance from the literature. Increased optimization, standardization, and dissemination of methods for in vivo 31PMRS would facilitate the development of targeted therapies to improve OXPHOS capacity and could ultimately favorably impact cardiovascular health. 31PMRS produces a noninvasive, in vivo measure of OXPHOS capacity in human skeletal muscle, as opposed to alternative measures obtained from explanted and potentially altered mitochondria via muscle biopsy. It relies upon only modest additional instrumentation beyond what is already in place on magnetic resonance scanners available for clinical and translational research at most institutions. In this work, we outline a method to measure in vivo skeletal muscle OXPHOS. The technique is demonstrated using a 1.5 Tesla whole-body MR scanner equipped with the suitable hardware and software for 31PMRS, and we explain a simple and robust protocol for in-magnet resistive exercise to rapidly fatigue the quadriceps muscle. Reproducibility and feasibility are demonstrated in volunteers as well as subjects over a wide range of functional capacities.
Het doel van dit werk is om een reproduceerbare methode om niet-invasief te meten in vivo skeletspier mitochondriale functie in natuurlijke personen die een breed scala aan mogelijkheden te schetsen. Afwijkende mitochondriale impairment is een kenmerk van een breed scala van metabool syndroom en genetische ziekten, van gemeenschappelijke voorwaarden, zoals de vergrijzing en diabetes om zeldzame aandoeningen zoals ataxie van Friedreich.
Metabool syndroom en mitochondriale dysfunctie
Metabool syndroom is aangetoond dat mitochondriale functies verstoren, drukt skeletspier OXPHOS en ectopische lipide opslag in skeletspier 1, 2. Zo kritisch organellen regeling van het metabolisme en energie homeostase, worden mitochondriën betrokken bij de pathofysiologie van obesitas 3, 4, 5 insulineresistentie </sup>, Type 2 Diabetes Mellitus (T2DM) 6, 7, diabetes gerelateerde micro- 8, 9, 10, 11 en macrovasculaire complicaties 12, 13, en non-alcoholische fatty leverziekte (NAFLD) 14, 15, 16, onder anderen .Insulin weerstand wordt gekenmerkt door ingrijpende veranderingen in de skeletspier mitochondriale activiteit, met inbegrip van verminderde mitochondriale tricarbonzuur (TCA) flux rate, ATP-synthese rate, en citraat synthase en NADH: O 2 oxidoreductase activiteit 5. Een hypothese is dat deze veranderingen kan worden veroorzaakt door de accumulatie van vrije vetzuren (FFA) metabolieten in de spier, die aanzienlijk zijn verhoogd bij obesitas en andere obesitas-ropgetogen ziekten 2, 17. De blootstelling van spier FFAs verhoogde lipide en tussenproducten kunnen de expressie van genen in de lipide oxidatieve route en de TCA cyclus elektronen transportketen (ETC) 18 verlagen. Deze vermindering van mitochondriale skeletspier OXPHOS capaciteit in de setting van een lipide overbelasting gaat gepaard met een daling van de kwantitatieve (inhoud en biogenese van mitochondria) 19 en kwalitatieve functie van skeletspieren mitochondriën 20. Blootstellen skeletspier en myocyten aan FFA leidt tot ernstige insulineresistentie en verhoogde FFA-opname in spieren geassocieerd met insulineresistentie bij zowel mensen als knaagdieren 21. De lipide ceramide tussenproducten en diacylglycerol (DAG) is aangetoond dat de insuline-signaleringsroute remt rechtstreeks door het veranderen van de activiteit van kinasen, zoals proteïne kinase C en protein kinase B 21. Daarom lipide afgeleide moleculen lijken een belangrijke rol in de ontwikkeling van skeletspieren insulineresistentie en T2DM spelen. Het blijft echter onduidelijk of veranderingen in het mitochondriaal capaciteit zijn een oorzaak of een gevolg van insulineresistentie 22.
Friedrich Ataxie en mitochondriale dysfunctie
Verminderde OXPHOS kan ook het gevolg zijn van genetische defecten. Friedrich's ataxie (FA), de meest voorkomende vorm van erfelijke ataxie, een erfelijke aandoening veroorzaakt door een mutatie in het frataxin (FXN) gen, wat resulteert in intra-mitochondriale ijzeraccumulatie, productie van reactieve zuurstof en afwijkingen van oxidatieve fosforylering 23, 24, 25, 26. Deze belangrijke ontdekking heeft geleid tot de ontwikkeling van doelgerichte therapie, which doel om de mitochondriale functie in het sub-cellulair niveau te verbeteren. Ondanks dit inzicht is er beperkte ontwikkeling van de in vivo, reproduceerbare biomarkers voor FA klinisch onderzoek geweest. In feite een kritische barrière in de effectieve evaluatie van gerichte therapieën in FA is het onvermogen om veranderingen in mitochondriale functie volgen. Huidige functionele maatregelen bijvoorbeeld kunnen identificeren verminderde cardiale output; ze zijn echter niet in staat de bepaling van de hoogte waar de disfunctie optreedt (figuur 1). De ontwikkeling van een betrouwbare merker van mitochondriale functie die kan worden gebruikt voor het identificeren en evalueren ziekteprogressie in ataxie Friedrich cruciaal is voor de desbetreffende mechanistische effect van gerichte therapieën meten.
Verminderde OXPHOS en Cardiac Dysfunction
Afwijkende mitochondriale functie, ofwel verworven of genetische, kunnen bijdragen tot de ontwikkeling of progressie van Cardiac disfunctie. Onder de voorwaarden van de druk overbelasting en hartfalen, de primaire energie substraat voorkeur schakelt van FFA in glucose. Dit gaat gepaard met een verminderde activiteit ETC en oxidatieve fosforylering 27. De pathofysiologie van mitochondriale bio-energetica in cardiale dysfunctie kunnen verschillen, afhankelijk van de primaire oorsprong van het mitochondriale defect. Diabetes en het metabool syndroom leidt tot mitochondriale afwijkingen in de hartspier, zoals verminderde biogenese en vetzuur metabolisme, wat leidt tot een verminderde substraat flexibiliteit, energie-efficiëntie, en uiteindelijk, diastolische dysfunctie 28, 29. In FA, daarentegen, een frataxin deficiëntie resulteert in significante mitochondriale ijzer accumulatie in cardiomyocyten 30, 31. Ijzer accumulatie leidt tot de productie van vrije radicalen via de Fenton reactie 32 </ Sup> en verhoogt de kans van vrije radicalen geïnduceerde cardiomyocyt schade. Intra-mitochondriale ijzeraccumulatie wordt ook geassocieerd met een verhoogde gevoeligheid voor oxidatieve stress en een verminderde oxidatieve capaciteit 30, 31. Ijzerstapeling en de daaropvolgende afwijkende mitochondriale functie, als gevolg van frataxin tekort, kan daarom verantwoordelijk voor de verminderde cardiale energetica en cardiomyopathie waargenomen in FA 33, 34. Het is ook interessant om op te merken dat de verminderde oxidatieve capaciteit in de skeletspier mitochondria loopt parallel met de uitoefening intolerantie en verminderde metabole capaciteit bij hartfalen (HF) 35. Meting van skeletspieren OXPHOS capaciteit, zoals hierin beschreven, gemakkelijk uitvoerbaar en robuust; in combinatie met de betekenis van de skeletspier OXPHOS in HF, deze functies maken het een aantrekkelijke biomarker in uitgebreide studies van horent ziekte 36.
Verminderde OXPHOS en de bijbehorende cardiale disfunctie is niet een onbelangrijk aspect van metabole en mitochondriale ziekte. Patiënten met diabetes en metabole ziekten zijn op een hoger risico op het ontwikkelen van hart- en vaatziekten en hebben oversterfte na een myocardinfarct (MI) 37, 38, 39, 40, 41; meer dan de helft van de FA onderwerpen hebben cardiomyopathie, en velen sterven van hartritmestoornissen of hartfalen 42. Daarom kwantificering van verminderde OXPHOS kan niet alleen zorgen voor vroege opsporing en behandeling van cardiale dysfunctie, maar het kan ook vermijden kan een belangrijke klinische last bij deze ziekten.
Therapieën gericht OXPHOS capaciteit gaat plaatsvinden is een veelbelovend gebied voor de behandeling van patiënten verbeteren, whether de oorzaak van metabole stoornissen is genetische of verworven. Momenteel is de ontwikkeling van nieuwe gerichte geneesmiddelen die ofwel abnormale mitochondriale functie 43 verlichten of verbeteren van de primaire genetische defect 44 kan de bio-energetica gestoorde kenmerkend FA verbeteren. Bij verkregen mitochondriale dysfunctie kan verhoogde fysieke activiteit verbeteren mitochondriale functie 45, 46, 47.
31 Fosfor Magnetische Resonantie Spectroscopie als een niet-invasieve Biomarker van mitochondriale functie
Ongeacht het geteste therapie, een geïntegreerd in vivo bepaling van skeletspier bioenergetica is een belangrijk hulpmiddel om het effect van gerichte interventies te beoordelen, vooral bij patiënten met ernstige inspanningsintolerantie of het onvermogen van conventionele metabo ondergaanlic testen. Resonantiespectroscopie afgestemd op fosfor (31 PMRS), een endogene kern in diverse energierijke substraten in cellen in het lichaam, is gebruikt om mitochondriale oxidatieve capaciteit gekwantificeerd met verschillende methoden, met inbegrip van magneet oefening terminaal protocollen spierstimulatie protocollen 48. De uitoefening terminaal protocollen berusten op verschillende inrichting variërend in complexiteit van MRI-compatibele ergometers die regelen en meten werkdruk eenvoudige configuraties van riemen en pads waardoor burst-type ohmse en quasi-statische oefening. Een van de belangrijkste doelen van elke van deze protocollen is een energiebalans waarvoor de vraag naar adenosinetrifosfaat (ATP) aanvankelijk door de enzymatische afbraak van creatinefosfaat (PCR) wordt bereikt met creatine kinase reactiemengsel 49 te produceren. Na stopzetting van de oefening, is de snelheid van de ATP productie wordt gedomineerd door oxidatieve phosphorylation en is de maximale in vivo capaciteit van de mitochondriën 50. Bovendien kunnen OXPHOS tijdens de hersteltijd na het sporten worden beschreven door een eerste-orde snelheid reactie 51. De post-oefening herstel van de PCR kan derhalve worden gekwantificeerd door het aanbrengen van een exponentiële tijdconstante (τ PCR), met kleinere waarden van τ PCR vertegenwoordigen grotere capaciteit voor oxidatieve ATP synthese. Aanzienlijke inspanningen zijn gedaan om 31 DVS tegen ex vivo en meer directe metingen van OXPHOS valideren en demonstreren de mogelijke klinische toepasbaarheid van deze techniek 52, 53, 54, 55.
Met name kan de in dit werk beschreven protocol worden uitgevoerd op klinisch beschikbare scanners, en is op grote schaal gevalideerd als biomarker noninvasive of mitochondriale functie 56. Echter, een oefening 31 DVS protocol geoptimaliseerd voor toepassing op mensen met wisselende ernst van neuromusculaire stoornis of de mobiliteit is niet goed vastgesteld 57. Een goed gedefinieerde, breed toepasbare inspanningsprotocol en 31 DVS techniek zou bijzonder nuttig zijn bij de evaluatie van ziekten met afwijkingen in fundamentele mitochondriale functie.
Verschillende eerdere studies hebben de toepassingen van niet-invasieve technieken onderzocht om de mitochondriale functie te kwantificeren bij patiënten. Zo hebben deze technieken getoond verminderde OXPHOS bij patiënten met type 2 diabetes 36. Lodi et al. Eerst de haalbaarheid van DVS technieken getest bij patiënten met FA en vonden dat 1) de fundamentele genetisch defect in FA schaadt skeletspier OXPHOS en 2) het aantal GAA triplet herhaling omgekeerd evenredig skeletspier OXphos 33. Recenter Nachbauer et al. gebruikte PMRS als secundaire uitkomstmaat in een FA drug proef met 7 vakken. PCR hersteltijd significant langer bij patiënten in vergelijking met controles, versterkend eerdere werk Lodi en waaruit blijkt dat de effecten van afwijkende frataxin meningsuiting in FA kan resulteren in een daling van de mitochondriale capaciteit die kan worden opgespoord met behulp van DVS technieken 58.
Betrouwbare methoden adequaat definiëren vivo skeletspierfunctie op een haalbare, kosteneffectieve en reproduceerbaar zijn cruciaal voor verbetering voorwerp resultaten bij een reeks van ziekten die mitochondriale functie beïnvloeden.
Dit werk schetst een deugdelijke procedure voor het verkrijgen van in vivo maximale oxidatieve capaciteit van de skeletspieren met behulp van 31 DVS. De in-magneet oefening protocol wordt goed verdragen door mensen verspreid over een breed scala aan fysieke en functionaliteitenl vaardigheden en biedt een vereenvoudigde onderwerp setup met behulp van goedkope en alom beschikbare apparatuur.
Dit artikel beschrijft een standaardprotocol voor 31 DVS onderzoek dat seriële en invasieve in vivo meting van skeletspieren mitochondriale functie verschaft. Het protocol houdt grote aantrekkingskracht bij het overwegen van de breedte van onderzoeken gericht op de groeiende last van metabool syndroom en de resulterende morbiditeit en mortaliteit.
