Kaposi sarcoom (KS) is een tumor die is veroorzaakt door infectie met de oncogene virus Humaan herpesvirus-8/KS herpesvirus (HHV-8/KSHV). De endothelial cell cultuur model hier beschreven is uniek geschikt voor de studie van de mechanismen waarmee KSHV gastheer cellen transformeert.
Kaposi sarcoom (KS) is een ongewone tumor bestaat uit delende cellen van de spindel dat is geïnitieerd door infectie van endotheliale cellen (EG) met KSHV, en meestal ontwikkelt in de instelling bij immuunsuppressie. Ondanks decennia van onderzoek, optimale behandeling van KS blijft slecht gedefinieerde en klinische resultaten zijn vooral ongunstige in resource-beperkte instellingen. KS laesies worden gedreven door pathologische angiogenese, chronische ontsteking en oncogenese en verschillende in vitro cel cultuur modellen zijn ontwikkeld om deze processen te bestuderen. KS vloeit voort uit KSHV-geïnfecteerde cellen van de oorsprong van het endotheel, dus EG-afstamming-cellen zorgen voor de meest geschikte in vitro surrogaten van de voorloper van de spindel cel. Echter, omdat EG hebben een levensduur beperkt in vitro , en zoals de oncogene mechanismen werkzaam bij KSHV zijn minder efficiënt zijn dan die van andere tumorigene virussen, is moeilijk te beoordelen van de processen van transformatie in primaire of telomerase-vereeuwigd EC. Daarom werd een nieuwe EG gebaseerde cultuur model ontwikkeld dat ondersteunt transformatie na infectie met KSHV. Ectopische uitdrukking van de genen van de E6 en E7 van humaan papillomavirus type 16 zorgt voor uitgebreide cultuur van leeftijd – en passage-matched mock – en KSHV-besmette EG en ondersteunt de ontwikkeling van een echt getransformeerd (dat wil zeggen, tumorigene) fenotype in geïnfecteerde celculturen . Dit hanteerbare en zeer reproduceerbaar model van KS heeft de ontdekking van verschillende essentiële signaalroutes met een hoog potentieel voor vertaling in klinische instellingen vergemakkelijkt.
Kaposi sarcoom (KS) is een multi focale angioproliferative tumor beïnvloeden dermale, cutane en viscerale sites die meestal in het kader van geavanceerde immuun onderdrukking1 ontwikkelt. Vier epidemiologische vormen zijn beschreven: classic, een vadsig formulier buikvliesontsteking meestal oudere mensen van mediterrane en Midden-Oosten erfgoed; iatrogene, als gevolg van behandeling met Immunosuppressieve medicijnen na orgaantransplantatie; epidemie, een AIDS-definiëren kanker; en endemisch, een HIV-zelfstandige vorm vaak voor bij kinderen in endemische gebieden in Afrika. Met de komst van effectieve combinatie regimes van de anti-retrovirale geneesmiddelen voor de behandeling van HIV, is epidemische KS veel minder vaak gediagnosticeerd in ontwikkelingslanden. De klinisch agressieve endemische en epidemische vormen blijven echter onder het meest gediagnosticeerde kankers in vele Afrikaanse landen2,–3,4. Daarom is de identificatie van effectieve pathogenese-gerichte medicijnen voor behandeling van KS een onderzoeksprioriteit.
Histologisch, worden KS laesies gekenmerkt door uitgebreide maar abnormale neovascularization waarbij spindel cellen van EG oorsprong discontinue vasculaire netwerken5vormen. Deze abnormale schepen (“vasculaire spleten”) toestaan extravasation van erytrocyten, waardoor laesies hun karakteristieke kleur. Daarnaast bevatten laesies talrijke leukocyten, die kenmerkend zijn voor chronische ontsteking (dat wil zeggen, lymfocyten, macrofagen en plasmacellen). Regressie van KS letsels na immuunsysteem reconstitutie werd genoemd, wat suggereert dat KS kenmerken van zowel een hyper-proliferatieve laesie en een ware tumor6,7,8,9 heeft.
KS herpesvirus (KSHV), de verwekker van KS, werd in 199410geïdentificeerd. Sinds dan veel in vitro cultuur van de cel modellen zijn ontwikkeld zodat de pathogenese studies, met inbegrip van cellen transplanteren van tumor biopsie materiaal en primaire of uiting van telomerase EG geïnfecteerd met KSHV in vitro11 , 12 , 13 , 14 , 15 , 16 , 17 , 18. geen van de momenteel beschikbare modellen volledig recapituleert de KS tumor communicatie, maar alle waardevolle kennis aan ons begrip van de pathobiology van KSHV infectie hebben bijgedragen. In tegenstelling tot de andere bekende tumorigene Humaan herpesvirus Epstein – Barr virus (EBV), wordt het KSHV niet gemakkelijk transformeren bij cellen in cultuur na DOVO infectie19,20,21, 22. echter deze beperking heeft overwonnen door transducing primaire menselijke EG van hetzij microvasculaire of lymfatische oorsprong gemengd met de E6 en E7 genen van humaan papillomavirus typen 16 voorafgaand aan infectie met KSHV23,24 . Uitdrukking van deze exogene oncogenen verhoogt drastisch het transformerende potentieel van KSHV in vitro gedeeltelijk door de toekenning van aanvullende remming van de Retinoblastoom eiwitten en p5323,24. Deze EG transductie methode heeft toegestaan meerdere laboratoria vast te stellen van belangrijke wijzigingen in ontvangende cel de expressie van genen die worden veroorzaakt door infectie van de KSHV en die lijken te vergemakkelijken van KS cel overleving en proliferatie25,26 , 27 , 28 , 29 , 30 , 31 , 32. de hierin beschreven protocollen zijn eenvoudig en zeer reproduceerbaar, en zal resulteren in het genereren van leeftijd – en passage-matched KSHV-geïnfecteerde EG en mock-geïnfecteerde besturingselementen die kunnen worden gekweekt voor veel langer dan primaire cellen en zal zorgen voor het onderzoek van oncogene mechanismen werkzaam bij KSHV. Hoewel het protocol bevat een methode voor de productie van wild type KSHV uit de primaire effusie lymfoom cellijn BCBL-1, E6/E7-vereeuwigd EG zijn ook zeer gevoelig voor infectie met recombinant BACmid afgeleid KSHV-BAC1630. Protocollen voor bereiding van BAC16 worden elders beschreven33,34.
Oncogenese is een meerstaps proces dat belangrijke waarborgen binnen een organisme36omzeilt. Zoals KS laesies bestaan langs een spectrum van chronische ontsteking aan ware sarcomen, vereist opheldering van bepaalde pathofysiologische processen gemedieerd door KSHV dat sommige studies worden uitgevoerd in cel cultuur modellen die ondersteuning bieden voor transformatie9.
