The retinal pigment epithelium (RPE) supports the sensory retina through recycling visual cycle byproducts, which accumulate as lipofuscin. These products are autofluorescent and can be qualitatively imaged in vivo. Here, we describe a method to quantitatively image RPE lipofuscin using confocal scanning laser ophthalmoscopy.
The retinal pigment epithelium (RPE) is juxtaposed to the overlying sensory retina, and supports the function of the visual system. Among the tasks performed by the RPE are phagocytosis and processing of outer photoreceptor segments through lysosome-derived organelles. These degradation products, stored and referred to as lipofuscin granules, are composed partially of bisretinoids, which have broad fluorescence absorption and emission spectra that can be detected clinically as fundus autofluorescence with confocal scanning laser ophthalmoscopy (cSLO). Lipofuscin accumulation is associated with increasing age, but is also found in various patterns in both acquired and inherited degenerative diseases of the retina. Thus, studying its pattern of accumulation and correlating such patterns with changes in the overlying sensory retina are essential to understanding the pathophysiology and progression of retinal disease. Here, we describe a technique employed by our lab and others that uses cSLO in order to quantify the level of RPE lipofuscin in both healthy and diseased eyes.
Het retinale pigmentepitheel (RPE) ondersteunt de functie van de sensorische retina door talrijke werkwijzen 1. Leeftijdsgebonden maculadegeneratie (AMD) is de belangrijkste oorzaak van onbehandelbare blindheid in geïndustrialiseerde landen en wordt gekenmerkt door veranderingen in het RPE, waaronder verlies van pigment, functieverlies en atrofie. AMD en normale veroudering, de RPE accumuleert fluorescent, lysosoom afgeleide organellen die gefagocyteerde fotoreceptor fragmenten, aangeduid als lipofuscine granules. De accumulatie van lipofuscine RPE is gedacht oxidatieve dysfunctie 1 geven, maar recente studies hebben aangetoond dat de RPE morfologie blijft normaal verouderde ogen met hoge niveaus lipofuscine 2. Echter, afwijkende patronen van lipofuscine distributie, in het bijzonder het verlies van lipofuscine, zijn gedocumenteerd markers voor AMD en AMD progressie, zowel histologisch en klinisch 3,4
defecte proceszing van RPE lipofuscine is ook aangetoond voor bij bepaalde erfelijke retinale degeneratie. Patiënten die lijden aan de ziekte van Stargardt (STGD) accumuleren lipofuscin in de RPE op jonge leeftijd, uiteindelijk het ontwikkelen van visie verlies vergelijkbaar met dat in AMD 5. Deze bevindingen suggereerden dat lipofuscine accumulatie zelf kan giftig zijn en rijden RPE dysfunctie 6,7. Echter, een gedetailleerde imaging studie bij patiënten met STGD na verloop van tijd niet bevestigen dat focale lipofuscine accumulatie geleid tot latere RPE verlies 8. Daarom hoewel lipofuscine afwijkingen markers voor retinale degeneratie, een rol voor directe toxiciteit van lipofuscine blijft onbewezen.
De RPE is de meest achterste cellaag van het netvlies, maar genereert de meeste fluorescente signaal van de oculaire fundus. Opwekking en detectie van autofluorescentie (AF) afgeleid van het RPE kan worden uitgevoerd met behulp van confocale scanning laser Ophthalmoscopie (cSLO), die zorgt voor visualization van de ruimtelijke verdeling van fundus AF. Bepaalde retinale degeneratie tonen onderscheidende patronen van fundus AF en AF imaging helpt bij de diagnose en monitoring van deze voorwaarden. Hoewel de standaard AF beeldvorming is klinisch belangrijk, is kwantitatieve AF (Qaf) uitgegroeid tot een belangrijk middel voor het beoordelen van gezondheidsrisico RPE. Wij en anderen hebben een gestandaardiseerde aanpak die betrouwbaar QAF niveaus kan bepalen op specifieke netvlies locaties 9 ontwikkeld. QAF heeft potentiële toepassingen in de diagnose en monitoring van retinale aandoeningen, en kunnen ook nut in prognose en risicostratificatie. Bovendien zijn de diagnostische mogelijkheden van qaf eveneens beschreven voor bepaalde retinale aandoeningen 10-12. Hier geven we stapsgewijze informatie voor het uitvoeren van onze techniek vergezeld van een visuele demonstratie van de toepassing bij de beoordeling van gezonde en zieke ogen.
Abnormale RPE lipofuscine distributie, of verhoogd of verlaagd, is een gevoelige marker van retinale ziekte en wordt meestal geassocieerd met het verlies van sensorische retina-functie. Hier beschrijven we de toepassing van qaf voor de evaluatie van RPE lipofuscine. Opname van een interne fluorescerende referentie voor het corrigeren van variabele laservermogen en detectorsensitiviteit 9 naast onze standaard beeldtechniek maakt betrouwbare kwantificering van AF niveaus.
