Ovarian cancer metastasis is characterized by numerous diffuse intra-peritoneal lesions, such that accurate visual quantitation of tumor burden is challenging. Herein we describe a method for in situ and ex vivo quantitation of metastatic tumor burden using red fluorescent protein (RFP)-labeled tumor cells and optical imaging.
Epithelial ovarian cancer (EOC) is the leading cause of death from gynecologic malignancy in the United States. Mortality is due to diagnosis of 75% of women with late stage disease, when metastasis is already present. EOC is characterized by diffuse and widely disseminated intra-peritoneal metastasis. Cells shed from the primary tumor anchor in the mesothelium that lines the peritoneal cavity as well as in the omentum, resulting in multi-focal metastasis, often in the presence of peritoneal ascites. Efforts in our laboratory are directed at a more detailed understanding of factors that regulate EOC metastatic success. However, quantifying metastatic tumor burden represents a significant technical challenge due to the large number, small size and broad distribution of lesions throughout the peritoneum. Herein we describe a method for analysis of EOC metastasis using cells labeled with red fluorescent protein (RFP) coupled with in vivo multispectral imaging. Following intra-peritoneal injection of RFP-labelled tumor cells, mice are imaged weekly until time of sacrifice. At this time, the peritoneal cavity is surgically exposed and organs are imaged in situ. Dissected organs are then placed on a labeled transparent template and imaged ex vivo. Removal of tissue auto-fluorescence during image processing using multispectral unmixing enables accurate quantitation of relative tumor burden. This method has utility in a variety of applications including therapeutic studies to evaluate compounds that may inhibit metastasis and thereby improve overall survival.
Epitheliale eierstokkanker (EOC) is de meest voorkomende doodsoorzaak van gynaecologische maligniteit, met een geschatte 21.290 nieuwe diagnoses in de VS in 2015 en naar schatting 14.180 doden 1. De overgrote meerderheid (> 75%) van de vrouwen gediagnosticeerd met late stadia van de ziekte (stadium III of IV) gekenmerkt door diffuse intraperitoneale metastase en slechte prognose. Terugkeer van de ziekte in de buikholte na eerstelijns chemotherapie is ook gebruikelijk en vormt een belangrijke oorzaak van sterfte 2,3. EOC uitzaaiing door een uniek mechanisme waarbij zowel rechtstreeks in het verlengde van de primaire tumor naburige peritoneale organen en door dissociatie of afstoten van cellen uit de primaire tumor oppervlak als enkele cellen of meercellige aggregaten. De cellen worden afgeworpen in de buikholte, waarin ze zich verzetten tegen-detachement geïnduceerde apoptose 4. Ophoping van peritoneale ascites komt vaak voor, zoals schuur tumorcellen blokkeren peritoneale lymfedrainage en tumors produceren groeifactoren die vasculaire permeabiliteit te wijzigen. Een gedeelte van loods tumorcellen hechten aan het oppervlak van peritoneale organen en structuren zoals darmen, lever, omentum en mesenterium, waarna ze verankeren en prolifereren meerdere wijd verspreide secundaire laesies 3,5 produceren. Hematogene metastase is ongewoon. Zo klinische behandeling bestaat gewoonlijk van cytoreductieve chirurgie met inbegrip van "optimale debulking", gedefinieerd als resectie van alle zichtbare tumor (hoe klein ook). Volledige cytoreductie gaat gepaard met een aanzienlijke toename van de totale overleving 6,7 en wordt geassocieerd met de uitdaging van identificatie en verwijdering van laesies <0,5 cm.
Kleine diermodellen hebben bewezen nut in eierstokkanker onderzoek in het verbeteren van ons begrip van de progressie van de ziekte en de identificatie van prognostische biomarkers en het testen van nieuwe chemotherapieën of combinatietherapie benaderingen. Als de primaireplaats van eierstokkanker incidentie en metastase is de peritoneale holte orthotopische modellen van EOC metastase omvatten de analyse en karakterisering van intraperitoneale ziekte. Hoewel recente verbeteringen in de mogelijkheid om beelden op tumorcellen hebben, zelfs op de enkele cel niveau, bestaan er nog steeds grote moeilijkheden bij het kwantificeren van de metastatische tumorlast van EOC. Deze problemen resulteren uit het aantal, grootte en anatomische locatie van metastatische laesies. Verder bestaat er behoefte aan label kankercellen te onderscheiden van normale gastheercellen. Eerdere studies hebben antilichamen gebaseerde labeling protocollen of transfectie van tumorcellen met luciferase 8,9 gebruikt. Direct fluorescent labeling van kankercellen werd eerst beschreven door Chishima en medewerkers in 1997 10. Fluorescente labels geen toevoeging van exogeen substraat vragen en tevens uitstekende tumorcel specificiteit, een meer effectieve manier om kanker metastase 11,12 bijhouden </sup>.
Hierin beschrijven we een optische imaging methode voor de kwantitatieve analyse van metastatische ziekte met behulp van een syngene orthotope xenograft model bestaat uit rood fluorescerend eiwit (RFP) gemerkte muizen ID8 eierstokkanker cellen 13 en immuno-competente C57 / Bl6 muizen. We tonen een nieuwe werkwijze van relatieve kwantificatie tumorlast combineren in vivo en ex vivo beeldvorming onder verwijdering van weefsel auto-fluorescentie. Deze aanpak heeft potentieel nut bij onderzoeken om het effect van specifieke genetische, epigenetische of micro-omgeving aan te passen en / of behandelingen op orgaanspecifieke metastase van eierstokkanker evalueren.
In tegenstelling tot onderzoek met humaan ovarium kankercellen die in immuungecompromitteerde muizen moeten worden uitgevoerd, de hierboven beschreven protocol gebruikt immuuncompetente C57 / Bl6 muizen en syngene muizen eierstokkankercellen. Hoewel dit maakt evaluatie van de mogelijke rol van immune infiltraten bij tumorprogressie en metastase, de aanwezigheid van donker haar op het buikoppervlak maakt beeldvorming minder gevoelig.
